Introdução: A utilização de meios de contraste, muitas vezes, permite uma análise mais adequada da morfologia e identificação de doenças. O meio de contraste é comumente administrado por uma bomba de infusão, garantindo fluxo contínuo. No entanto, essa administração automatizada pode aumentar a incidência dos extravasamentos, acarretando a vazão de grandes volumes em um curto período de tempo. Objetivo: correlacionar fatores de risco para o extravasamento do meio de contraste a base de iodo com as barreiras utilizadas e medidas adotadas quando instalada a ocorrência. Métodos: revisão sistematizada utilizando as bases de dados Pubmed e Scielo, nas quais formam analisados 16 artigos dos últimos 10 anos. Resultados: embora pequena a ocorrência, entre 0,01 a 0,09% dos exames contrastados realizados, o extravasamento é um assunto constante nos centros de imagens radiológicas, sendo de maior incidência em exames de tomografia computadorizada. Um adequado acesso venoso juntamente com meio de contraste que favoreça a infusão estão entre os principais fatores para a transcorrência adequada deste tipo de procedimento. Conclusão: elaboração e aplicação de protocolos que assegurem a não ocorrência, assim como os que abranjam a diminuição das complicações como a síndrome compartimental, proporcionará o aumento de satisfação ao serviço. Casos em que a prevenção não se faz suficiente, uma adequada abordagem que vise diminuir maiores efeitos indesejáveis, diferenciar o serviço oferecido e a credibilidade dos profissionais envolvidos. Contrast medium is commonly administered by an infusion pump, ensuring continuous flow. However, this automated administration can increase the incidence of overflows, resulting in the flow of large volumes in a short period of time. Objective: a systematic review of the correlation of risk factors for extravasation of iodine- based contrast medium, with the most used barriers, as well as measures adopted when such an occurrence occurs. Methods: analysis and review of correlated data from PUBMED and Scielo platforms and websites. Eleventh articles were selected, of these, after reading the abstracts and adequacy of objectives, 16 articles from the last 10 years were used. Results: although the occurrence is small, between 0.01 and 0.09% of contrast-enhanced exams performed, extravasation is a constant issue in radiological imaging centers, with higher incidence in computed tomography exams. The study seeks to elucidate the direct correlation between venous access and the conditions of the contrast medium used, which together favor the infusion, especially when specific risk factors are identified early, thus favoring the proper course of this procedure. Conclusion: elaboration and application of protocols that ensure the non-occurrence, as well as those that cover the reduction of complications such as compartment syndrome, will provide increased satisfaction with the service. In cases where prevention is not enough, an adequate approach aimed at reducing further undesirable effects will differentiate the service offered and the credibility of the professionals involved. Introdução: Os avanços tecnológicos das últimas décadas permitiram que a cardiologia intervencionista se expandisse visivelmente propiciando o diagnóstico e terapêutica de cardiopatias através de procedimentos médicos pouco invasivosguiados por imagens radiológicas de escopia. Devido à necessidade de aproximação ao paciente e ao tubo de raios-X durante todo o procedimento existe um risco potencial para o aumento da dose nos IOEs. Objetivo: Traçar o perfil dosimétrico da sala de hemodinâmica durante um procedimentopadrão utilizando-se medidores de alta precisão. Método: A dosagem de radiação foi realizada no serviço de Hemodinâmica da Santa Casa de Misericórdia de São Paulo que utiliza um equipamento modelo MAVIG INNOVA IGS 5 da marca General Electrie-GE e no Hospital Santa Isabel, que utilizaum equipamento modelo ALLURA XPER FD 20 da marca PHILIPS. A medição consistiu em observar alterações na dose acumulada principalmente nos pontos em que se concentram os profissionais durante os procedimentos, quando dainterposição de barreira plumbífera através da distribuição da dose nestes pontos. Resultados: O estudo mostrou que a dose pode ser reduzida em até 90% com a interposição da barreira plumbífera de proteção. Foi possível observar a redução de 75% da dose ao aplicarmos a lei do inverso do quadrado da distância nos principais pontos estudados. Conclusão: O uso de aparatos de proteção proporcionou diminuir a alta taxa de dose e a probabilidade de ocorrência dos efeitos biológicos da radiação ionizante.Palavras-chave: Hemodinâmica, Exposição ocupacional, Proteção radiológica, Raios X, Radiologia intervencionistaABSTRACTIntroduction: The technological advances of the last decade allowed the intervencionist cardiology to expand visibly providing the diagnostic and therapeutic of cardiopathies through less invasive medical procedures guided by radiological images of endoscopy. Due to the need for the presence of the patient and the x-ray tube during all the procedure a potencial risk exist in the increase of the dose for the Occupational Exposed Individual (IOEs). Objective: Trace a dosimetric profile of the hemodynamic room during the standard procedure using measuring equipments of high precision. Method: Radiation dosing was performed at the Hemodynamics Service of Santa Casa de Misericórdia de São Paulo, which uses a MAVIG INNOVA IGS 5 model equipment from General Electrie-GE and at Hospital Santa Isabel, which uses an ALLURA XPER FD 20 model equipment. PHILIPS brand. The measurement consisted ofobserving changes in the accumulated dose, mainly in the points where the professionals are concentrated during the procedures, when the lead barrier is interposed through the dose distribution in these points. Results: The studyshowed that the dose can be reduced by up to 90% with the interposition of the protective lead barrier. It was possible to observe a 75% reduction in the dose when applying the inverse law of the square of the distance in the main pointsstudied. Introdução: O Câncer de Próstata (CP) é o tumor maligno mais comum na população masculina acima dos 50 anos, sendo o adenocarcinoma o tipo histológico responsável por cerca de 95% dos casos. É o terceiro câncer com maior taxa de mortalidade entre os homens no mundo. A recorrência bioquímica do CP após prostatectomia radical é um problema clínico relevante. Objetivo: essa revisão teve como objetivo descrever o uso PET/CT-PSMA-68Ga no reestadiamento do CP nos casos de recidiva bioquímica após prostatectomia. Método: realizamos uma revisão bibliográfica na base de dados da PubMed, nos últimos três anos, de artigos publicados na íntegra. Resultados: Treze artigos foram usados na análise, a média da taxa de positividade do PET entre os estudos foi de 67.9%, variando de 34.4% a 89.5%. Discussão: todos os estudos concordam que maiores taxas de detecção foram diretamente proporcionais aos valores de PSA. Sete artigos mensuraram o impacto do PSMA na mudança terapêutica com uma média de 66.5% de alteração do tratamento (de 28.6% a 87.7%). Conclusão: Com base na análise dos artigos concluiu-se que o PET/CT-PSMA- - 68Ga na recorrência bioquímica do CP é útil na detecção de lesões locais e/ou metastáticas, e ainda importante no reestadiamento contribuindo nas decisões terapêuticas futuras.Palavras chave: Neoplasia prostática, Tomografia computadorizada, Recorrência, Bioquímica, PET/CT, Antígeno prostático específico Introduction: Prostate cancer (CP) is the most common malignant tumor in the male population over 50 years old, with adenocarcinoma being the histological type responsible for about 95% of cases. It is the third cancer with the highest mortality rate among men in the world. Biochemical recurrence of PC after radical prostatectomy is a relevant clinical problem. Objective: this review aimed to define the use of PET / CT-PSMA-68Ga without PC restaging in cases of biochemical recurrence after prostatectomy. Method: we performed a bibliographic review in the PubMed database, in the three years, of articles published in full. Results: Thirteen articles were used in the analysis, the average PET positivity rate between studies was 67.9%, varying from 34.4% to 89.5%. Discussion: all studies agree that higher detection rates were directly proportional to the PSA values. Seven articles measured the impact of PSMA on therapeutic change with an average of 66.5% of treatment change (from 28.6% to 87.7%). Conclusion: Based on the analysis of the concluded articles, PET / CT-PSMA-68Ga in the biochemical recurrence of PC is useful in the detection of sites and / or metastases, and also important in restaging, contributing to future therapeutic decisions.
