A ivermectina (IVM) é um fármaco derivado da avermectina B1 de alta permeabilidade e baixa solubilidade. É utilizado como antiparasitário de amplo espectro e estudos revelam promissoras atividades antiviral e anti-inflamatória. Além disso, formulações e sistemas de liberação têm sido aplicados para otimizar a solubilidade e biodisponibilidade do fármaco. Desse modo, pretende-se destacar o desenvolvimento de novas formulações e sistemas de liberação contendo a IVM tendo em vista não só os benefícios tecnológicos, mas também os proveitos de efetividade diante de distúrbios distintos para os quais o fármaco não foi originalmente aplicado. Os sistemas de liberação têm sido aplicados para otimizar a solubilidade e biodisponibilidade da IVM, reduzir sua citotoxicidade e aumentar o efeito terapêutico em doses menores. Nesse sentido, a literatura revela distintas formulações e sistemas de liberação contendo IVM que visam melhorar suas propriedades físico-químicas, tais como micropartículas, microesferas, lipossomas, microemulsões, gel de formação in situ, formulações de liberação controlada e prolongada, dispersão sólida e micelas mistas. Como potencial agente antiviral, este fármaco inibe a replicação de cepas virais e reduz níveis séricos de proteína viral em estudos in vitro e in vivo; quando incorporada em lipossomas, a eficácia foi elevada e toxicidade reduzida. A IVM demonstrou potencial para aplicação em distúrbios dermatológicos devido a atividade anti-inflamatória, destacando o reposicionamento bem sucedido para o tratamento de rosácea papulopustular. Portanto, reposicionar fármacos mais antigos para tratar doenças comuns ou raras, bem como diante de uma urgente necessidade, torna-se uma ferramenta atraente no âmbito da pesquisa e desenvolvimento de medicamentos.
(1) Background: Riparin-A presents several pharmacological activities already elucidated, such as antimicrobial modulator, antileishmania, anxiolytic, anti-inflammatory, antinociceptive, and antioxidant. Even with important bioactive effects, the applicability of Riparin-A is limited due to its low solubility in water, impairing its dissolution in biological fluids. Thus, the objective of this study was to develop a nanohybrid based on Riparin-A and Laponite to obtain a better dissolution profile and evaluate its cytotoxic potential. (2) Methods: The formation of a hybrid system was highlighted by X-ray powder diffraction, infrared spectroscopy, and thermal analysis. Solubility, dissolution, and cytotoxicity studies were performed; (3) Results: An increase in the solubility and aqueous dissolution rate of Riparin-A was observed in the presence of clay. Diffractometric analysis of the hybrid system suggests the amorphization of Riparin-A, and thermal analyses indicated attenuation of decomposition and melting of the Riparin-A after interaction with clay. Furthermore, the nanosystem did not exhibit cytotoxic activity on normal and tumorigenic lines. (4) Conclusions: These results are promising for the development of the Riparin-A/Laponite nanosystem for therapeutic purposes, suggesting an increase in the range of possible routes of administration and bioavailability of this bioactive compound.
Objetivo: Realizar uma revisão sistemática dos estudos com os análogos sintéticos da Aniba riparia, as riparinas A, B, C, D, E e F, com o intuito de evidenciar os potenciais farmacológicos já elucidados. Metodologia: Utilizou-se três bases de dados para seleção dos artigos a serem incluídos na revisão Embase, Pubmed e Web of Science, com a seguinte estratégia de busca simplificada (Riparin or Aniba riparia), ao final da seleção, resultou em 10 artigos para a elaboração deste trabalho. Resultados: As publicações dos estudos com derivados sintéticos foram identificadas entre os anos de 2014 a 2021. Sendo que a maioria das pesquisas utilizavam a riparina A, seguida pela B, e todos tratavam-se de estudos pré-clínicos. Foram relatados importantes efeitos farmacológicos, como as atividades antibacteriana, antifúngica, anti-inflamatória, antinociceptiva, antitumoral, imunomoduladora, antiprotozoário, antiparasitária, ansiolítica e antioxidante. Discussão: Os estudos realizados com os análogos sintéticos têm colocado as mesmas como possíveis ferramentas terapêuticas, considerando as importantes atividades farmacológicas já relatadas. Conclusão: Contudo, considerando que a literatura já relata seis riparinas sintéticas, o número de artigos publicados voltados para elucidação de possíveis atividades farmacológicas parece reduzido, mesmo sendo promissoras candidatas para o desenvolvimento de pesquisas direcionadas para descobertas de novos fármacos.
Esse artigo objetiva avaliar o risco de ocorrência de quedas relacionado ao uso de medicamentos em pacientes de um serviço hospitalar público, antes e após admissão ao serviço hospitalar, em Teresina-PI. Trata-se de um estudo transversal e descritivo, coletando dados sobre idade, sexo, comorbidades e medicamentos através do formulário de conciliação medicamentosa no intervalo de três meses. A amostra foi constituída por 286 pacientes admitidos nos postos de internação de clínica médica e cirúrgica, tendo idade média de 56,5 (± 17,81) anos, sendo o sexo masculino predominante. Identificou-se maior número de medicamentos prescritos voltados para tratamento de doenças cardiovasculares. O percentual de pacientes com risco de queda relacionado a farmacoterapia antes da admissão era de 21% e na internação passou a ser de 36,7%. Observou-se associação entre idade, polimedicação, presença de possível interação medicamentosa e ter uma ou mais comorbidade com o risco de ocorrência de queda relacionado ao uso de medicamentos nos pacientes internados. A utilização de certos medicamentos é reconhecida como um fator de risco importante e modificável para quedas. Logo, a identificação dos pacientes que fazem uso desses fármacos mostra-se de fundamental importância para prevenir a ocorrência desse evento, através de medidas preventivas que levem em consideração a terapia medicamentosa utilizada.
Esta revisão integrativa teve por objetivo determinar quais são as principais bactérias que possuem correlações positivas entre o aumento do consumo de antimicrobianos e o aumento da resistência bacteriana em âmbito hospitalar. Resumiram-se os artigos, através de revisão integrativa, encontrados nas bases Pubmed® e Embase®, utilizando as seguintes estratégias de busca: (("Microbial Sensitivity Tests"[Mesh]) AND "Hospitals"[Mesh]) AND "Drug Utilization"[Mesh] e 'drug utilization'exp AND 'hospital'exp AND 'antibiotic sensitivity'exp, respectivamente. Sintetizando o autor, ano de publicação, local de estudo, população estudada, metodologia de avaliação do consumo e da relação estatística e as correlações positivas encontradas entre as espécies ou gêneros bacterianos associados com os dados de consumo de uma determinada classe de antimicrobianos. As estratégias de busca encontraram no total 414 artigos, sendo que após a aplicação dos critérios de inclusão restaram 10 artigos que atendiam a todos os critérios utilizados. As principais bactérias ou gêneros bacterianos envolvidos com correlações positivas foram P. aeruginosa (42,6%), E. coli (24,1%) e Acinetobacter spp. (9,3%). A maioria das correlações positivas encontradas foram relacionadas a mesma classe, todavia cerca de 48,2% foram cruzadas. Destacaram-se a P. aeruginosa, E. coli e o gênero Acinetobacter spp., como as bactérias com maiores quantidade de correlações positivas. Bancos de dados internacionais como o GLASS, podem fortalecer a evidência estatística dessa relação, permitindo amostras maiores e mais diversas, contudo eles dependem de cooperação internacional. Esse trabalho pode amparar a farmacovigilância em hospitais destacando correlações já elucidadas, ao qual podem contribuir na otimização dos recursos.
