Objetivo: Analisar a literatura buscando compreender melhor a presença de arritmias cardíacas na Síndrome Pós-Covid. Revisão bibliográfica: A prevalência de alterações eletrocardiográficas em pacientes portadores da Covid Longa vem se mostrando elevada. Sendo que a alteração mais comumente encontrada foi a taquicardia sinusal, que parece se apresentar como duas síndromes de mecanismos semelhantes, que são a Síndrome de Taquicardia Postural Ortostática e a Síndrome de Taquicardia Sinusal Inapropriada. A fisiopatologia parece ser o dano causado ao Sistema Nervoso Autônomo, através de uma ativação autoimune que ocorre após a infecção viral. Além disso, outros eventos arrítmicos foram encontrados nesses pacientes, como o desenvolvimento de arritmias atriais e ventriculares associadas a miocardite viral. Considerações finais: Apesar da elevada prevalência e carga sobre a população acometida, existe uma escassez de evidências científicas que buscam compreender os mecanismos arritmogênicos causados pelo SARS-CoV-2. Além disso, é de extrema importância que sejam estabelecidos protocolos que permitam rastreio otimizado desses pacientes, o mais cedo possível.
Objetivo: Analisar as características da Síndrome de Cushing (SC). Revisão bibliográfica: A SC é ocasionada pela exposição prolongada a altos níveis circulantes de cortisol. A maioria dos casos de SC é mediada exogenamente, através da administração de glicocorticóides. A SC endógena é muito mais rara e apresenta incidência anual de 3 casos por 1.000.000 de indivíduos. A SC endógena ocorre por mecanismos dependentes ou independentes do hormônio adrenocorticotrofina (ACTH). As características comuns SC incluem depósito de gordura centrípeta, estrias abdominais, pletora facial, atrofia muscular, perda de densidade óssea, imunossupressão e complicações cardiovasculares. Considerações finais: A SC constitui um grave problema de saúde com complicações musculoesqueléticas, cardiovasculares e neuropsiquiátricas. Seu diagnóstico é feito através da suspeição clínica e dos testes de triagem: teste de supressão de 1 mg de dexametasona, cortisol salivar noturno ou alto cortisol urinário livre. A terapêutica de escolha para SC é a ressecção por meio cirúrgico da lesão primária que levou ao excesso do cortisol. Além disso, opções farmacológicas podem ser utilizadas em casos nos quais há contraindicação cirúrgica ou de forma adjuvante ao método operatório.
Objetivo: Analisar as características da Distrofia Muscular de Duchenne (DMD). Revisão bibliográfica: A DMD é a distrofia muscular infantil mais comum em todo o mundo, sendo uma doença recessiva ligada ao X, causada por mutações no gene que codifica a distrofina, afetando 1 em 3.500 a 5.000 meninos. A distrofina é uma grande proteína do citoesqueleto que facilita as interações entre o citoesqueleto, a membrana celular e a matriz extracelular. Na DMD, a distrofina está ausente o que ocasiona fragilidade e permeabilidade excessivas da membrana, desregulação da homeostase do cálcio e dano oxidativo. Os sintomas se manifestam durante a primeira infância, piorando progressivamente com perda da deambulação, e, posteriormente, insuficiência respiratória e cardíaca. Considerações finais: A DMD é uma doença neuromuscular em que há fraqueza muscular severa e a morte se da aproximadamente aos 30 anos em decorrência da insuficiência cardiorrespiratória. Atualmente, não existe cura para a doença, sendo aplicadas medidas de suporte. A administração de glicocorticóides, como prednisona e deflazacort, deve ser iniciada imediatamente após o diagnóstico com objetivo de preservar a função muscular. Além disso, medidas de ventilação não invasiva devem ser fornecidas para os pacientes que precisam.
