Objectives To share our experience with digital slide telepathology for intraoperative frozen section consultations (IOCs) and to describe its evolution over time by reporting performance metrics and addressing organizational and economic aspects. Methods Since 2013, a technician has been alone at the surgical site. At the other site, the pathologist opens the digital slide from a local server via the intranet. Three periods were compared: a 6-month period of conventional IOC (period 1), a 24-month period of telepathology at 6 months after implementation (period 2), and a 12-month period of telepathology at 3.5 years after implementation (period 3). Results In total, 87 conventional IOCs and 464 and 313 IOCs on digital slides were performed respectively during periods 1, 2, and 3; mean turnaround time was 27, 36, and 38 minutes, respectively, and there were a mean number of 1.1, 1.1, and 1.3 slides, respectively, per IOC. Diagnostic accuracy was achieved in 95.4%, 92.7%, and 93.9%, respectively, of IOCs (not significant). The additional cost is in the same range as the cost of urgent transport by courier. Conclusions Developing IOC with digital slides is a challenge but is necessary to optimize medical time in the current context of pathologist shortage and budget restrictions.
L'examen extemporané est un examen anatomopathologique particulier qui se déroule au cours d'une intervention chirurgicale ; il est donc par définition urgent avec un délai communément admis de moins de 30mn. Compte tenu de la qualité moindre des coupes, du biais d'échantillonnage et du contexte urgent, la réponse de l'examen extemporané ne peut être qu'un résultat préliminaire qui doit être confirmé ou infirmé par l'analyse de la lame paraffine définitive. L'examen extemporané à distance ou Télédiagnostic extemporané qui permet la réalisation d'examens extemporanés par un pathologiste non présent sur le site opératoire, a été rendu possible par la technologie des microscopes robotisés dès la fin des années 90 et surtout par la technologie des lames numériques à partir des années 2010 (Tableau 1). Le recours au Télédiagnostic extemporané représente un besoin nouveau et croissant de notre spécialité pour des raisons économiques et démographiques. Le regroupement des structures d'Anatomie et Cytologie Pathologiques (ACP) dans le but d'une mutualisation des plateaux techniques pose la question de l'accès à l'examen extemporané dans les établissements de santé où se maintient une activité chirurgicale sans service ACP sur place. La réalisation d'examens extemporanés à distance par un pathologiste non présent sur le site opératoire apporte une réponse à cette question et améliore l'offre de soins sur le territoire. Côté ACP, le télédiagnostic extemporané est un moyen d'optimiser l'organisation médicale ACP dans un contexte d'activité accrue et de pénurie démographique car il permet d'éviter le détachement ou les déplacements d'un médecin pathologiste entre 2 sites. Ce besoin n'est pas propre à notre pays. A titre d'exemple, le Plan Cancer 2014-2018 du Luxembourg mentionne spécifiquement le recours à la Télé-anatomopathologie pour les examens extemporanés Le volet Extemporané du démonstrateur d'un réseau de Télépathologie en Ile de France qui s'est déroulé de juin 2014 à décembre 2015 a permis de préciser les aspects techniques du Télédiagnostic extemporané.
