O transtorno esquizotípico de personalidade (TEP) é definido clinicamente no DSM-IV 1 como um padrão invasivo de déficits sociais e interpessoais, marcado por desconforto agudo e reduzida capacidade para relacionamentos íntimos, além de distorções cognitivas ou perceptivas e comportamento excêntrico, que costuma surgir no início da idade adulta e estar presente em uma variedade de contextos, associando-se a pelo menos cinco dos nove traços esquizotípicos: ideias de referência; crenças bizarras ou pensamento mágico que influenciam o comportamento e são incompatíveis com as normas da subcultura do indivíduo (por exemplo: superstições; crença em clarividência; telepatia ou "sexto sentido"; em crianças e adolescentes, fantasias e preocupações bizarras); experiências perceptivas incomuns, incluindo ilusões somáticas; pensamento e discurso bizarros (por exemplo, vago, circunstancial, metafórico, superelaborado ou estereotipado); desconfiança ou ideação paranoide; afeto inadequado ou constrito; aparência ou comportamento esquisito, peculiar ou excêntrico; ausên-cia de amigos íntimos ou confidentes, exceto parentes em primeiro grau; ansiedade social excessiva, que não diminui com a familiaridade e tende a estar associada com temores paranoides, em vez de julgamentos negativos acerca de si próprio.O diagnóstico de TEP é considerado um importante fator de vulnerabilidade para o desenvolvimento de esquizofrenia, que tem um curso crônico e marcado por recaídas e remissões, manifestando-se costumeiramente no início da idade adulta 2 . Essas são fortes razões para a preocupação com a detecção de fatores de risco para esquizofrenia na população geral. De fato, o estudo e o reconhecimento de pródromos têm sido uma importante atividade de pesquisa em esquizofrenia. Cerca de 80% a 90% dos esquizofrênicos relatam uma gama de sintomas prodrômicos, que incluem mudanças nas percepções, nas crenças, na cognição, no humor e no comportamento, antes de ficarem psicóticos 3 . É, pois, importante identificar quais são as populações que, em um dado momento, manifestam possíveis sintomas prodrômicos e estarão sujeitas à evolução para um quadro de esquizofrenia. São considerados indivíduos de alto risco aqueles que têm um parente esquizofrênico em primeiro grau, mais frequentemente um dos pais ou irmãos. Todavia, recentemente foram propostos critérios de identificação para grupos de risco ultra-alto, subgrupados da seguinte forma 4 : 1. Indivíduos com síndrome de sintomas psicóticos atenuados: no último ano, sintomas positivos subclínicos ocorreram, mas não sintomas francamente psicóticos; os sintomas devem ter ocorrido no mínimo uma vez por semana no último mês. 2. Indivíduos com síndromes psicóticas intermitentes breves: nos últimos três meses, ocorreram sintomas psicóticos francos, que foram breves e autolimitados; tais sintomas não preenchem crité-rios do DSM-IV para algum transtorno psicótico; os sintomas não são seriamente desagregadores ou perigosos. 3.
