Electronic poster abstractspediatric and surgical care of the newborn the aim of this study was to describe the outcome of patients prenatally diagnosed with congenital kidney and urinary tract anomalies. Methods: A prospective observational study included a sample of 19 cases prenatally diagnosed with CAKUT in a one-year period. This study took place in the Maternity and Neonatology Center of Tunis. Results: The average age of pregnant women was 31 years old and 48% of them were multiparous. Kidney and urinary tract anomalies were diagnosed in the second trimester ultrasound in 65% of the cases. Termination of pregnancy was proposed in 7 cases of kidney and urinary tract anomalies associated with oligohydroamnios: bilateral renal agenesis, multicystic dysplastic kidney, hydronephrosis and megacystis. The clinical course of 12 patients prenatally diagnosed with CAKUT was heterogeneous. The diagnosis was confirmed in 7 cases. Urinary tract infection occurred in one case of non-obstructive megaureter. Surgery was performed in one case of pelviureteric junction obstruction with a favourable outcome. Conclusions: Prenatal diagnosis of kidney and urinary tract anomalies allows a better management of this fetal abnormality and an appropriate counselling for the mothers to be. The prognosis depends on whether these anomalies were associated with other anomalies or chromosomal aneuploidy or not and oligohydramnios. The patient was admitted to our department on 01/21/2008, i.e. on a term of 37 weeks + 1 day for premature rupture of the membrane for more than 12 hours. On examination: BDC positive, TV: cervix dilated to 4cm full seat high, sac of ruptured waters with clear amniotic fluid. Ultrasound: complete breech presentation, deflected head, BIP: 96, bones of both femurs appear Discontinuous. Caesarean birth of a newborn male PN 3100g, in apparent death Apgar 02/02 and cardiopulmonary failure. Failed resuscitation and neonatal death at H1 of life. Examination of the newborn finds a large fontanel and deformed limbs. On the x-ray: the bones of the upper and lower limbs were curved and site of fractures, as well as the presence of fractures at the level of certain ribs.The disease, in the vast majority of cases, is of dominant transmission. There is relative intra-family homogeneity; which means that all people with the family have a form of the same severity. Osteogenesis imperfecta falls within a broad framework of very different aspects in severity. The recent pathophysiological bases, the modes of transmission currently accepted and the classification of any disease observed in parents or children already born, are all elements which should henceforth intervene in the indication and carrying out of the prenatal diagnosis of the disease. Introdução: o diagnóstico diferencial de massa adrenal fetal engloba diferentes condições, incluindo a hemorragia adrenal. Objetivo: relatar um caso de hematoma de adrenal identificado ainda durante o período pré-natal, chamando atenção para o seu diagnóstico diferencial. Material e Métodos: foi realizado o relato do caso e uma revisão da literatura. Os achados clínicos e os resultados de exames complementares foram obtidos por meio de revisão do prontuário da gestante e do recém nascido. A revisão da literatura foi feita utilizando-se descritores Mesh, que foram aplicados à base de dados PubMed/MEDLINE. Esta incluiu somente trabalhos publicados depois de 2007 e nos idiomas português/inglês. A montagem do relato teve duração ao redor de 1 mês. Resultados: a gestante apresentava 36 anos e veio encaminhada para a Medicina Fetal com ultrassom fetal mostrando rim direito multicístico. A avaliação ultrassonográfica realizada no hospital mostrou rim direito com 3 imagens císticas anecoicas, coalescentes e separadas por septações finas no seu polo superior, que mediam 1,9 cm X 1,2 cm X 1,1 cm. O rim esquerdo era normal. A ressonância magnética identificou, em topografia da glândula adrenal direita, imagem cística multisseptada e lobulada medindo 4,8 cm X 4,5 cm X 3,6 cm, causando compressão sobre o rim deste lado. Este achado foi sugestivo de neuroblastoma cístico congênito. A ecocardiografia fetal foi normal. A criança nasceu de parto cesáreo, com 39 semanas de gestação, pesando 3345 gramas e com escores de Apgar de 9 e 10. A ecografia abdominal total, realizada com 2 dias de vida, demonstrou, adjacente ao polo superior do rim direito, grande lesão expansiva líquida multisseptada, não vascularizada ao estudo através do Doppler. O paciente foi submetido posteriormente à cirurgia de retirada da glândula adrenal direita, cuja avaliação anatomopatológica foi compatível com hematoma adrenal. Conclusão: hemorragia de adrenal é um achado até comum em neonatos e tem sido classicamente associada com trauma ao nascimento. Entretanto, sua descrição pré-natal tem sido pouco frequente.
Introdução: a síndrome de Seckel é uma doença genética autossômica recessiva rara caracterizada por nanismo, microcefalia com deficiência mental e uma aparência facial de “cabeça de pássaro”. Objetivo: descrevemos aqui uma paciente com diagnóstico de síndrome de Seckel, apresentando alterações hematológicas. Material e métodos: foi feito o relato do caso junto com uma revisão da literatura. Os achados clínicos e os resultados de exames complementares foram obtidos através da revisão do prontuário da paciente. Os descritores Mesh e a base de dados PubMed/MEDLINE foram utilizadas para a revisão da literatura. Estudos em idiomas português/inglês e publicados após 2007 foram incluídos no trabalho, cujo tempo de montagem foi ao redor de 1 mês. Resultados: a paciente, uma menina, nasceu de parto cesáreo, com 8 meses de gestação, pesando 1220 gramas. Ela chegou a apresentar episódios de crises convulsivas e evoluiu com atraso no desenvolvimento neuropsicomotor. A tomografia computadorizada de crânio evidenciou atrofia cortical e microcefalia. A criança foi hospitalizada aos 3 anos de idade por apresentar quadro de anemia importante, sugestiva de anemia de Fanconi. A pesquisa complementar de quebras cromossômicas induzidas pela mitomicina-C foi normal. O raio-X de mãos e punhos para idade óssea, realizado aos 2 anos, mostrou uma idade óssea de 1 ano e 2 meses (atraso >2 desvios-padrões). Ela foi submetida a uma cirurgia de correção de luxação congênita do quadril com 6 anos de idade. No exame físico, com 10 anos e 8 meses, a paciente apresentava altura de 112 cm (média para 3 anos e 11 meses), microcefalia absoluta e relativa, face pequena com nariz proeminente e micrognatia, prega palmar única e clinodactilia de quintos dedos das mãos. O seu cariótipo foi feminino normal. Conclusão: a soma dos achados foi compatível com o diagnóstico de síndrome de Seckel. A chance de recorrência nestes casos é de 25%. Chama a atenção que anormalidades hematológicas, incluindo anemia, mielodisplasia e leucemia mieloide aguda, também têm sido relatadas em alguns casos desta síndrome.
