OBJECTIVE: To estimate the frequency of preschool children that have had a dental visit and to identify factors associated. MATERIAL AND METHODS: Children aged two to five years, enrolled in preschools were included (n=200). Parents/guardians answered a questionnaire and oral exams were performed using the Caries Assessment Spectrum and Treatment (CAST). The outcome was having consulted a dentist in one’s lifetime and Andersen’s behavioral model was adopted to assess independent variables associated, including predisposing factors (sex, age, maternal education), enabling factors (family income), need factors (dental caries severity, dental pain, oral health-related quality of life and oral hygiene habits), and psychosocial factors (dental fear) were evaluated. Associations were tested with Poisson regression, obtaining the prevalence ratio (PR) and 95% confidence interval (CI). RESULTS: Most of the children had never visited a dentist (71%; n=141), 15% (30) visited for prevention and 14% (29) visited due to oral health problems. Predisposing and enabling factors were not associated with having visited a dentist. Need factors, including severe dental caries (PR=3.11;95% CI=1.85-5.20), dental pain (PR=2.04;95% CI=1.32-2.17), toothbrush performed by parents (PR=1.99; 95% CI=1.05-3.77) and presence of impact on OHRQoL (PR=1.87; 95%CI=1.21-2.89), and absence of dental fear (psychosocial factor) were also associated with dental service use (PR=2.28; 95% CI=1.45-3.59). CONCLUSIONS: Most preschool children had never visited a dentist. Need and psychosocial factors were associated with dental services use. CLINICAL RELEVANCE: Most of the preschool children have never visited a dentist and the presence of signs and symptoms and the perception of dental problems influenced search for dental treatment.
A maloclusão é definida como uma irregularidade dos dentes, ocupando a terceira maior prevalência entre as patologias orais. Objetivo: Avaliar a prevalência de maloclusão e a necessidade de tratamento ortodôntico em escolares de 8 a 12 anos do município de Pelotas (RS), relacionando-as com dados demográficos e socioeconômicos. Material e métodos: Uma amostra de 1206 crianças de 20 escolas da rede pública e privada de Pelotas (RS) foi avaliada. Foi aplicado questionário aos pais para obtenção de dados demográficos e socioeconômicos. Dentistas treinados avaliaram maloclusão e necessidade de tratamento ortodôntico, pelo critério Dental Aesthetic Index. Para análise dos dados, os testes qui[1]quadrado, qui-quadrado de tendência linear e regressão de Poisson bruta e ajustada foram utilizados para associações entre desfecho e exposições, estimando-se as razões de prevalência e seus intervalos de confiança de 95%. Resultados: 83,7% apresentavam algum tipo de maloclusão, porém a necessidade de tratamento ortodôntico foi de 40,6% (mandatório em 9,7%). Espaçamento no segmento anterior (50,4%) e apinhamento (35,9%) foram as maloclusões mais prevalentes. Observou-se diminuição com a idade e da proporção de crianças com algum tipo de maloclusão. A análise ajustada mostrou que apenas a idade, e não características socioeconômicas, influenciou na necessidade de tratamento ortodôntico. Conclusão: A prevalência de maloclusão foi alta e, apesar de a necessidade de tratamento ortodôntico ter diminuído dos 8 aos 12 anos, continua elevada. Os resultados sugerem que o papel de medidas preventivas e interceptativas deva ser reforçado, especialmente na rede pública, como maneira efetiva de reduzir a necessidade de tratamento ortodôntico.
O SARS-CoV-2 pode ter provocado um aumento dos casos da Síndrome Inflamatória Multissistêmica Pediátrica (SIM-P), causando choque e falência de órgãos. O objetivo do artigo foi elucidar o que avaliar para diagnosticar a SIM-P associada í COVID-19. Método: Revisão bibliográfica na plataforma Evidence Aid e Pubmed com os descritores: pediatric multisystem inflammatory syndrome e COVID-19. Encontrou-se 89 artigos, selecionando 26 após triagem. Adicionou-se, manualmente, 2 boletins epidemiológicos e 1 postagem da Sociedade de Pediatria de São Paulo. Resultados: Os aspectos epidemiológicos, a presença de febre, rash cutâneo e alterações gastrointestinais e cardíacas, ajudam a caracterizar a síndrome. Nos exames laboratoriais são encontrados aumento de: proteína C-reativa, D-dímero, procalcitonina, ferritina sérica, fibrinogênio e marcadores cardíacos. Conclusão: A identificação das alterações é crucial para o diagnóstico diferencial e correto da SIM-P. Ademais, a padronização da coleta de dados e a identificação e descrição dos sinais e sintomas cooperaria para o diagnóstico.
