Summary. About 30% of patients with severe aplastic anaemia (SAA) unresponsive to one course of immunosuppressive (IS) therapy with antithymocyte or antilymphocyte globulin can achieve complete or partial remission after a second IS treatment. Among various second-line treatments, rabbit ATG (r-ATG) could represent a safe and effective alternative to horse ALG (h-ALG). In a multicentre study, 30 patients with SAA (17 males and 13 females, median age 21 years, range 2±67) not responding to a ®rst course with h-ALG plus cyclosporin (CyA) and granulocyte colony stimulating factor (G-CSF), were given a second course using r-ATG (3´5 mg/kg/d for 5 d), CyA (5 mg/kg orally from day 1 to 180) and G-CSF (5 mg/kg subcutaneously from day 1 to 90). The median interval between ®rst and second treatment was 151 d (range 58±361 d). No relevant side-effects were observed, but one patient died early during treatment because of sepsis. Overall response, de®ned as transfusion independence, was achieved in 23/30 (77%) patients after a median time of 95 d (range 14±377). Nine patients (30%) achieved complete remission (neutrophils >2´0´10 9 /l, haemoglobin >11 g/dl and platelets >100´10 9 /l). The overall survival rate was 93% with a median follow-up of 914 d (range 121±2278). So far, no patient has relapsed. Female gender was signi®cantly associated with a poorer likelihood to respond (P 0´0006). These data suggest that r-ATG is a safe and effective alternative to h-ALG for SAA patients unresponsive to ®rst-line IS treatment.
Introdução: O câncer é um problema de saúde pública e a prevenção e identificação precoce são requisitos importantes para a redução das taxas de morbidade e mortalidade. Objetivo: Avaliar o conhecimento acerca da doença oncológica e práticas de cuidado com a saúde anterior ao diagnóstico de câncer em pacientes assistidos em um Centro de Alta Complexidade em Tratamento de Câncer. Método: Estudo transversal descritivo realizado com 480 pacientes em tratamento oncológico atendidos no período de abril a dezembro de 2011. Resultados: A média de idade dos entrevistados foi de 58,23 anos±13,11, adultos 51,7%, do sexo feminino 53,73%, casados 66,9%, de cor branca 88,3%, escolaridade 68,4% e renda baixa 63,1%. Em relação a doenças oncológicas, 54% dos entrevistados as desconhecem, 80% procuram pelos serviços de saúde devido à presença de sinais ou sintomas, 72% têm a preocupação de cuidar de sua saúde em geral e consideram como cuidados com a saúde realizar consulta médica ou de enfermagem. Conclusão: Faz-se necessário o desenvolvimento de ações intervencionistas voltadas para a educação em saúde, estabelecendo um vínculo entre profissionais e usuários.
RESUMOO aumento de portadores de doenças crônicas dependentes de cuidados tem sido responsável por levar algumas famílias a vivenciarem a experiência do cuidar no ambiente doméstico. Neste contexto, este estudo objetivou analisar a percepção de doentes crônicos acerca do cuidado familiar recebido no espaço domiciliar. O estudo consiste de uma pesquisa qualitativo-descritiva cujos dados foram obtidos por meio de entrevista aberta com nove pacientes. Para a análise seguiram-se os passos da análise temática. Os resultados evidenciaram uma categoria que versa acerca do cuidado familiar no domicílio na ótica de doentes crônicos dependentes de cuidado. Para estes indivíduos o cuidado tem significados de atenção, presteza, zelo, afeto, proteção e compreensão. Eles reconhecem, ainda, que a relação de cuidado pode resultar em conflitos, desentendimentos e desgastes advindos do processo de cuidar. Conclui-se que há fragilidades e dificuldades para a prestação do cuidado por familiar no ambiente domiciliar e, para tanto, os profissionais de saúde, em especial os da enfermagem, devem incluir a família no conjunto de usuários que necessitam de orientações e apoio no sentido de qualificar a assistência a este grupo populacional. INTRODUÇÃOMesmo com os avanços tecnológicos na área da saúde, diversas doenças continuam sem cura, muitas delas crônicas e de longa duração que, na maioria das vezes, levam o indivíduo à dependência parcial ou total, passando a maior parte do tempo limitado ao espaço doméstico e, frequentemente, necessitando de cuidados. Para a Organização Mundial de Saúde, as doenças crônicas caracterizam-se por serem permanentes e produzirem incapacidade/deficiências residuais; são causadas por alterações patológicas irreversíveis, exigem uma formação especial do doente para a reabilitação ou podem demandar longos períodos de supervisão, observação ou cuidados (1) . Destaca-se que os cuidados ofertados no espaço doméstico qualificam a assistência, pois há maior proximidade da família com os hábitos e costumes, permitindo maior valorização do indivíduo, e não de sua doença. Assim, deve-se considerar o ambiente familiar como um espaço que favorece a recuperação da saúde dos pacientes, em especial, porque nele comumente estão presentes os familiares, e sabe-se que os vínculos afetivos e sociais ajudam sobremaneira na situação de adoecimento de um integrante da família.Doenças de caráter crônico podem desencadear mudanças fisiológicas e psicológicas, em que a capacidade funcional pode estar ou ficar comprometida. Diante de certas condições a dependência, a perda da autonomia e da identidade, a impossibilidade de realizar o autocuidado e o comprometimento de funções, demandam cuidados constantes (2) .
