Background: currently, Hodgkin lymphoma (HL) in children is characterized by excellent treatment outcomes due to the use of modern risk-adapted treatment strategies. Overall survival rate for HL is more than 90%, therefore, further development of therapeutic protocols should be focused on minimizing toxicity and late adverse reactions.Aim: to improve HL treatment outcomes in children by intensive risk-adapted chemotherapy and personalized approach to radiotherapy.Patients and Methods: 134 children with HL stage II (with large tumor mass) to IV diagnosed for the first time were enrolled in the study. Morphological immunological verification of the diagnosis was performed in all children. HL was staged using imaging techniques (i.e., gallium-67 or technetium-99 imaging, computed tomography, positron-emission tomography). HL-2007 SRC POH protocol based on escBEACOPP regimen was applied. To assess treatment efficacy, overall, relapse-free, and event-free survival rates were calculated. Statistical analysis was performed using SPSS19.0 software.Results: risk-adapted treatment based on the terms of complete remission achievement has provided high survival rates even in advanced HL stages. Thus, 14-year overall survival rate was 97.7±1.3%, relapse-free survival rate 92.2±2.4%, and event-free survival rate 90.7±2.6%. Among late adverse reactions, secondary tumors (i.e., breast cancer after mediastinal radiotherapy) were reported. In girls with early (after 4 chemotherapy courses) complete response, we were able to avoid radiotherapy which was not associated with poorer treatment outcomes (overall survival rate was 98.5±1.5%).Conclusion: personalized HL treatment using HL-2007 SRC POH regimen provides high survival rate in most children and the avoidance of radiotherapy in girls with early complete response without negative impact on treatment outcomes. Большинство детей, больных лим-фомой Ходжкина (ЛХ), удается излечить независимо от стадии болезни благодаря современной программной риск-адаптированной терапии. Однако у 3-5 % детей раз-виваются рецидивы заболевания или отмечается реф-рактерность к проводимой терапии. Цель -сравнитель-ный анализ эффективности противоопухолевой терапии ViGePP vs ICE у больных с рецидивами и рефрактерными формами ЛХ; оценка необходимости выполнения ауто-ТГСК и места комбинированного химиолучевого лечения у этой категории пациентов. Методы. С июня 2003 г. по декабрь 2014 г. 35 больным с рецидивами (n = 18) и рефрактерными формами (n = 17) ЛХ проводилась терапия, основанная на двух режимах: ICE (n = 14; 40 %) и ViGePP (n = 14; 40 %). У 7 (20 %) боль-ных осуществлена смена одного режима на другой в свя-зи с неудовлетворительным противоопухолевым ответом на первые два курса лечения. Результаты. Непосредственная эффективность лечения была существенно выше у больных, получавших химио-терапию по схеме ViGePP в сравнении с ICE, независимо от статуса болезни: рецидив или первичная рефрактер-ность. Полный ответ чаще регистрировался у тех боль-ных с рецидивами ЛХ, у которых на начальных этапах программное лечение включало лучевую терапию. Более высокие показатели выживаемости оказались у девочек, чем у мальчиков, а также у детей с полным общим от-ветом на противорецидивное лечение. При рецидивах от-даленные результаты лечения (безрецидивная и общая выживаемость) были лучше у детей, получивших 4 курса ViGePP, в сравнении с получившими 2 курса ICE. Высоко-дозная химиотерапия с последующей аутоТГСК позволя-ет преодолеть развившуюся к химиотерапии рефрактер-ность только у 45 % пациентов. Заключение. Дети с рецидивами и рефрактерными фор-мами ЛХ нуждаются в интенсивной противорецидивной химиотерапии с целью достижения полного ответа с по-следующей высокодозной химиотерапией и аутоТГСК. Results. The direct effectiveness of the therapy was significantly higher in patients on ViGePP as compared to ICE irrespective of the disease status (relapsed or refractory). A complete response was achieved more often in those children with relapse HL whose initial treatment included radiation therapy. Higher survival rates were registered in girls, as well as in children with a complete overall response to the antirelapse therapy. In case of relapses, delayed treatment effects (disease free survival and overall survival) were higher in children treated with 4 courses of ViGePP than 2 courses of ICE.
