ResumoO presente artigo é fruto de tese cujo objetivo geral foi propor um modelo de diretivas antecipadas de vontade para o Brasil. Para tanto, realizou-se uma revisão de literatura sobre as diretivas antecipadas nas Américas e na Europa, especialmente nos Estados Unidos da América e na Espanha, e entrevistas semiestruturadas com médicos oncologistas, intensivistas e geriatras de Belo Horizonte-MG. Percebeu-se que o modelo brasileiro deve se distanciar dos padrões de formulários utilizados em muitos estados norte-americanos e províncias espanholas, visando deixar espaço para a subjetividade de cada paciente. Conclui-se, assim, que o modelo proposto tem o condão de auxiliar o cidadão que deseja fazer sua diretiva antecipada, bem como os médicos que desejam apresentar essa possibilidade para seus pacientes, mas deve ser sempre utilizado como guia e não como um modelo fechado às peculiaridades de cada situação concreta. Por lo tanto, se realizó una revisión bibliográfica sobre las directivas anticipadas en las Américas y en Europa, especialmente en Estados Unidos y España, y las entrevistas semiestructuradas con los médicos oncólogos, intensivistas y geriatras de Belo Horizonte-MG. Se ha percibido que un modelo brasileño debe alejarse de los modelos de formularios utilizados en muchos estados norteamericanos y provincias de España, con el fin de dejar espacio para la subjetividad de cada paciente. Se concluye, por tanto, que el modelo propuesto tiene la capacidad de ayudar a los ciudadanos que desean hacer su directiva anticipada, así como los médicos que desean ofrecer esta opción a sus pacientes, pero siempre debe ser utilizado como una guía y no como un modelo cerrado a las peculiaridades de cada situación concreta. Abstract Advanced directive: a brasilian modelThis article is the result of the doctoral thesis, whose general objective was to propose a model of advance directives to Brazil. Therefore, we carried out a literature review on advance directives in America and Europe, especially in the United States and Spain, and semi-structured interviews with medical oncologists, intensivists and geriatricians in Belo Horizonte-MG. It was realized that a Brazilian model should distance themselves from form models used in many American states and provinces in Spain, in order to leave room for the subjectivity of each patient. We conclude, therefore, that the proposed model has the ability to assist citizens who want to advance their policy, as well as doctors who wish to provide this option for their patients, but it should always be used as a guide and not as a closed model to the peculiarities of each situation.

Revista de Saúde Pública ResumoObjetivo O processo de envelhecimento populacional brasileiro tem sido acompanhado por transformações epidemiológicas e sociais que, em outros países, aumentaram a demanda por instituições de longa permanência. Nesse sentido, desenvolveu-se estudo sobre dinâmica da institucionalização de idosos através da análise da oferta de leitos e características demográficas dos residentes em asilos. Métodos Foi realizado levantamento dos asilos de idosos do Município de Belo Horizonte, MG. Foram registradas datas de nascimento e admissão de 1.128 residentes de 33 dos 40 asilos em funcionamento no município. A lotação foi calculada através da capacidade informada e da taxa de institucionalização, segundo dados da Pesquisa Nacional por Amostra de Domicílios (PNAD). Resultados As taxas de institucionalização de idosos (65+) do município foram 0,9% (mulheres) e 0,3% (homens). A lotação mediana dos asilos era de 92%. Dentre os 1.128 residentes arrolados (92,5% do total), as mulheres (81%) apresentavam maiores médias de idade (76,4 x 70,4 anos; p=0,00) e estada (5,6 x 4,5 anos; p=0,01). Discussão/Conclusões Elevada lotação, listas de espera nos asilos filantrópicos (85% do total) e rígidos critérios para admissão (metade recusava idosos demenciados ou dependentes) sugerem que a baixa taxa de institucionalização se deve à escassez de vagas. A predominância de idosas reflete a prevalência de viúvas ou separadas na comunidade (66% contra 76% de homens casados). A proporção de homens adultos (31% < 65 anos) sugere dificuldade de se auto-sustentarem após a viuvez. Elevada taxa de mortalidade (24% durante 20 meses de acompanhamento de amostra aleatória de 263 residentes), incompatível com o rigor dos critérios de admissão, determina a baixa mediana de estada (3 anos). Os dados refletem o anacronismo de um sistema não direcionado à manutenção dos idosos em seus próprios domicílios. To study characteristics of neurological disorders in HIV/AIDS patients and their relationship to highly active antiretroviral treatment, a cross-sectional study was conducted in an infectious disease public hospital in Belo Horizonte, Brazil, between February 1999 and March 2000. Of the 417 patients enrolled, neurological disease was observed in 194 (46.5%) and a new AIDS-defining neurological event developed in 23.7% of individuals. Toxoplasmosis (42.3%), cryptococcosis meningitis (12.9%) and tuberculosis (10.8%) were the most common causes of neurological complications. The majority (79.3%) of patients were on highly active antiretroviral treatment and these individuals using HAART showed higher CD4 cell counts (p = 0.014) and presented stable neurological disease (p= 0.0001), although no difference was found with respect to the profile of neurological complications. The neurological diseases continue to be a frequent complication of HIV/AIDS and infections are still its main causes in Brazil, even in the highly active antiretroviral treatment era. RESUMOCom o objetivo de estudar as doenças neurológicas em pacientes HIV/AIDS e sua relação com a terapia anti-retroviral altamente ativa, foi realizado estudo transversal em hospital público de doenças infecciosas de Belo Horizonte, Brasil, no período de fevereiro de 1999 a março de 2000. Doença neurológica foi observada em 194 (46,5%) dos 417 indivíduos incluídos e um novo episódio de doença neurológica definidora de AIDS ocorreu em 23,7% pacientes. Toxoplasmose (42,3%), criptococose (12,9%) e tuberculose (10,8%) foram as principais causas de complicações neurológicas. A maioria dos pacientes estava em uso de terapia anti-retroviral altamente ativa (79,3%) e esses indivíduos apresentaram maiores contagens de linfócitos CD4 (p = 0,014) e maior freqüência de doença neurológica clinicamente estável, embora não tenha havido diferença no perfil etiológico das complicações neurológicas. As doenças neurológicas continuam sendo causas freqüentes de complicações da infecção pelo HIV/AIDS no Brasil, e a despeito da terapia anti-retroviral altamente ativa, as infecções são ainda a principal etiologia das doenças do sistema nervoso. Rickettsial diseases except those belonging to spotted fever group rickettsioses are poorly studied in South America particularly in Brazil where few epidemiological reports have been published. We describe a serosurvey for Rickettsia rickettsii, R. typhi, Coxiella burnetii, Bartonella henselae, B. quintana, and Ehrlichia chaffeensis in 437 healthy people from a Brazilian rural community. The serum samples were tested by indirected micro-immunoflourescence technique and a cutoff titer of 1:64 was used. The seroprevalence rates for R. rickettsii, R. typhi, C. burnetii, B. henselae, B. quintana, and E. chaffeensis were respectively 1.6% (7 samples (WHO 1993, Rozental et al. 2002, Walker 2004a. Until recently the rickettsiales order comprised the genus Rickettsia, Coxiella, Bartonella, Orientia, Anaplasma, and Ehrlichia (Raoult & Roux 1997) and despite of the resulting withdraw of some of them from this order especially due to 16S rRNA studies, they all can be functionally considered rickettsial agents (Raoult & Roux 1997, Walker 2004b.Most of the available publications of Brazilian rickettsial infections have focused on spotted fever group rickettsioses especially R. rickettsii infections (Lemos et al. 1994, Del Guercio et al. 1997, Rozental et al. 2002, Galvão et al. 2003. This issue has been extensively reviewed recently by Galvão (2004). Despite of the fact that new rickettsial agents have been described more recently in Brazil (Oliveira et al. 2002), the whole scenario of rickettsial infections still have black holes reinforcing the nonexistence concept for such infections in this part of the world.The Piau county as well as the southern part of the state of Minas Gerais had been considered free of any rickettsial diseases until 1994 when a family cluster of Brazilian spotted fever was reported (Hallack et al. 1996).
