Resumo Este artigo objetiva analisar a influência dos aspectos macro e micropolíticos na organização e processo de trabalho do NASF. Trata-se de uma scoping review, realizada durante o mês de junho de 2020, nas bases de dados Lilacs e Medline. Identificou-se que questões referentes à infraestrutura, gestão do trabalho do NASF, formação para o SUS, relação entre as equipes e características dos profissionais do NASF influenciam a organização e o processo de trabalho do NASF, evidenciando-se a ocorrência de um maior número de desafios, em detrimento das potencialidades para esse processo. Destaca-se que atravessamentos marcados pela conjuntura atual podem refletir incisivamente no trabalho dessas equipes. Aponta-se a necessidade da implantação de estratégias no sentido de dirimir os obstáculos desvelados por este estudo, tais como a criação de espaços de educação permanente no cotidiano das equipes, o estabelecimento de ferramentas de cogestão, e o incentivo a iniciativas que fomentem a integração ensino-serviço-comunidade.
Government in the Unified Health System (SUS) is admittedly complex, requiring management teams to understand and develop a set of skills and competencies that help into their routines and in the challenges of public administration. This article presents a continuing education process developed with leadership teams from two health regions in Paraná, with the aim of broadening the participants’ view of the reorganization of the management and implementation of the SUS in the territory, based on interdisciplinarity and interprofessional. This is an experience report developed with teams, carried out in 2019, presented in five sections: the first describes the context in which the experience was developed, elucidating reasons, actors, and where it took place; the second section presents the work proposal, the preparation of the workshops, and the methodological resources used; the third reports the pathway travelled in its operation; the fourth discussions the characteristics of interdisciplinarity that exists in the continuing education process; and the last section presents some considerations. The proposal presents a powerful strategy to promote dialogue between different actors and institutions, and especially in the context of the State’s sanitary and fiscal crisis, to qualify and make reflective the work in the field of health management.
RESUMO A gestão no Sistema Único de Saúde (SUS) requer das equipes gestoras a compreensão e o desenvolvimento de um conjunto de habilidades e competências que os auxiliem no cotidiano e nos desafios da gestão pública. Este artigo apresenta um processo de Educação Permanente em Saúde (EPS) desenvolvido com equipes gestoras de duas regiões de saúde do Paraná, com intuito de ampliar o olhar dos participantes para a reorganização da gestão e da implementação do SUS no território, pautado na interdisciplinaridade. Trata-se de um relato de experiência desenvolvido com equipes gestoras, realizada no ano de 2019, apresentado em cinco seções: a primeira descreve o contexto em que a experiência foi desenvolvida, elucidando motivos, atores e a localidade em que ocorreu; a segunda apresenta a proposta de trabalho, a confecção das oficinas e os recursos metodológicos utilizados; a terceira relata o caminho percorrido na sua operacionalização; a quarta discute as características de interdisciplinaridade existentes no processo de EPS e a última apresenta algumas considerações. A proposta mostra-se como uma potente estratégia para promover o diálogo entre diferentes atores e instituições, principalmente no contexto de crise sanitária e fiscal do Estado, qualificar e tornar reflexivo o trabalho no campo da gestão em saúde.
Resumo Com o objetivo de analisar as repercussões do contexto sociopolítico e normativo para a organização do trabalho na Atenção Básica, ao se considerarem a composição, as formas de provimento dos trabalhadores e a percepção de gestores sobre o desempenho conforme o tipo de vínculo, realizou-se um estudo qualitativo em 16 municípios da macrorregião Norte do Paraná. Foram entrevistados coordenadores e secretários de saúde de julho a outubro de 2021. Os achados foram apresentados em três categorias: organização da Atenção Básica; formas de contratação dos profissionais; e percepção dos gestores sobre o desempenho dos trabalhadores conforme o vínculo de trabalho. Há preponderância da Estratégia Saúde da Família na organização do trabalho nos 16 municípios estudados, nos quais se verificou a existência de equipes de Atenção Primária em apenas três. Sobre as formas de contratação, houve aumento de vínculos precários. Quanto à percepção dos gestores a respeito do desempenho relacionado ao tipo de vínculo, fica evidente o ideário de que profissionais terceirizados apresentam maior dedicação quando comparados àqueles que contam com estabilidade. O contexto sociopolítico e normativo interferiu na organização e na forma de provimento dos profissionais, assim como na percepção dos gestores em relação ao desempenho e vínculo de trabalho.
Primary Health Care is a strategic level of care for forming a sustainable health system that responds to diverse needs. This article analyzed federal regulation and its implications for establishing primary care teams in Brazil. Exploratory mixed methods research involves analyzing 25 federal ordinances and secondary data of national scope referring to the teams approved in the National Register of Health Establishments from 2017 to 2021. The results indicate changes in the direction of the policy regarding the configuration, funding, and accreditation of teams. There was an expansion of primary care teams, a reduction in community health agents, and a weakening of the Family Health Support Center. The results suggest that the incentives for other team arrangements, the flexibility of the coverage of the community agent, and the multidisciplinary action compromise the sustainability of the Family Health model in the Unified Health System.
