Introducción. El dolor pélvico crónico (DPC) tiene gran impacto en la calidad de vida, tanto por su clínica dolorosa como por los costes que implica. Establecer el diagnóstico etiológico y manejo de estas pacientes es un desafío para el clínico. Una posible causa es el síndrome de congestión pélvica (SCP), una patología con mucho por descubrir, y que se fundamenta en la alteración de la vasculatura venosa pélvica provocando: dolor pélvico crónico de duración mayor a 6 meses, exacerbado por bipedestación y relaciones sexuales, en mujeres con único hallazgo de varicosidades periováricas en estudios de imagen.Objetivos. Esta revisión busca entender los mecanismos fisiopatológicos, manifestaciones clínicas y abordaje adecuado de esta entidad, de modo que el lector logre comprender la enfermedad, las posibilidades diagnósticas y terapéuticas que puede ofrecer a estas pacientes de acuerdo con la evidencia disponible.
