Introdução: O câncer de pulmão ocupa o segundo lugar mundial em mortalidade geral, e é a principal causa de morte por neoplasia. Divide-se em tumor (TU) não pequenas células e tumor pequenas células, este sendo mais agressivo. A associação direta entre carga tabágica e a neoplasia faz do tabagismo fator de risco influenciador na gravidade da doença. O tumor de Pancoast é localizado no ápice do lobo superior pulmonar. A lesão exerce efeito de massa que culmina com compressão do plexo braquial, destruição das costelas contíguas, invasão da pleura parietal e eventualmente destruição vertebral e infiltrado medular. Em decorrência, surgem sinais e sintomas que configuram a síndrome de Pancoast como dor no ombro, a qual pode irradiar para tórax, cabeça, pescoço, braço ipsilateral e trajeto do nervo ulnar. Desenvolvimento: Relata-se o caso de um paciente, masculino, 49 anos, fumante há 34 anos, com Pancoast em estadio IV (T4N2M1) ao diagnóstico. O diagnóstico do tumor de Pancoast é obtido através da associação entre exame de imagem que evidencie a massa em ápice pulmonar, a relevante clínica do doente e biópsia confirmatória de neoplasia primária pulmonar. A frequência de metástases relaciona-se diretamente com o tipo histológico e grau de diferenciação, sendo o não diferenciado o mais relacionado. Os locais mais acometidos pelos implantes secundários são sistema nervoso central (SNC), osso, fígado, adrenais. Discussão: O prognóstico é ruim com tempo de sobrevida em 5 anos baixo, mesmo nos estádios iniciais. Hoje existem três modalidades de tratamento para a neoplasia pulmonar: radioterapia, quimioterapia e ressecção cirúrgica, a depender do estadiamento. O TU de Pancoast inicial apresenta-se como T3, embora seja passível de abordagem cirúrgica podendo ser completamente ressecado. A redução da prevalência do tabagismo permanece como estratégia soberana na prevenção da neoplasia pulmonar e de suas temidas consequências. ABSTRACT Introduction: Lung cancer takes the second place worldwide in general mortality and is the main reason of death by neoplasm. The classification of the tumor is split at non-small cells and small cells, this one being more aggressive. The direct association between smoking load and the neoplasm makes the smoking the biggest influencer in the seriousness of the disease. The Pancoast tumor takes place at pulmonary apices usually. The wound is associated with the mass effect, the destruction of local ribs, the invasion of parietal pleurae and, eventually, the spinal destruction and medullary infiltrate. As a result, the signs and symptoms that configures the Pancoast syndrome emerge, as the shoulder pain, which can irradiate to cheats, head, neck, ipsilateral arm and the path of ulnar nerve. Development: report a patient, male, 49 years, smoker by 34 years, with Pancoast tumor in stage IV (T4N2M1) at the diagnosis. Discussion: The association among the image exam that shows pulmonary apices’ mass, the relevant sick’s clinic and the confirmatory biopsy of primary pulmonary neoplasm takes the diagnosis of Pancoast tumor. The frequency of metastasis relates direct with the histological sort, being the not differenced the most related. The places with more impairment by the secondary implants are the central nervous system, bone, liver and adrenal. The prognostic is mean with time of survival in 5 years low even in initial stages. Nowadays there are three genres of treatment for the pulmonary neoplasm: radiotherapy, chemotherapy and surgery resection, which depends of the stage. The initial Pancoast tumor shows usually as T3 however could be believable of surgical approach, being able to be fully resected. The reduction of the prevalence of the smoke remains to be the sovereign at the preventions of pulmonary neoplasm and their terrifying after effect. A cardiopatia com dupla via de saída do ventrículo esquerdo (DVSVE) é uma anomalia rara, podendo estar associada com ventrículo esquerdo único, o que a torna mais incomum. Lactente, 6 meses, sexo feminino, admitida em pronto socorro com história de dificuldade de ganho de peso, cansaço às mamadas e atraso no desenvolvimento neuropsicomotor. À ausculta cardíaca apresentava bulhas hiperfonéticas e sopro holossistólico panfocal, mais audível em borda esternal esquerda. Ictus cordis palpável e propulsivo. Apresentava aumento do diâmetro anteroposterior torácico, com retração intercostal e subcostal. Radiografia de tórax demonstrou aumento da área cardíaca e da trama vascular; eletrocardiograma evidenciou sobrecarga atrial e ventricular esquerdas. O ecocardiograma demonstrou situs solitus em levocardia com ventrículo único tipo esquerdo, câmara ventricular direita rudimentar, com dupla via de saída de ventrículo esquerdo, aorta anterior e artéria pulmonar posterior dilatada. Mantida em internação para aguardar correção cirúrgica, evoluiu com quadro de pneumonia nosocomial, descompensação cardiovascular e respiratória, com necessidade de intubação orotraqueal e ventilação mecânica, tendo como desfecho o óbito. A DVSVE é uma entidade heterogênea com diversas variações anatômicas, sendo um delas o ventrículo único. A coexistência dessas associações se torna rara e pouco descrita na literatura. A avaliação do recém-nascido com suspeita de cardiopatia deve ser realizada de maneira cautelosa. O ecocardiograma permite a confirmação diagnóstica. Crianças com cardiopatia congênita apresentam maior risco de complicações infecciosas, sobretudo naquelas com hiperfluxo pulmonar. O tratamento é paliativo e consiste em intervenção cirúrgica. O diagnóstico precoce de cardiopatias congênitas passíveis de intervenção é fundamental para que seja programada a correção cirúrgica o mais breve possível, a fim de promover maior sobrevida ao paciente e reduzir a morbimortalidade. ABSTRACT Double outlet left ventricle heart disease is a rare anomaly and may be associated with a single ventricle of the left type, which makes it more uncommon. Infant, 6 months, female, admitted in the emergency room with history of difficulty gaining weight, tiredness to feedings and delayed neuropsychomotor development. Cardiac auscultation presented hyperphonectic sounds and panfocal holosystolic murmur, more audible at the left sternal border. Palpable and propulsive Ictus cordis. Thoracic diameter increased with intercostal and subcostal retraction. Chest X-ray showing increased cardiac area and vascular weave; Electrocardiogram showed atrial and left ventricular overload. The echocardiogram showed situs solitus in levocardia with single left ventricle, rudimentary right ventricle chamber, with left ventricular outflow tract, anterior aorta and dilated posterior pulmonary artery. Maintained in hospital to await surgical correction, evolution with hospitable pneumonia, cardiovascular and respiratory decompensating, requiring orotracheal intubation and mechanical ventilation, with the outcome of death. The LVEDV is a heterogeneous entity with several anatomical variations, one of them being the single ventricle. A coexistence associates becomes rare and little described in the literature. An evaluation of the newborn with suspected cardiopathy should be performed with caution. The echocardiogram allows diagnostic confirmation. Children with congenital heart disease have an increased risk of infectious complications, especially in those with pulmonary hyperflow. The treatment is palliative and consists of surgical intervention. The prior diagnosis of congenital heart diseases that are amenable to intervention is essential for the timing of surgical correction or as soon as possible, in order to promote greater patient survival and reduce morbidity and mortality. O tumor teratoide rabdoide é uma neoplasia primária rara do sistema nervoso central (SNC) que tem maior prevalência em crianças menores de três anos. Tem evolução peculiar com tempo de sobrevida muito curto principalmente nas faixas etárias inferiores a três anos. Relatamos um caso de criança de cinco anos com tumor (TU) em região fronto-parietal esquerda, queixas de cefaleias persistentes, progressivas, refratárias a analgésicos comuns e que posteriormente evoluiu com rigidez de nuca e dislalia. A tomografia computadorizada (TC) de crânio evidenciou lesão expansiva compatível com tumor de SNC. Paciente foi operado, o diagnóstico foi estabelecido por análise histopatológica e a imuno-histoquímica revelou tratar-se de tumor teratoide rabdoide atípico (TTRA). Devido a recidiva tumoral em um curto período de tempo, pontuação na Escala de Karnofsky de 50 e impossibilidade de ressecção tumoral total, o paciente foi submetido apenas a tratamento paliativo. A terapêutica representa hoje um desafio na onco-pediatria, onde muitas abordagens são descritas, mas não há consenso com relação ao melhor metódo. Não se observa até o presente a superioridade de um esquema terapêutico sobre outro, não havendo aumento significativo da sobrevida ou cura do paciente.
