IntroduçãoApesar da enorme influência da medicina hospitalar, a figura do clínico geral ou do médico de família talvez simbolize ainda hoje uma imagem idealizada da medicina. Esse tipo de prática foi se configurando com formas e importância distintas em diversos países sendo atualmente denominada de atenção primária, o que inclui, algumas vezes, o exercício de especialidades bá-sicas ou a presença de outros profissionais. Duas concepções de Atenção Primária de Saúde são predominantes, a de cuidados ambulatoriais na porta de entrada ou a de uma política de reorganização do modelo assistencial, quer seja sob forma seletiva ou ampliada.Optamos por enfocar a trajetória da atenção primária enquanto política de reforma setorial, reunindo observações de estudos que realizamos complementados com trabalhos mais recentes feitos para o caso brasileiro. Um dos desafios advém do fato de que nas práticas locais, objetos de nosso interesse, se entrelaçam determinantes das esferas macro e microssocial numa rede de fatores cuja complexidade nem sempre poderá ser desenvolvida dentro dos recortes e limites deste trabalho. Por isso, a importância de todas as contribuições que se fizerem necessárias, uma vez que a idéia é de que o panorama apresentado possa oferecer um conjunto de ponderações para facilitar a discussão do tema.DEBATE DEBATE
Discute-se a avaliação de políticas de atenção primária, a partir da análise do caso de implantação do Programa Saúde da Família (PSF) em Florianópolis, Santa Catarina, Brasil. Os resultados são apresentados em duas etapas. Na primeira, o programa foi contextualizado nas políticas municipais identificando-se sua percepção em nível de gestão. Na segunda, foram estudadas as práticas numa amostra de cinco equipes usando-se o acesso e a integralidade como categorias operativas. O PSF começa com relativo atraso na capital passando de principiante para um modelo singular. Há maior disponibilidade da oferta para a população não beneficiária de planos de saúde, embora haja ainda grande diversidade das práticas. O grau de implantação é moderadamente adequado: há maior integralidade na atenção, mas existem problemas no acesso (relação equipe/número de famílias). Sugere-se que tais propostas de reorientação de modelos assistenciais tendem à racionalização, legitimação política ou democratização dos serviços. O encaminhamento da problemática referente ao acesso pode determinar a direção predominante.
Diretriz política, ideal ou objetivo, a integralidade torna-se um atributo relevante na avaliação da qualidade do cuidado e dos sistemas de saúde. Discute-se o significado desse conceito na prática de alguns serviços e sistemas de saúde, analisando metodologias para sua operacionalização. No Brasil, predomina uma percepção ampliada com ênfase no caráter completo, contínuo e coordenado do cuidado e da gestão. Destaca-se a importância de associar a avaliação da integralidade com medidas de acesso, sugerindo-se a escolha de indicadores dos pactos de metas das programações municipais para seu monitoramento. Tal iniciativa poderia aumentar a validade do instrumental de avaliação existente, numa perspectiva comunicativa.
