Atualmente, na constante busca de minimizar o tempo de intemação hospitalar e melhorar a recuperação no pós-operatório, várias altemativas cirúrgicas têm sido aventadas. Iniciamos o uso da estemotomia parcial como acesso para diversos tipos de operações cardíacas. O presente trabalho tem como objetivo apresentar a evolução hospitalar dos pacientes operados através desta técnica. No período de novembro de 1996 a março de 1997, estudamos 51 pacientes operados através da esternotomia parcial. A idade média foi de 46,8 anos, sendo 26 pacientes do sexo feminino. Os procedimentos mais realizados foram o tratamento cirúrgico valvar e as revascularizações miocárdicas. O acesso utilizado para os pacientes com lesões valvares foi a esternotomia parcial em "T" invertido no segmento superior do esterno; para outras lesões, uma esternotomia em "T" no segmento inferior e, pela dificuldade técnica imposta nas re-operações e nos procedimentos múltiplos, utilizou-se uma terceira variação, que foi a esternotomia parcial em "H" deitado, estendendo-se no corpo esternal do primeiro ao quarto espaço intercostal. O comprimento médio da incisão de pele foi de 9,9 cm. Foram realizadas 19 incisões em "T", 17 em "T" invertido e 15 em "H" deitado. O tempo médio de ventilação mecânica foi de 2,8 horas, de permanência na UTI de 31,5 horas e de internação hospitalar de 5,9 dias. Não houve complicação diretamente relacionada com o acesso e em apenas 1 caso foi necessária a conversão para a esternotomia total. Analisando a evolução destes 51 pacientes, pudemos concluir que a esternotomia parcial é um acesso seguro para o tratamento cirúrgico de diversas cardiopatias, isoladas, associadas ou re-operações. Traz um resultado estético favorável e facilita a recuperação no pós- operatório, devendo fazer parte do repertório de todo o cirurgião cardiovascular.
Recently, in the constant search for shortening hospital stay and emproving postoperative recovery, many surgical alternatives have been used. We started the use of a partial sternotomy as an approach to different types of cardiac operations. This paper aims to show the hospital outcome of patients submitted to this technique. From November 1996 to March 1997, 51 patients who underwent partial sternotomy were studied. The mean age of the patients was 46.8 years with 26 females. The most common procedures were valve surgeries followed by myocardial revascularization. The approach used in the patients with valve pathologies was a partial sternotomy in an inverted "T" incision in the superior segment of the sternum, for other pathologies a sternotomy in a "T" incision in the inferior segment was the choice and due to technical difficulties imposed by reoperations and in multiple procedures a third variation, partial sternotomy in an "H" incision was used. Tendo em vista as complicações decorrentes com uso das próteses valvares, maior tem sido o empenho em se conservar a valva mitral. Objetivo: O presente trabalho analisa a plástica da valva mitral com emprego do anel de Gregori-Braile em 66 pacientes operados consecutivamente entre outubro de 1989 a outubro de 1995. Casuística e Métodos: Quarenta e cinco (74,1%) pacientes eram do sexo feminino e 17 (25,7%) pacientes do masculino. A idade média foi de 32,9 anos. Moléstia reumática esteve presente na maioria dos casos (49 pacientes) e insuficiência mitral em 38 (57,5%) pacientes. O tempo de evolução foi de 2,560 meses/pacientes com média de 38,8 meses e o acompanhamento foi realizado em 64 (96,9%) pacientes. Os métodos de avaliação foram: quadro clínico (GF), sopro sistólico no foco mitral (SSFM) e estudo Dopplerecocardiográfico no período pré e pós-operatório. As técnicas empregadas sobre a valva mitral foram: implante do anel, mobilização das cúspides e cordas tendíneas e restrição da mobilidade valvar. Encurtamento das cordas tendíneas foi realizado em 44 (66,6%) pacientes. Os procedimentos associados foram: redução do AE (8 casos), troca valvar aórtica (3 casos) e cirurgia de Cox (3 casos). (1,5%) uma paciente faleceu sete dias após a operação por tromboembolismo pulmonar. Resultados: Houve importante melhora do grau funcional após a operação. No pré-operatório 41 (62,1%) pacientes estavam no GF III e 23 (34,8%) pacientes no GF IV. Após a plástica, 53 (80,3%) pacientes se encontravam no GF I e 8 (12,1%) pacientes no GF II. O mesmo ocorreu com o SSFM, pois em 92,4% dos operados ele era ausente ou de + de intensidade. O GF e SSFM melhoraram de maneira significativa (p < 0,001). Quanto ao estudo Dopplerecocardiográfico, a média do diâmetro diastólico do VE era de 5,96 cm no PO (p < 0,001). A média do tamanho do AE era de 5,67 cm no pré e 4,65 cm no PO (p < 0,001). O diâmetro da aorta ascendente era de 2,97 cm no pré e 3,13 cm no PO (p < 0,01). A média do encurtamento percentual era de 35,38% no pré e 34,12% no PO ( sem diferença estatística). A média da área valvar no pré e PO era de 1,7 cm2 e 2,43 cm2 (p < 0,003), respectivamente. A média do gradiente de pressão através da valva mitral era de 11,19 mmHG no pré e 5,58 mmHG no PO (p < 0,003). Três (4,5%) pacientes faleceram no pós-operatório tardio. Após 72 meses, 95,5% dos pacientes estavam livres de mortalidade, 96% livres de reoperação (6,0%) e 98,4% livres de tromboembolismo. Conclusão: A plástica da valva mitral é procedimento reprodutível, a melhora do grau funcional é significativa estatisticamente, é bastante eficaz no tratamento da insuficiência e dupla lesão mitral e deve ser realizada sempre que possível.

Loures D R -Remodelamento do ventrículo esquerdo pela técnica da endoventriculoplastia com exclusão septal: experiência inicial. Rev Bras Cir Cardiovasc 2000; 15(4): 302-7. RESUMO: Objetivo: Neste trabalho apresentam-se os resultados iniciais do tratamento cirúrgico de aneurismas anteriores do ventrículo esquerdo (Aneu VE), pela técnica de Dor, endoventriculoplastia circular, com exclusão septal, baseada nos conceitos de Hutchins & Bulkley, da geometria do VE e da sua reconstrução geométrica, e nos da operação da redução circular de Jatene.Casuística e Método: No período de abril a outubro de 1999, foram realizados 6 procedimentos para a correção de Aneu VE, com a técnica de Dor. O tempo entre o infarto de parede anterior e a operação variou de 8 dias a 10 meses, sendo que 3 pacientes se encontravam na classe III, 2 na classe IV e 1 na classe I da NYHA. Quatro pacientes eram do sexo masculino e a idade média do grupo era de 58,3 anos. No estudo hemodinâmico pré-operatório, a fração de ejeção global variou de 27 a 46% (média de 37,2%), o volume sistólico final de VE de 64,4 a 135,3 ml (média de 107,9 ml) e o volume diastólico final de VE de 132,9 a 236,4 ml (média de 183,8 ml). A operação foi associada à retirada de trombos do VE em 3 casos, a revascularização coronária concomitante em 5 casos, com o uso de 10 enxertos venosos e 3 arteriais (2,2 enxertos/paciente) e uma endarterectomia. A parede anterior foi revascularizada em 9 casos e as paredes, lateral e inferior, em 2 cada. Os tempos médios de pinçamento aórtico e circulação extracorpórea foram, respectivamente, 44 e 110 minutos.Resultados: O tempo médio de permanência hospitalar pós-operatória foi de 7,7 dias. Os pacientes foram seguidos de 5 a 31 semanas, sendo que todos se encontram na classe funcional I; em 3 casos o cateterismo cardíaco foi realizado e nos outros 3, um ecocardiograma bidimensional, para avaliação da fração de ejeção global, que variou de 42,5 a 60% (média de 49,4%), com melhora de 32,8% em relação à inicial.Conclusão: Diante destes resultados, pode-se considerar que esta seja mais uma técnica para a melhoria da função e da nova geometria do VE, em casos de correção cirúrgica de Aneu VE.
Desde os primeiros relatos sobre aneurismectomia do ventrículo esquerdo, vários aspectos foram estabelecidos. Existem controvérsias relacionadas à técnica ideal de reconstrução ventricular, seus efeitos na morfolofia, função ventricular, estado sintomático pós-operatório e sobrevida a longo prazo. Este estudo visa levantar a casuística do Serviço, observando as principais indicações e tratamento cirúrgico, o estado sintomático pré e pós-operatório e a sobrevida imediata e tardia dos pacientes. Foram avaliados 12 portadores de aneurisma do ventrículo esquerdo, 9 masculinos, idade média de 60 anos, cujas principais indicações cirúrgicas foram angina (58%) e angina + insuficiência cardíaca (42%). O cateterismo cardíaco demonstrou acinesia/discinesia segmentar em 92% dos pacientes, aneurisma apical ou ântero-apical em 83% e doença coronária obstrutiva em 100%, acometendo 3 ou mais vasos em 75%; a artéria interventricular anterior foi mais comprometida (29%). A fração de ejeção variou de 32% a 66% e o encurtamento percentual de 15% a 36%. Todos foram submetidos a revascularização do miocárdio, 9 sofreram aneurismectomia do ventrículo esquerdo, 7 com sutura linear e 2 com reconstrução geométrica. O tempo médio de circulação extracorpórea e clampeamento aórtico foi 96 e 50 minutos para sutura linear e 180 e 86 para reconstrução geométrica. As principais intercorrências pós-operatórias foram síndrome de baixo débito cardíaco e fibrilação atrial (16%). A mortalidade hospitalar foi de 16%. Os pacientes encontram-se com seguimento médio de 15 meses, sobrevida geral de 75%; estão livres de angina 89%, aqueles operados por ICC estão em classe II. Esses fatores melhorando a qualidade de vida justificam a aneurismectomia.
Since the first reports about left ventricular aneurysmectomy, many features have been established, but there are controversies concerning the ideal technique of left ventricular reconstruction and their effects on morphology, ventricular function, symptomatic state and longevity. This study was designed to evaluate the cases of left ventricular aneurysm relating the main surgical indications and treatment and the patients' symptomatic state before and after operation. The study involves 12 patients with left ventricular aneurysm, nine males, mean age 60 years, whose main indications for surgery were angina (58%) and angina plus congestive heart failure (42%). Ventricular cineangiograms revealed an akinetic/dyskinetic segment in 92% of patients, apical or anteroapical aneurysm in 83% and coronary artery disease in 100%, affecting 3 or more vessels in 75%, the left anterior descending artery was most affected (29%). The ejection fraction ranged from 32% to 66%. All patients underwent coronary artery bypass grafting, nine had left ventricular aneurysmectomy, seven through linear closure and 2 through geometric reconstruction.
