ABSTRACT:The aim of this qualitative study was to analyze family participation in care delivery to newborns in daily practice at a Neonatal Intensive Care Unit. The study was carried out at five hospitals in Belo Horizonte, Minas Gerais, Brazil. The study subjects were healthcare professionals taking care of newborns hospitalized at the Neonatal Intensive Care Unit and the relatives of these infants. Data were collected through participant observation and daily conversations and analyzed using Content Analysis. The data permitted capturing aspects of family participation in newborn care related to institutional organization, interaction with healthcare professionals and the care process. Different actions favoring mothers' participation in care delivery to their children were identified at the Neonatal Intensive Care Unit. Nevertheless, the study showed that these actions have not been incorporated into the daily activities at the institution, nor into healthcare professionals' practice. El estudio se llevó a cabo en cinco hospitales de Belo Horizonte, Minas Gerais, Brasil. Tuvo como sujetos los profesionales sanitarios en la Unidad de Terapia Intensiva Neonatal y los familiares del recién nacido. Los datos se recolectaron por observación participante y conversaciones del cotidiano y fueron analizados a través de la técnica de Análisis de Contenido. Los datos permitieron captar aspectos de la participación de la familia relacionados con la organización institucional, la interacción con los profesionales y la realización del cuidado. Se identificó la existencia de diferentes acciones que favorecen la participación de la madre en el cuidado del hijo en la Unidad de Terapia Intensiva Neonatal, aún así, estas no están incorporadas en el cotidiano institucional y en el quehacer de los profesionales sanitarios. A terapia ocupacional no contexto da assistência à mãe e à família de recém-nascidos internados em unidade de terapia intensiva. Rev. Ter. Ocup. Univ. RESUMO: O objetivo deste artigo é relatar a experiência da atuação da Terapia Ocupacional no Hospital Sofia Feldman, visando uma assistência integral e humanizada à mulher e ao recém-nascido. Para isso, são descritas as ações desenvolvidas junto à gestante de risco e à mãe e à família de recém-nascido internado em Unidade de Terapia Intensiva Neonatal (UTIN), respaldadas pelo referencial teórico e filosófico do cuidado centrado na família. Percebe-se que essa experiência tem possibilitado à mulher viver de forma mais tranqüila e segura a gravidez e/ou internação de seu bebê, tem favorecido o estabelecimento do vínculo mãefilho e família em situação de prematuridade e tem contribuído na orientação às famílias sobre os cuidados com o bebê. Isso fica evidenciado na participação e adesão das mesmas aos grupos desenvolvidos e na segurança demonstrada para realizar os cuidados com o bebê durante a internação e após a alta hospitalar. Considera-se esta uma experiência ainda restrita e com escassos estudos no país, o que indica a necessidade de investimentos em conhecimentos e habilidades específicas, bem como estudos que aprofundem mais o tema abordado. Introduction: The high rate morbidity associated with preterm birth, which gestational period is less than 37 weeks, frequently demands the hospitalization of the newborn in neonatal intensive care units (NICUs). The period of hospitalization triggers in the parent's feelings of insecurity, sadness, stress, and anxiety. The multi-professional staff that accompanies the newborn and his family plays a key role in supporting parents. Occupational Therapists have developed comprehensive and interactive programs to help parents deal with anxiety about the baby's hospitalization in the NICU by conducting activity groups. Objective: To know the contributions of Occupational Therapy groups in the anxiety levels of mothers with preterm newborns admitted in the NICU. Method: Forty mothers of hospitalized newborns who met the inclusion and exclusion criteria participated in the study. The State-Trait Anxiety Inventory (STAI) was applied with an I-STAI scale (A-state) before and after the performance of the activity group. Finally, a Focal Group with a semi-structured interview was carried out. Results: The inventory showed reductions in scores for items such as "I feel anxious" (p <0.001), "I feel nervous" (p = 0.008) and "I am worried" (p <0.001), which was corroborated by the analysis of the mothers' report in the focal group. Conclusion: The interaction between the mothers provided by the Occupational Therapy groups and the execution of activities directed to the experienced context contribute to reducing the anxiety felt by the mothers during the hospitalization of the child in the NICU.
OBJETIVOS: conhecer o cotidiano no alojamento de mães, cujas crianças estão internadas em uma Unidade de Terapia Intensiva Neonatal. MÉTODOS: pesquisa qualitativa realizada no Hospital Sofia Feldman, em Belo Horizonte, Minas Gerais, Brasil. Os dados foram obtidos por meio de observação participante e grupo focal, e os instrumentos utilizados para registro dos dados foram os diários de campo, gravadores e fitas. Para análise dos dados utilizou-se a análise de conteúdo. RESULTADOS: ao decidir permanecer no alojamento materno a mãe se sente dividida entre atender as necessidades do filho hospitalizado, as demandas da família e as suas próprias necessidades. A vivência no ambiente hospitalar exige que a mãe se adapte a uma nova dinâmica, determinada pelas rotinas e normas institucionais. A permanência no alojamento materno, ao mesmo tempo em que surge como uma possibilidade das mães estabelecerem relações de amizade e solidariedade, é um espaço de relações conflituosas entre elas. CONCLUSÕES: na adaptação da mãe ao novo cotidiano, revelou-se importante a convivência entre as mães, a família e os profissionais de saúde. O estudo aponta para a importância do profissional de saúde aproximar-se da realidade vivenciada pelas mães, para facilitar sua adaptação.
Introduction: The term co-occupation has been used by occupational therapists to define the implicit involvement of two or more individuals in an occupation. In this perspective, maternal care is considered a co-occupation, and occupational therapists seek to promote the insertion of the mother in the Neonatal Intensive Care Unit (NICU), maternal guaranteeing autonomy and independence in the care of the baby. Objective: To analyze how the construction of maternal co-occupation in the NICU takes place. Method: A qualitative, multiple-case study conducted at a philanthropic hospital in Belo Horizonte, Minas Gerais, Brazil, and attended by six mothers of infants admitted to the NICU. Semi-structured interviews and the participant's diary were used as instruments of data collection. The data were submitted to content analysis in thematic modality with the use of MAXQDA software. The themes were grouped giving rise to two categories with their respective subcategories. Results: It was verified that the mothers experience different feelings related to the baby's hospitalization in the NICU, and their involvement in co-occupations in this context happens gradually, intensifying from the clinical improvement of the baby. Conclusion: The way mothers construct co-occupation during the infant's hospitalization in the NICU is based on their understanding and previous experience about motherhood, as well as on aspects related to the context of the NICU. It is necessary that occupational therapists know the expectations of mothers in order to build with them possibilities of involvement in co-occupations that meets their expectations.Resumo: Introdução: O termo co-ocupação tem sido utilizado por terapeutas ocupacionais para definir o envolvimento implícito de dois ou mais indivíduos em uma ocupação. Nessa perspectiva, o cuidado materno é considerado uma co-ocupação, sendo que os terapeutas ocupacionais buscam promover a inserção da mãe na Unidade de Terapia Intensiva Neonatal (UTIN), garantindo a autonomia e independência materna nos cuidados do bebê. Objetivo: Analisar como se dá a construção da co-ocupação materna na UTIN. Método: Estudo de caso múltiplo, descritivo-exploratório de abordagem qualitativa, realizado em um Hospital filantrópico de Belo Horizonte, Minas Gerais e que teve como participantes seis mães de bebês internados na UTIN. Utilizou-se como instrumentos de coleta de dados a entrevista semiestruturada e o diário da participante. Os dados foram submetidos à análise de conteúdo na modalidade temática com a utilização do software MAXQDA. Resultados: Verificou-se que as mães vivenciam diferentes sentimentos relacionados à internação do bebê na UTIN, sendo que seu envolvimento em co-ocupações, nesse contexto, acontece de forma gradativa, se intensificando a partir da melhora clínica do bebê. Conclusão: O modo como as mães constroem a co-ocupação durante a internação do bebê na UTIN baseia-se no seu entendimento e na sua vivência prévia acerca da maternidade, bem como nos aspectos relacionados ao contexto da UTIN. ...
