El carcinoma de células de Merkel es un tumor raro neuroendocrino, altamente agresivo con altas tasas de mortalidad asociadas a sus metástasis a distancia. Es más frecuente en pacientes inmunosupresos, edad avanzada y exposición a luz ultravioleta. La mayoría de asocian con Poliomavirus sin embargo existen casos sin presencia viral. Se recomienda la resección quirúrgica con márgenes extensos cuando se encuentra localizado y en casos seleccionados la biopsia de ganglio centinela. Es una neoplasia quimiosensible y radiosensible. Existen avances terapéuticos inmunológicos para casos avanzados.
La enfermedad de Kawasaki es una vasculitis aguda, autolimitada, actualmente de etiología desconocida caracterizada por una respuesta inflamatoria en pacientes genéticamente predispuestos a padecer la enfermedad. Afecta principalmente a niños menores de 5 años y su diagnóstico se realiza basado principalmente en las características clínicas y con apoyo de estudios de laboratorio o con la demostración de aneurismas coronarios. Esta patología es la principal causa de enfermedad cardiaca adquirida en la niñez, debido a que la vasculitis afecta a vasos de mediano calibre, principalmente las arterias coronarias, lo cual desencadena formación de aneurismas coronarios con posterior trombosis. Se debe de realizar un diagnóstico temprano para poder dar el tratamiento adecuado y evitar consecuencias fatales por el desarrollo de aneurismas coronarios, los cuales se desarrollan en un 25% de los pacientes que no reciben tratamiento.
El microbioma cutáneo está compuesto por una variedad extensa de microorganismos, este se ve modificado por diversos factores y es dinámico, ya que existen variaciones a lo largo de la vida. Los microorganismos y el huésped viven bajo una relación de simbiosis, en el momento que este sistema se rompe, se produce la disbiosis y esta tiene una influencia importante en la dermatitis atópica. Se reporta que existe un alta colonizacion de los pacientes enfermos por el Staphylococcus aureus y esto concuerda con la mayoría de los estudios donde se determina el microbioma de piel lesional y no lesional de pacientes con dermatitis atópica. Otros factores como las mutaciones en la proteína filagrina aumentan la susceptibilidad de colonización por S.aureus. Por otro lado, también se han encontrado agentes microbianos con capacidades inhibitorias sobre los microorganismos patógenos y se les ha atribuido un rol protector; este hallazgo motivó a algunos autores a utilizarlos como tratamientos alternativos e inclusive en uso combinado con terapia tópica como los emolientes. Adicionalmente, se reporta el restablecimiento de la microbiota con tratamientos convencionales como los humectantes, corticoesteroides, inhibidores de calcineurina y fototerapia. Múltiples estrategias se han estudiado con el fin de restablecer la biodiversidad cutánea en un intento de buscar tratamientos eficaces que actúan sobre la modificación de los procesos de disbiosis asociados con esta enfermedad.
