The fast spread of COVID-19 has overcrowded Public Health Systems facilities in major countries due to the large number of seriously ill patients, particularly those requiring admission to intensive care units. Reducing viral load, along with other recommended epidemiological measures, such as social distancing and home confinement, can in time significantly help to reduce the infection R0 (Basic Reproductive Rate) and then mitigate disease burden. Early negativization or otherwise reduction of the viral load can potentially diminish disease severity, resulting in a better-controlled public health response, avoiding collapse of healthcare systems. Nitazoxanide, a widely used thiazolide approved by the FDA as an antiparasitic drug, also approved in Brazil for Norovirus and Rotavirus treatments, has an excellent safety record for a variety of indications. Nitazoxanide exhibits activity in vitro against MERS-CoV and other coronaviruses; and a specific antiviral effect (in micro molar doses) against SARS-CoV-2. The objective of this study was to evaluate the efficacy and safety of Nitazoxanide in reducing the SARS-COV 2 viral load within 7 days of treatment in respiratory samples from COVID-19-infected patients with mild to moderate disease, compared to placebo. An interim analysis showed that the ratio of patients with a viral load reduction ≥ 35% from baseline up to day 7 of treatment was significantly greater for Nitazoxanide compared to placebo (47.8% vs. 15.4%; Δ 34.6%; 95% CI: 64.7; 4.6: p = 0.037).
Avoiding relatives' presence while performing cardiopulmonary resuscitation is the most frequent choice made by the surveyed physicians who treat critical Argentine patients. The main causes for discouraging family presence during cardiopulmonary resuscitation or other critical procedures include the following: risk of misinterpretation of the physician's actions and/or words; risk of a relative's decompensation; uncertainty about possible reactions; and interpretation of the relative's presence as negative.
CC performance quality decreased and fatigue was frequent before 10 minutes had elapsed on a neonatal simulator. Provider fatigue was associated with both lack of aerobic activity and BMI ≥ 25. Our findings support the need for guidelines requiring frequent rotation of CC providers during prolonged neonatal resuscitation.
Background: Cardiovascular Disease is a leading cause of preventable death. Cardiovascular risk factors’ identification is the cornerstone for effective and early interventions decreasing the frequency of acute health-threatening events. Since adolescence and youth are very vulnerable stages to develop risky habits, we decided to run this study in the Multidisciplinary Universitary Camp for Research and Service. Methods: A descriptive cross-sectional study was performed in the Multidisciplinary Universitary Camp for Research and Service in which 450 medical students from Colombia were assessed. After a probabilistic random simple sampling (n=50), we applied the World Health Organization test and Finnish Risk Score to calculate Cardiovascular and Diabetes Mellitus risk, respectively. We characterized the population by sociodemographic variables and anthropometric measurements. Results: The study shows that from 18 participating universities (n=50), the mean age of the participants was 21.14 years (SD 7.3) of whom 40% were male and 60% were female. Overall, 92% have a low risk of cardiovascular disease, 6% are at moderate risk and 2% are at high risk. 92% have low risk of diabetes mellitus and 8% are at moderate risk of having diabetes mellitus in the long term. Conclusion: The early identification of and intervention on risk factors could decrease significantly the onset of acute health-threatening cardiovascular pathologies. As medical students, adolescents and young adults are at risk of developing unhealthy habits which increase the incidence of cardiovascular disorders. The use of anthropometric measures and validated risk score scales is an appropriate way to get evidence for starting early interventions.
Objective To compare the efficacy of video-assisted self-directed neonatal resuscitation skills course with video-assisted facilitator-led course. Methods This multicenter, randomized, blinded, non-inferiority-controlled trial compared two methods of teaching basic neonatal resuscitation skills using mask ventilation. Groups of novice providers watched an instructional video. One group received instructor facilitation (Ins-Video). The other group did not (Self-Video). An Objective Structured Clinical Exam (OSCE) measured skills performance, and a written test gauged knowledge. Results One hundred and thirty-four students completed the study. Sixty-three of 68 in the Self-Video Group (92.6%) and 59 of 66 in the Ins-Video Group (89.4%) achieved post-training competency in positive pressure ventilation (primary outcome). OSCE passing rates were low in both groups. Knowledge survey scores were comparable between groups and non-inferior. Conclusions Video self-instruction taught novice providers positive pressure ventilation skills and theoretical knowledge, but it was insufficient for mastery of basic neonatal resuscitation in simulation environment.
