RESUMENIntroducción: la colecistectomía laparoscópica es el estándar de oro para la enfermedad por cálculos biliares; sin embargo, no se encuentra exenta de complicaciones, como la hemorragia, seroma, biloma, fuga biliar, infección de la herida quirúrgica y lesión de vía biliar, siendo la presencia de conductos aberrantes uno de los principales factores de riesgo. Las variaciones anatómicas de la vía biliar se deben a alteraciones en el desarrollo embriológico, es importante que el cirujano sepa reconocerlas, ya que durante la colecistectomía existe el riesgo de ligadura, sección, fuga biliar o estenosis de un conducto accesorio o aberrante. Las fugas biliares han aumentado desde la estandarización de la colecistectomía laparoscópica, con frecuencia aproximada de 0.3-0.5%. La colangiopancreatografía retrógrada endoscópica (CPRE) es el estudio ideal, ya que confirma el diagnóstico al identificar el sitio de fuga biliar y durante el mismo procedimiento se puede realizar la esfinterotomía y colocación de endoprótesis, permitiendo el cierre del defecto, con una tasa de éxito mayor a 90%. Casos clínicos: presentamos dos casos de fuga biliar posterior a colecistectomía laparoscópica secundaria a la presencia de variaciones anatómicas de la vía biliar extrahepática, ambos manifestados por dolor abdominal requiriendo hospitalización.
