Pyridoxine-dependent seizures are a disorder of GABA metabolism probably due to a defective binding of pyridoxal phosphate coenzyme (PALP) with glutamate decarboxylase (GAD), the rate-limiting enzyme in GABA synthesis. The resulting GABA deficiency causes severe epilepsy in infancy. We report on a boy with seizures starting soon after birth, and only controlled by pyridoxine at pharmacological dosages. After two months without seizures, a CT scan showed hypodense white matter in frontal and occipital lobes suggestive of a retarded or defective myelination. We are not aware of other descriptions of such morphological abnormalities in a patient with this disorder.
RESUMO -Cefepime, uma cefalosporina de quarta geração, com amplo espectro de ação, é um antibiótico largamente utilizado no tratamento de infecções graves em ambientes hospitalares. O registro de segurança deste fármaco é considerado favorável. Vários casos de encefalopatia grave, associada ao uso de cefepime, reversível, foram descritos recentemente. No presente artigo, descrevemos sete casos de encefalopatia induzida por cefepime, com achados eletroencefalográficos (EEG) característicos, que apresentaram reversão do quadro com a suspensão da droga. As relações do padrão EEG encontrado nestes pacientes com estado epiléptico não-convulsivo são consideradas, bem como a possibilidade de enquadrar os pacientes estudados na entidade "encefalopatia epileptiforme". Cefepime-induced encephalopathy : clinical and electroencephalographic features in seven patientsABSTRACT -Cefepime, a fourth-generation cephalosporin, with large antibacterial spectrum, is a commonly used antibiotic for the treatment of serious hospitalar infections. Its security report is considered favourable. Recently, many cases of a severe and reversible cefepime-induced encephalopathy were described. In this p a p e r, we report seven patients with reversible cefepime-induced encephalopathy, with a peculiar EEG pattern, characterized by semiperiodic diffuse triphasic waves. We discuss the EEG abnormalities found and their association with nonconvulsive status epilepticus.
RESUMO -Estudo das variações do EEG paroxístico provocadas pelo uso do hidrato de cloral a 20%, na dose de 50mg/Kg, na indução do sono em 50 crianças epilépticas de 1 a 12 anos de idade, em monoterapia ou sem anticonvulsivantes. Foi observado que o hidrato de cloral é capaz de induzir o sono sem efeitos colaterais e é capaz de modificar o EEG em sono, melhorando a organização dos fusos de sono e diminuindo os paroxismos generalizados.PALAVRAS-CHAVE: hidrato de cloral, EEG em sono, epilepsia.Comparison between natural sleep EEG and chloral hydrate induced sleep EEG: paroxystic changes and baseline rythms SUMMARY -EEG changes associated with the use of chloral hydrate (50 mg/Kg) to induce sleep were evaluated in 50 epileptic children (ages 1 to 12 years), either taking no anticonvulsants or on monotherapy. It was observed that chloral hydrate was capable of inducing sleep without side effects and was capable of modifying the sleep EEG, improving organization of sleep spindles and generalized paroxysms.
