The present study was undertaken to determine the incidence and related risk factors for coronary heart diseases and hypertension in the rural population of Kheda district, Gujarat (India). The observations from the first five years of this ongoing project (May 1987-May 1992) are described in this paper. Out of an initial sample of 750 individuals in the age group 30-62 years, who were selected by stratified random sampling, 714 persons (males = 429; females = 285) were actually studied, after excluding those suffering from coronary heart diseases (CHD). Initially, all the included subjects were examined clinically and appropriate laboratory investigations were done. A detailed socio-economic history was also obtained. Subsequently all of them were followed up and biannual clinical and laboratory investigations were performed. Cases of CHD were diagnosed according to the recommendations of the New York Heart Association. The overall five-year incidence of CHD was 25.17 per thousand persons. The incidence in males was 3 times higher than in females. More males suffered from myocardial infarction (MI), while in the females the incidence of sudden death was higher (33.3%). The average yearly mortality rate due to CHD was 2.46 per thousand persons. CHD was significantly associated with increased blood pressure (both diastolic and systolic), smoking, and family history of heart disease, and was weakly associated with body weight (p = 0.06).
In the time of unavailable pharmaceutical measures, efforts for preventive control of nCovid 19 was on nonpharmaceutical methods like hand wash, facemask and social distancing. Though Government and social media were actively contributing, there was paucity of feedback regarding actual practice of implemented preventive measures in community. Present survey was conducted during July 2020, lockdown period, to correlate knowledge and practice of using facemask as preventive barrier in nCovid 19 infections. Our observations point that though knowledge regarding use of facemask was appropriately delivered, in reality it was not practiced or practiced incorrectly by public. Result of present survey point that effective ness of preventive measure depends on its implementation and obtaining feedback to achieve the goal.
Satisfaction means achieving one's expectation regarding service or product. Pateint's satisfaction means fulfilling their expectations during medical care. Total 211 patient's response towards medical care at civil hospital was assessed by direct approach exit interview method. Present study finds more than 60% responders were satisfied regards to reception and admission procedure. More than 70% were not happy with quality of food, cleanliness and rules of the hospital. Above 52% respoonders disapproved behaviour of ward and sanitary attendents but 70% plus responders appreciated other health care workers in their soft skill. Over 60% responders expressed satisfaction in medical treatment and preventive education for disease they suffer. Satisfaction level was low for discharge protocol and time.Overall impression of the present study display satisfactory response towards services and quality of health care provided by the Civil Hospital. It identify a few shortfalls, which need to be attended for improving quality of health care services and to maintain acceptable standard of the hospital. Though survey evaluates quality of health care services of Civil hospital from patients' point of view but, it facilitate identification of problem and may provide feedback to management for quality improvement towards maintaining standard of Hospital Services.
