Telemedicine gives a safe and effective way of providing healthcare. During the COVID-19 pandemic, it was possible to offer teleconsultations in primary care (Primary Care Teleconsultation-PCT). The study aimed to present an analysis of the PCTs served in the years 2020–2021 in the field of primary care in Poland to determine how the COVID-19 pandemic contributed to the development of telemedicine in primary care in Poland. The database, containing a list of medical services provided remotely obtained from the National Health Fund, was analyzed. Economic and tax indicators obtained from the Ministry of Finance were also analyzed. Personal Income Tax (PIT) value was used as an indicator of household wealth, and the Corporate Income Tax (CIT) was used as an indicator of economic activity in individual counties for 2019. Along with the COVID-19 pandemic, patients as healthcare beneficiaries can take advantage of previously unserved telemedicine services as part of primary care. The data analysis showed that, along with the introduced recommendations and restrictions in connection with the pandemic, the number of teleconsultations in 2021 increased compared to 2020. In response to the pandemic, an educational campaign targeted older patients. These indicate the most significant percentage of PCTs among patients aged 70 and older. The study shows that the awareness barrier in implementing services for the elderly population decreased significantly. There was a clear correlation between the increase in PCTs and patient age.
StreszczenieWstęp: Warunkiem skuteczności i/lub efektywności zarządzania jest znajomość jego przedmiotu i celu. Szczególnie dotyczy to jakości świadczeń zdrowotnych, która jest traktowana jako stopień zgodności efektu (leczenia) z założonym celem (określonym/pożądanym stanem zdrowia). Celem badania było poznanie sposobu postrzegania jakości szpitalnych świadczeń zdrowotnych oraz definiowania, wybierania i hierarchizowania czynników, które tę jakość determinują. Materiał i metody: Ankieta została wysłana do 836 osób pełniących funkcje zarządcze w publicznych podmiotach leczniczych na terenie województwa łódzkiego. Odpowiedź uzyskano od 122 respondentów. Wyniki: Tylko 22 (18,49%) respondentów przedstawiło definicję jakości. Przeważało rozumienie jakości jako procesu mającego na celu spełnienie oczekiwań pacjentów i ich zadowolenie. Prawie wszyscy respondenci (96,64%) stwierdzili, że kompetencje personelu medycznego wpływają na jakość świadczeń zdrowotnych. Prawie 64% kadry zarządczej nie zgadza się z opinią, że liczba personelu medycznego nie wpływa na jakość świadczeń zdrowotnych świadczonych przez podmiot leczniczy. W opinii respondentów wzrost środków na leczenie średnio o 46% spowodowałby znaczący wzrost jakości świadczeń. Ponad połowa (66,76%) respondentów odpowiedziała, że oferowane świadczenia pokrywają potrzeby pacjentów średnio w 82%. Również ponad połowa (56,52%) stwierdziła, że relacje z przełożonym wzmacniają motywację do większego zaangażowania w wykonywane obowiązki, a tym samym wpływają na jakość świadczeń. Mniej niż połowa (42,61%) respondentów uważa, że na jakość świadczeń zdrowotnych wpływa system motywacyjny personelu medycznego. Wnioski: Wyniki badania wskazują na potrzebę jednoznacznego określenia/zdefiniowania jakości świadczeń zdrowotnych przez osoby pełniące funkcje zarządcze w podmiotach leczniczych. Introduction and objective. There is a significantly higher risk of lipid disorders occurrence, including atherogenic dyslipidemia in women after menopause than it is in general population. The aim of the work was to investigate the correlation between health behaviours and the occurrence of lipid disorders in women after menopause working in agriculture. Material and methods. The study was conducted in years 2015-2016 and included 843 post-menopausal women working in agriculture. The following were used: a questionnaire including socio-demographic data, laboratory lipid tests, inventory of health behaviours. The following were estimated: logistic regression models for serum lipids concentration versus frequency of health behaviours in the examined women. Results. Adverse lipid profile was found in over a half of post-menopausal women working in agriculture, whereas the frequency of health behaviours were estimated at the average level, although the frequency of correct eating habits and health practices was significantly lower than preventive behaviours and positive psychological attitudes. A correlation was found between the frequency of health behaviours and the occurrence of lipid disorders in women after menopause working in agriculture: more frequent health practices co-existed with the lower concentration of total cholesterol and a higher concentration of HDL-cholesterol, more frequent preventive behaviours co-existed with lower concentration of LDL-cholesterol. Women with higher concentration of triglycerides undertook pro-health practices relatively more often. Conclusions. The study revealed a high prevalence of lipid disorders in postmenopausal women working in agriculture. More effective health education programmes are necessary in the area of reduction the risk factors of CVD in the population of women working in agriculture.
Introduction and Objective. Female physiological urinary incontinence may occur in the reproductive age, but the incidence of uncontrolled urination increases in the postmenopausal period. Urinary incontinence is playing an increasingly important social and economic role. In recent years, there has been a marked increase in the number of patients affected by incontinence symptoms presenting to the doctor due to urinary tract diseases. The main aim of the study was to investigate the prevalence of urinary incontinence among patients receiving medical care in selected institutions located in the Łódz region of Poland, and factors increasing risk of the disease. Materials and method. The study was conducted between January -September 2021 among women receiving health care in selected facilities in the Łódż region. The study was conducted with the use of the authors' original, anonymous survey questionnaire consisting of 43 questions. The patients were asked to complete the survey questionnaire while waiting for an appointment at the doctor's surgery. Completed survey questionnaires were returned by 117 women. Results. Of the 117 interviewed women, the majority, i.e. 63 people (53.9%), admitted that they had noticed the problem of incontinence. Of these 63 women, the largest number, i.e. 25 (f=0.4), admitted to losing urine 2-3 times a week. Conclusions. In view of the high prevalence of urinary incontinence among the studied women, there is a need to take action to disseminate knowledge about risk factors and methods of prevention of incontinence.
Background Cardiovascular diseases, classified as civilization diseases, constitute the most common cause of death among both women and men. Metabolic syndrome is a set of factors that predispose to the occurrence such diseases. Importantly, can be partly prevented by reducing modifiable risk factors. Objective The aim of the study was to uncover the distribution of selected risk factors for cardiovascular diseases among patients utilising primary health care in the Lodz Voivodeship. Material and methods An original online questionnaire consisting of 32 questions was distributed. The study involved 164 inhabitants of the Lodz Voivode-ship who use primary health care facilities. Inclusion criteria: residents of the Lodz Voivodeship, aged between 30 and 60 years as well as informed consent to participate in the study. Results Hypertension affected 26.2% of participants. In the age range 30–40 years: 4.1% of participants, 41–50 years: 28.3%, and 51–60 years: 61.4% of participants. Diabetes affected 21.3% of participants. 64% of respondents had a BMI above normal. 28.7% of respondents did not engage in physical activity. Among the surveyed, smokers accounted for 37.8%. Among university diploma holders, smokers accounted for 28.2%, and those with non-university education – 54.1%. Conclusions The incidence of hypertension in the population of Lodz increases with age. The prevalence of dyslipidemia was independent of gender but dependent on education – higher among respondents with non-university education. Most of the inhabitants of the Lodz Voivodeship were physically active. Moreover, the most frequently chosen forms of physical activity were walking and Nordic walking, and less frequently – aerobics. The prevalence of smoking was similar among women and men. Smokers were more likely to be people with non-university education. The need of taking measures to raise awareness of cardiovascular risk factors, promote healthy lifestyles, and combat obesity has been identified.
