Oral foregut cysts are congenital choristomas that arise in the oral cavity during embryonic development from remnants of foregut-derived epithelium. This is an unusual report of a neonate with a large congenital sublingual cystic lesion, extending superficially from the left ventral tongue to the anterior floor of the mouth, impeding breast-feeding. The differential diagnosis included dermoid cyst, epidermoid cyst, mucous retention cyst, and oral lymphangioma. The treatment of choice was enucleation under general anesthesia. Histology showed a cystic lesion with a ciliated pseudostratified columnar epithelium with numerous goblet cells. Immunohistochemistry was positive for cytokeratin 7 and thyroid transcription factor 1 and negative for cytokeratin 20, resulting in a final diagnosis of an oral foregut cyst. Three weeks after surgery, the tongue had healed with good mobility, and breast-feeding could be established. No recurrence was present at 6 months of follow-up.
A vitamina D (VD) é uma vitamina lipossolúvel com fundamental desempenho em diversas funções fisiológicas. Suas fontes são alimentação, suplementação ou síntese na pele em resposta à exposição à luz solar. As complicações da hipovitaminose D na gestação podem afetar a mãe e o lactente , sendo que em casos mais graves levam a osteomalácia materna e raquitismo infantil. As mulheres podem apresentar alterações no período gestacional, periparto e pós-parto. Já em crianças, as complicações tendem a ser mais graves e afetar suas vidas por tempo mais prolongado. O objetivo desta revisão é apresentar evidências atuais da associação da hipovitaminose D gestacional e os desfechos adversos maternos e fetais. Foi realizada uma pesquisa nas bases Science Direct, PubMed e Google Scholar para publicações de janeiro de 2013 a outubro de 2017 utilizando-se combinações de palavras-chave: “vitamin D deficiency”, “pregnancy” e “prenatal care”. Em mulheres gestantes a deficiência de VD tem sido associada a pré-eclâmpsia, diabetes mellitus gestacional, vaginose bacteriana, trabalho de parto prematuro e sintomas depressivos pós-parto. Na prole, pode levar a baixo peso ao nascer e recém-nascidos para a idade gestacional, hipocalcemia neonatal, raquitismo, diabetes tipo I, cárie dentária, comprometimento de linguagem, esclerose múltipla, distúrbios neurológicos, infecções de vias aéreas superiores e asma. Portanto, a suplementação de VD é útil para evitar resultados adversos na gravidez, mas pode não ser sempre necessária, especialmente quando as recomendações de estilo de vida são boas o suficiente para preveni-las. A fim de tomar a decisão adequada sobre a suplementação com VD, cada situação clínica individual deve ser analisada e colocada no equilíbrio correto de risco e benefício antes de prescrever a suplementação de VD. Its sources are diet, supplementation or synthesis in the skin in response to exposure to sunlight. Complications of hypovitaminosis D during pregnancy can affect the mother and infant, and in more severe cases lead to maternal osteomalacia and childhood rickets. Women may present changes during the gestational, peripartum and postpartum period. In children, complications tend to be more severe and affect their lives for a longer time. The objective of this review is to present current evidence of the association of gestational hypovitaminosis D and adverse maternal and fetal outcomes. A search was made at the bases Science Direct, PubMed and Google Scholar for publications from January 2013 to October 2017 using combinations of keywords: "vitamin D deficiency", "pregnancy" and "prenatal care". In pregnant women, VD deficiency has been associated with pre-eclampsia, gestational diabetes mellitus, bacterial vaginosis, preterm labor, and postpartum depressive symptoms. In offspring, it can lead to low birth weight and small babies for gestational age, neonatal hypocalcemia, rickets, type I diabetes, dental caries, language impairment, multiple sclerosis, neurological disorders, upper respiratory infections and asthma. Therefore, VD supplementation is useful to avoid adverse pregnancy outcomes, but may not always be necessary, especially when lifestyle recommendations are good enough to prevent them. In order to make the proper decision on VD supplementation, each individual clinical situation must be analyzed and placed in the correct balance of risk and benefit before prescribing VD supplementation. O Brasil possui números alarmantes de cesarianas. Em 2009, a cesariana chegou a corresponder a mais da metade do número de partos realizados no país - realidade que se revela bastante além do preconizado pela OMS e pela Sociedade Brasileira de Ginecologia e Obstetrícia. Em contrapartida, existe atualmente a popularização do conceito de humanização do parto e maior valorização do parto normal. Diante dessa realidade, muitas gestantes que tiveram filho por meio de parto cesáreo, agora desejam o parto vaginal. Contudo, até pouco tempo, a literatura desencorajava a indução de parto normal em pacientes com cicatriz uterina prévia, pelo maior risco de ruptura uterina. Entretanto, estudos mais recentes têm mostrado que, em locais adequados, a indução do parto vaginal após cesárea é uma opção aceitável para mulheres, sem apresentar contraindicações, e é uma conduta obstétrica recomendável. ABSTRACT Brazil has alarming numbers of caesarean births. In 2009, the cesarean surgery corresponded to more than half of the number of births performed in the country - a reality that goes far beyond what was recommended by the OMS and the Brazilian Society of Gynecology and Obstetrics. In contrast, there is currently popularization of the concept of humanization of childbirth and greater appreciation of normal childbirth. Faced with this reality, many pregnant women who had a child through caesarean delivery now desire vaginal delivery. However, until recently, the literature discouraged the induction of normal delivery in patients with previous uterine scarring, due to the higher risk of uterine rupture. However, more recent studies have shown that induction of vaginal delivery after cesarean section is an acceptable option for women, without contraindications, and is a recommended obstetric behavior. In the present report, the authors describe a case of induction of vaginal delivery in pregnant women with a previous cesarean history. Conflito de interesses RESUMOIntrodução: A ruptura uterina (RU) é uma emergência obstétrica com o maior risco de morbidade e mortalidade para a mãe e principalmente para o feto. Apesar da ausência de definição consistente na literatura, a RU pode ser descrita como deiscência ou ruptura da parede do útero, precedendo eventos sintomáticos ou clinicamente significativos. Objetivo: abordar a RU por meio de revisão da literatura existente, a fim de fornecer dados atualizados aos profissionais que lidam com essa complicação obstétrica. Metodologia: Foi realizada uma pesquisa nas bases Science Direct, Pub Med e Google Scholar para publicações de dezembro de 2012 a agosto de 2017 sobre a ocorrência de ruptura uterina utilizando a palavra chave: uterine break. Foram incluídos para a revisão 53 artigos. Os dados encontrados nos relatos de casos foram analisados em frequências absolutas e relativas. Resultados: Em revisão dos relatos de caso encontrados, constatou-se que o principal sintoma relatado pelas pacientes no momento do diagnóstico de RU foi dor abdominal intensa. A idade média relatada das pacientes foi de 30,14 anos. A maioria encontrava-se no terceiro trimestre de gestação (51,85%). Ocorreu morte fetal em 64,28% dos casos e nenhuma morte materna foi relatada. A principal conduta obstétrica foi a laparotomia exploradora (LE) (85,71%). A localização do defeito uterino ocorreu principalmente na parede anterior, e a causa provável mais relatada foi a deiscência de cicatriz prévia (35,71%). Constatamos que o fator de risco (FR) para RU mais citado foi cesariana anterior/cicatriz uterina. Conclusão: A RU deve ser pensada como diagnóstico possível sempre que uma gestante se apresente com dor abdominal aguda e presença de coleção líquida na cavidade peritoneal, mesmo que esteja no início da gestação e não apresente FR identificados na história obstétrica. A neoplasia trofoblástica gestacional abrange um grupo de doenças que têm origem no tecido placentário, apresentando tendências distintas de invasão, podendo chegar até a metástase, que é a sua forma mais grave. É uma neoplasia curável e isto se dá devido à alta sensibilidade das dosagens seriadas de gonadotrofina coriônica humana aliada ao tratamento quimioterápico e em certos casos radioterápico. O objetivo deste estudo pautou-se em levantar, junto à literatura publicada, informações a respeito da neoplasia trofoblástica gestacional e a utilização da radioterapia em seu tratamento. A metodologia utilizada foi a revisão de literatura, usando como fonte de pesquisa base de dados como Google Acadêmico; Coleção Nacional das Fontes de Informação do SUS (Coleciona SUS); Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS). A amostra deste estudo foi composta por nove artigos publicados. Os resultados obtidos na pesquisa demonstraram que quando o paciente evolui para neoplasia trofoblástica gestacional, é importante procurar por metástases, uma vez que elas podem trazer complicações, requerendo um tratamento quimioterápico, e no caso de metástase cerebral, requerem radioterapia. Ao final, concluiu-se que a neoplasia trofoblástica gestacional nada mais é do que um grupo de tumores raros que estão relacionados com a gravidez, e se caracterizam pela proliferação anormal de células que possuem características histológicas semelhantes a do trofoblasto. ABSTRACT Gestational trophoblastic neoplasm includes a group of diseases derived from the placental tissue, with distinct invasion tendencies, which might lead to metastasis, its most severe form. It is a curable neoplasm, due to the high sensitivity of serial dosages of human chorionic gonadotrophin, combined with chemotherapy treatment, and, in certain cases, radiotherapeutic treatment as well. The purpose of this study is to gather, along with the published literature, information regarding gestational trophoblastic neoplasm, and the usage of radiotherapy in its treatment. The methodology used was literature review. Also, data were collected from research databases such as Google Scholar; National Collection of Information Sources of SUS (Collects SUS); Latin American and Caribbean Literature in Health Sciences (LILACS). Samples of this study were composed by nine published articles. The results of this research have shown that when the patient advances to gestational trophoblastic neoplasm, it is important to search for metastases, since metastases may lead to complications, requiring a chemotherapeutic treatment, and, in case of cerebral metastasis, requiring radiotherapy as well. The final conclusion is that gestational trophoblastic neoplasm is nothing but a group of rare tumours that are related to pregnancy, and are characterized by the abnormal proliferation of cells that have histological characteristics in common with trophoblast.
