Introdução: Ao final do ano de 2019, o mundo foi marcado pelo surgimento de uma misteriosa pneumonia causada por uma variação do coronavírus (SARS-CoV-2), que foi inicialmente detectado em Wuhan, na China. Por conseguinte, os serviços de saúde foram afetados em todo o mundo, sendo necessária a adaptação deles para que as atividades conseguissem se manter em níveis adequados, o que inclui a mudança no protocolo de captação e doação de órgãos em diversas Organizações de Captação de Órgãos e Sociedades de Transplante. Objetivo: Delimitar como um novo protocolo de captação de órgãos influencia, em dados quantitativos, o Hospital Municipal Souza Aguiar, um hospital público de referência no município do Rio de Janeiro e também na América Latina. Metodologia: Foi feita uma busca ativa nas planilhas do centro cirúrgico do hospital, no período de 2014 a 2020, e um levantamento de dados quantitativos e qualitativos no PET (Programa Estadual de Transplante) acerca das captações realizadas. Resultado: Observou-se que, entre o período de 2015 a 2019, houve uma média de dez captações/ano, enquanto que em 2020 esse valor correspondeu à metade dos casos, sendo de apenas cinco captações. Discussão: A disparidade de captações pode ser justificada pela adaptação do protocolo de captação de órgãos e pelo manejo inadequado de potenciais doadores em leitos de terapia intensiva, uma vez que o número de leitos ocupados por pacientes com a COVID-19 disparou nos primeiros meses da pandemia. Além disso, foi observada uma ascensão no número de pessoas na fila de espera para transplante de órgãos a partir do ano de 2018.
A Síndrome de Charles Bonnet (SCB) é uma condição na qual há ocorrência de alucinações visuais em indivíduos que apresentam alguma patologia ao longo das vias visuais. Estes pacientes geralmente mantém cognição intacta e insight da irrealidade das alucinações. O tratamento é baseado em abordagem multiprofissional com reforço ao paciente quanto à benignidade das alucinações, porém quando estes métodos falham, a terapia medicamentosa tem se mostrado pouco promissora. Background and Aims
Liver abnormalities are frequent in COVID-19 disease, AST and ALT abnormalities are present in about 60% of serious disease patients. However, liver insufficiency and liver mortality were not important concerns. Decompensate cirrhotic patients are a group of high risk for morbidity and mortality. Consequently, we aimed to study cirrhotic patients with at least one complication: ascites, encephalopathy or esophageal varices; to investigate mortality, transplantation and hospitalization due to SARS-Covid-19 infection pandemic.
Methods
Liver unit patients were enrolled after ethical approval and signed consentiment term. Combined outcomes during pandemic were analyzed. Participants were submitted to SARS-Cov 2 test by PCR oro/pharingeal swab. Call phone and medical records were consulted for covid 19 symptoms and outcomes. Survival, transplantation and clinical complications were studied.
Results
Fourthy seven patients were enrolled, 26 followed. Men was 73% of patients and median age was 62,7 years. The cirrhosis etiology in 35% was MAFLD, 32% alcohol, 15% HCV and 18% others. Frequence of COVID-19 infection was 42%, at last 10 months, and three (11%) patients died. Liver-related complications with death were present in 19% of patients without COVID-19 infection. Five patients (19%) were submitted to liver transplantation, without COVID-19 disease.
Conclusion
Although an incipient analyzes, our data show high death rate of cirrhotic decompensate patients during COVID-19 pandemic. This population needs a specific approach in order to prevent Covid-19 infection, liver-related mortality and complications during pandemic.
