Les tératomes ovariens sont des tumeurs issues des cellules germinales pluripotentes, on décrit 3 types de tératomes: tératome mature, immature et monodermique. Le tératome immature constitue moins de 1% des cancers ovariens, et touche préférentiellement les sujets jeunes. Nous rapportons le cas d'une patiente de 25 ans, admise pour prise en charge d'une masse abdomino-pelvienne pour laquelle elle a bénéficiée d'une échographie et d'un scanner pelvien suivie d'un traitement conservateur a base d'une annexectomie gauche, avec multiples biopsies, dont le résultat anatomo-pathologique était en faveur d'un tératome ovarien immature. Par la suite le traitement a été complété par une hystérectomie avec curage lombo-aortique et omentectomie. Nous insistons à travers cette observation et sous la lumière d'une revue de la littérature sur les terrains particuliers prédisposant à ce type de tumeur rare et grave et sur les différents critères radiologiques orientant vers ce type histologique rare des tumeurs ovariennes, afin d'améliorer le pronostic et la prise en charge de cette pathologie qui reste multidisciplinaires.
Le dispositif intra-utérin (DIU) est une des méthodes contraceptives les plus efficaces et les plus utilisé à travers le monde: environ 100 millions d'utilisatrices. La perforation reste exceptionnelle âpres la pose d'un DIU cependant c'est une des complications les plus graves. Nous rapportant le cas d'une patiente de 49 ans, notion de pose de stérilet il y a 20 ans, suivie en oncologie pour un carcinome canalaire infiltrant du sein ayant bénéficiée d'un patey puis chimiothérapie adjuvante actuellement sous hormonothérapie, qui dans le cadre du bilan d'extension, un scanner thoraco-abdomino-pelvienne a objectivé la présence d'un DIU en sous hépatique. Sous guidage coelioscopique, on a réussie à retirer le DIU qui était enchâssé dans l’épiploon au niveau de la gouttière pariéto-colique droite. Nous insistons à travers cette observation et sous la lumière de la revue de la littérature sur l'efficacité et l'innocuité du DIU lorsque la technique et les indications sont rigoureusement respectées, mais aussi sur une des complications rarissime de la pose du DIU, et à mettre en évidence le rôle diagnostic et thérapeutique de la cœlioscopie dans la prise en charge de ces migrations.
Endometrial stromal nodule (ESN) is the least common of the endometrial stromal tumors. They are rare neoplasms which are diagnosed in most instances by light microscopy. Although such nodules are benign, hysterectomy has been considered the treatment of choice to determine the margins of the tumor required for diagnosis and to differentiate it from invasive stromal sarcoma Whose prognosis is totally different. We report a case of a 45 years old woman, with presurgical diagnosis of adnexal mass or uterine tumor. She underwent a total abdominal hysterectomy. Pathologic examination revealed an endometrial stromal nodule. Through this observation, we insist on the fact that the ESNs are rare and benign entities which must be differentiated from the other invasive malignant stromal tumors; this can change the final prognosis.
Epidermoid cyst is a frequent benign cutaneous tumor. The pelvic localization does not occur very often. The literature that taps into such cases is very limited in scope. Here is a report of a 27-year-old woman with a giant pelvic retroperitoneal epidermoid cyst. The use of ultrasound exploration and computed tomography has indicated ovarian origins. The surgery also revealed a retroperitoneal epidermoid cyst, uterus and ovaries were all intact. The evacuation of a cyst was found to contain lamellas of keratin. Histology permitted us to confirm the diagnosis. The patient was faring well after two years of followup.
