JOÃO BAPTISTA DOS REIS ANTONIO BEI0 exame do líquido cefalorraqueano na meningite tuberculosa é de suma importância, vindo auxiliar de modo absoluto o diagnóstico da moléstia. Por este motivo, diversos autores, há muito tempo, têm-se preocupado com o estudo de suas alterações nessa entidade mórbida. De modo geral, são concordes em admitir um conjunto de modificações mais ou menos constantes.Assim, tomando por base os estudos de Merritt e Fremont-Smith em 84 casos de meningite tuberculosa (comprovados pela presença de bacilos ao exame direto ou na inoculação em cobaia do liqüido cefalorraquidiano, ou pela autópsia), as seguintes alterações foram encontradas, em média: a pressão geralmente estava aumentada, ao redor de 37 cm de água; o aspecto foi límpido ou opalescente, porém em nenhum dos casos purulento; a côr variou do incolor ao xantocrômico ; houve formação de retículo fibrinoso em todos os casos. 0 número de células se achava aumentado, encontrando-se pleocitose de 25 a 500 por mm 3 , de tipo linfomononuclear predominante. A taxa de proteínas variou de 0,25 a 11,40 g/l, apresentando-se em média ao redor de 2 g/l. A taxa de glicose oscilou entre 0,05 e 0,59 g/l, tendo como valor médio 0,28 g/l. As taxas de cloretos apresentaram-se entre 4,71 e 7,53 g/l, em média 6,08 g/l. A reação de Wassermann foi sempre negativa, a não ser em 2 casos nos quais houve superposição de meningite tuberculosa e neurolues. Frisam os autores que, nos indivíduos luéticos, devido a uma permeabilidade maior da barreira hemoliquórica, pode haver passagem de reaginas sifilíticas, surgindo então uma reação de Wassermann positiva. Afirmam ainda que nenhuma dessas alterações constitui isoladamente sinal patognomônico de meningite tuberculosa, porém, no conjunto, formam uma síndrome liquórica raramente encontrada em outras moléstias. A presença do bacilo de Koch no exame direto e, principalmente, pela inoculação em cobaia, são os únicos dados de certeza diagnostica.Se a pleocitose na grande maioria das vêzes é de tipo linfomononuclear, pode, raramente, ser de tipo predominantemente polinuclear. Outros autores acham que a pleocitose é mais intensa no adulto que na criança. E' fato bastante conhecido a dificuldade técnica da demonstração do bacilo ao exame direto pelos métodos comuns utilizados em laboratório
O cálcio encontra-se no organismo depositado ou dissolvido, porém sempre no compartimento estracelular. No sangue está praticamente todo no plasma, havendo nos glóbulos apenas traços 18 .As primeiras referências sobre a presença de duas frações do cálcio no sangue datam de 1913, quando Rona e Takahashi, empregando método de diálise compensada, descreveram as frações difusível e não difusível, admitindo estar esta última ligada às proteínas plasmáticas 18 . A fração difusí-vel é a farmacològicamente ativa 28 , sofrendo influência do parathormônio. Ela atua sobre o coração de rã, podendo ser, por este meio, dosada biològi-camente com precisão satisfatória, conforme afirmam os autores do méto-do 20 ' 23 . A outra fração, não difusível, ligada às proteínas, não interfere no metabolismo cálcico, variando apenas o seu teor conforme o das proteínas. McLean e Hastings 20 , entretanto, afirmam a existência de um equilíbrio entre as duas frações sujeito à lei de ação das massas.Sendo o LCR normal muito pobre em proteínas, o valor da fração não difusível é mínimo. Cameron e Moorhouse 3 , Cantarrow 5 e Lafontaine 14 consideraram a calciorraquia como expressão do cálcio difusível do soro. Este conceito se baseia na hipótese de se comportar a barreira hemoliquórica como obedecendo unicamente a leis fisicoquímicas de membrana. Klotz e Elmaleh afirmam a existência de um mecanismo suplementar, considerando o LCR como um dializado do plasma 13 .Merrit e Bauer 19 verificaram que, em casos de hiperparatiroidismo, com grande aumento da fração difusível do cálcio sérico, a calciorraquia permanece dentro dos limites de normalidade. Estudando experimentalmente este ponto, obteve Lafontaine, injetando parathormônio em cães, alteração da calciorraquia, porém muito discreta e muito menos nítida que no sangue. Klotz e Elmaleh 13 , retirando cirürgicamente as paratiróides de cães, encontraram pequena queda do cálcio do LCR (aproximadamente de 1 mg%), em contraste Trabalho da Clínica Neurológica da Escola Paulista de Medicina (Prof. Paulino W.
