10% of the coarctation of the aorta (COA) cases are diagnosed higher than 40-year of age and other cardiac pathologies accompany to 40% of these cases. The cause of mortality in 20% of undiagonised cases is the aortic rupture of ascending aorta. Aneurysm of the ascending aorta (AAA), COA, bicuspid aortic valve (BAV)and subaortic membrane were observed in a young male patient referring to our medical center with dispnea. The patient was operated and all pathologies were repaired in one single session. AAA or aortic dissections (AD) accompanying with other cardiac pathologies like BAV, ventricular septal defect (VSD) can be repaired in one single operation by an accurate surgical approach.
Behçet's Disease (BD) is a multisystemic vasculitis which usually affects optical, genital, and oral mucosae and often reoccurs intermittently. Chylothorax is a very rare complication of BD which usually causes thrombosis in the major venous system. A 27-year-old man with a 10-year history of BD referred to our cardiovascular surgery department with symptoms of serious aches in the left arm, edema, and apparent veins on the left anterior chest wall. A total thrombosis of the left internal and external jugular veins and the left subclavian vein was observed. One month after a successful treatment and discharge, the patient returned to our clinic with symptoms of dyspnea and coughs. A chest radiograph showed a consolidated region. A milky liquid was aspirated through thoracocentesis from the left thorax, and its biochemical analysis helped us arrive at a diagnosis of chylothorax. The patient was hospitalized and administered corticosteroids and immunosuppressive therapy with a high-carbohydrate and low-fat dietary regimen for BD. Thereafter, a left thoracic drainage system was established. On the seventh day of hospitalization, due to a progressing cheilosis flow, a pleurodesis process was applied with talcum powder. However, the chylous drainage was continued and 60 mL of venous autologous blood was injected into the left thorax through a drainage tube. The treatment was successful, and the patient was discharged from the hospital uneventfully. At 1 month’s follow-up, the chest radiograph was normal.

A 72 year-old male was referred to our department because of dyspnea and mediastinal widening at chest radiography. Computed tomography (CT) scan of the chest with contrast revealed a 70x65mm saccular aneurysm of the arcus aorta with intraluminal thrombus and ascending aorta dilatation with a diameter of 45mm (Figure 1). Echocardiography revaled normal left ventricular systolic function and severe mitral regurgitation. The patient has been suffering from low back pain for 10 years which have been considered to be associated with age-related degenerative changes and overlooked by the patient and the physicians. Preoperative Rheumatology consultation was requested to exclude vasculitis and the work-up revealed ankylosing sponAnkylosing Spondilitis is a systemic inflammatory disease that involves the whole spine and sacroiliac joints. Cardiac manifestations include mitral regurgitation, aortic regurgitation, conduction abnormalities, atrial fibrillation, myocardial dysfunction and pericarditis. Thymoma is one of the most common tumor types in anterior mediastinum, and its surgical treatment according to the type and degree of invasion can be very difficult. A 45-year-old woman with myasthenia gravis underwent a thorax tomography, a finding compatible with thymoma was detected and robotic thymectomy was planned. It was returned to sternotomy because of the presence of severe adhesions. After resection, pericardial patchplasty was performed in the superior vena cava and a graft interposition was made between the left brachiocephalic vein and the right atrium auricular. In this case report, we present a vascular reconstruction alternative in tumor surgery with severe environment and vascular tissue invasion.
