Modern approaches to the management of advanced stages of Parkinson's disease include deep brain stimulation (DBS). Adequate selection of candidates for DBS is a key factor for the effectiveness and safety of this method. Selection criteria are mainly based on data from large multicenter studies. It is known that the use of DBS can be accompanied by the development of side effects, the most common of which is neuropsychiatric disorders. Suicide is an extreme manifestation of neuropsychiatric disorders and a potentially reversible cause of death in patients with Parkinson's disease. Patients with chronic diseases, including neurological diseases, have an increased risk of suicide or suicide attempts. Depression and impulse control disorders following DBS are likely key contributors to suicidal behavior in patients. The authors present a clinical case – an attempt to commit suicide after DBS with the use of levodopa with an analysis of the factors contributing the development of suicidal behavior and an analysis of the management of neuropsychiatric disorders.
Болезнь Паркинсона (БП) является вторым по распространенности возрастзависимым нейродегенеративным заболеванием после болезни Альцгеймера. Эпидемиологические исследования показали, что БП затрагивает 1-2% населения старше 65 лет и 4-5% людей старше 85 лет [1]. Около 10% случаев БП развиваются в более молодой возрастной группе (20-50 лет) и в 1,5 раза чаще встречаются у мужчин.БП клинически проявляется моторными симптомами (МС; тремор, брадикинезия, ригидность, постуральная неустойчивость) в сочетании с немоторными проявлениями [2].Симптомы БП являются гетерогенной группой и могут быть классифицированы на дофаминергические и недофаминергические, что обусловливает рациональность применения различной по патогенетическому эффекту терапии для их коррекции. При этом следует отметить, что не только МС, но и многие немоторные симптомы (НМС) являются дофаминчувствительными [3].Существующие международные и российские рекомендации по медикаментозной терапии БП рекомендуют следующие группы препаратов: препараты леводопы, агонисты дофаминовых рецепторов (АДР), ингибиторы моноаминоксидазы B (иМАО-В), антагонисты N-метил-D-ас-О Б З О Р Ы 103
In clinical practice neurologists are faced with patients' questions regarding the prognosis, however, there are no specifc guidelines devoted to this problem in neurology, despite the fact that the outcome of the disease is a clinical issue with the highest priority for both the practitioner and the patient. This article examines the approaches to discuss the prognosis of neurological disease, data on the prognosis of common neurological diseases.
