Introdução: A obesidade é um problema de saúde em expansão entre mulheres em idade reprodutiva. Existem inúmeros desfechos adversos maternos relacionados a obesidade gestacional. E em razão da nítida correlação entre a obesidade gestacional e diversos eventos desfavoráveis, a busca por informações sobre obesidade e os respectivos graus dessa patologia durante a gestação é extremamente relevante. Objetivo: Avaliar os desfechos gestacionais adversos relacionados ao grau de obesidade prévio. Métodos: Trata-se de um estudo de corte transversal, realizado na Maternidade Darcy Vargas em Joinville–SC, no período de agosto a dezembro de 2020. Realizou-se uma entrevista a uma amostra composta de puérperas obesas. Dividiu-se as pacientes em 3 grupos, pacientes com obesidade grau 1 (IMC 30-34,9), grau 2 (IMC 35-39,9) e grau 3 (IMC>40,0), e foram comparados os desfechos gestacionais adversos nos grupos, através do cálculo de razão de chance ajustado para fatores de confusão, utilizou-se o intervalo de confiança de 95%. O projeto foi aprovado sob o número CAAE 28786020.5.0000.5363 pelo Comitê de Ética em Pesquisa (CEP) do Hospital Regional Hans Dieter Schmidt, Joinville, SC, Brasil. Resultados: A amostra foi composta de puérperas com obesidade grau 1 n=263 (68,8%), grau 2 n=79 (20,6%) e grau 3 n=40 (10,4%). Quanto as características maternas, houve diferença no peso, escolaridade, cesariana prévias, número de consultas pré-natal, Diabetes Mellitus Gestacional (DMG), Diabetes Mellitus prévio, Hipertensão Arterial Sistêmica prévia e tabagismo. Já, nas características do recém-nascido, houve diferença na via de parto e a necessidade de cesariana de emergência. Após o cálculo de razão de chance ajustado, utilizando o grupo de obesidade grau 1 como referência, verificou-se que a obesidade grau 2 aumentou a chance de recém-nascidos com baixo peso ao nascer (RC=5,720 IC95% 1,154-28,344) e a obesidade grau 3 aumentou a chance de DMG (RC=2,576 IC95% 1,263-5,257), quando comparadas com as pacientes com obesidade grau 1. Não houve interferência nos demais desfechos. Conclusão: A obesidade grau 2 elevou a chance de baixo peso ao nascer em 5,7 vezes, enquanto, a obesidade grau 3 elevou a chance de DMG em 2,5 vezes, quando comparadas as pacientes obesas grau 1.
Objetivo: Avaliar desfechos adversos perinatais relacionados a Diabetes Mellitus Gestacional (DMG). Métodos: Trata-se de um estudo caso-controle, realizado na Maternidade Darcy Vargas em Joinville–SC, no período de agosto a dezembro de 2020. Realizou-se uma entrevista a uma amostra composta de puérperas maiores de 18 anos. No cálculo de razão de chance ajustado para fatores de confusão, utilizou-se o intervalo de confiança de 95%. Os fatores de confusão utilizados foram: idade, cesariana prévia, tabagismo, alcoolismo e outras drogas. Resultados: As pacientes foram divididas em 2 grupos: puérperas com DMG (n=345/20,6%) e sem DMG (n=1325/79,3%). Quanto as características maternas, puérperas com DMG tiveram maior idade, IMC e número de consultas pré-natal, além de menor ganho de peso, comparadas as pacientes sem DMG. Além disso, pacientes com DMG tiveram mais gestações anteriores, cesarianas, abortos prévios, obesidade, Doença Hipertensiva Específica da Gestação (DHEG) e Hipertensão Arterial Sistêmica prévia, comparadas as gestantes sem DMG. Já, nas características do recém-nascido de mães com DMG, observou-se menor idade gestacional, Apgar de 1º e 5º minutos, maior incidência de UTI neonatal e cesariana, além de diferença na adequação ao peso. Após o cálculo de razão de chance ajustado, DMG aumentou a chance de recém-nascidos Grandes para a Idade Gestacional (GIG) (RC=1,399 IC95% 1,013- 1,933), necessidade de UTI neonatal (RC=1,733 IC95% 1,065-2,819) e Apgar baixo no primeiro minuto (RC=1,775 IC95% 1,117-2,820), não interferiu nos demais desfechos. Conclusão: A DMG aumentou a chance de recém-nascidos GIG em 1,4 vezes, necessidade de UTI neonatal em 1,7 vezes e Apgar baixo no primeiro minuto em 1,7 vezes.
O diabetes mellitus gestacional (DMG) é uma complicação comum da gravidez, na qual a hiperglicemia espontânea é desenvolvida, visto que o estado metabólico muda significativamente na gravidez, afetando a ação e a sensibilidade da insulina. A abordagem para o manejo ideal de um paciente diagnosticado com DMG requer uma abordagem multidisciplinar e isso inclui ensinar ao paciente o automonitoramento dos níveis de glicose no sangue, modificações na dieta e monitoramento nutricional, mudanças no estilo de vida e gerenciamento do ganho de peso materno. A atividade física se tornou um dos fatores principais da promoção da saúde e prevenção de doenças, não apenas na população não grávida, mas também para mulheres gestantes, visto que todas as principais diretrizes sobre cuidados de saúde pré-natais recomendam exercícios na gravidez para mulheres pois geram mudanças fisiológicas e metabólicas significativas e benéficas. Este artigo teve como objetivo principal revisar ensaios clínicos que avaliaram o tratamento e a prevenção de Diabetes Mellitus Gestacional (DMG) envolvendo a prática de atividades físicas variadas. Após as buscas nas bases de dados PubMed, Scielo e Science Direct, leitura dos resumos e aplicação dos critérios de inclusão e exclusão, foram incluídos 8 ensaios clínicos publicados nos últimos cinco anos. Concluiu-se que a atividade física é uma importante estratégia de prevenção e tratamento para DMG, sendo indispensável durante a gravidez, visto que modifica positivamente a saúde da gestante em diversos aspectos, como na perda de peso, na diminuição da TOTG, na diminuição do DMG e na redução da glicose basal e pós-prandial nas pacientes com DMG.

Objetivo: Avaliar as características maternas das tabagistas e os desfechos adversos perinatais relacionados ao tabagismo na gestação. Métodos: Trata-se de um estudo coorte transversal. Realizou-se uma entrevista a uma amostra composta aleatória de puérperas. Dividiu-se as pacientes em 2 grupos, pacientes que fumaram e pacientes que não fumaram na gestação. Os desfechos perinatais foram avaliados através do cálculo de razão de chance ajustado para os fatores de confusão, utilizou-se o intervalo de confiança de 95%. Resultados: Após o cálculo de razão de chance ajustado, verificou-se que a incidência foi maior em gestantes solteiras (RC =2,176 IC95% 1,375-3,445), de ensino primário (RC = 2,181 IC95% 1,457- 3,265). O tabagismo aumentou a chance de recém-nascidos Pequenos para Idade Gestaional (PIG) (RC= 2,590 IC95% 1,344-4,990), não interferiu nos demais desfechos. Conclusão: O risco de fumo durante a gestação é 2,1 vezes maior em mulheres solteiras e que possuem até o ensino primário completo. Ainda, o tabagismo aumentou em 2,5 vezes a chance de recém-nascidos PIG.
