Resumo: A pandemia pela doença de coronavírus 2019 (COVID-19) representa enorme desafio para o sistema de saúde em todo o mundo. Foi nosso objetivo revisar e sintetizar os principais desafios associados ao cuidado intensivo do paciente adulto crítico com COVID-19 e apresentar evidências de recomendações. Trata-se de revisão integrativa de literatura, baseada na experiência de outros países, que tem o potencial de instrumentalizar os profissionais de saúde intensivistas para a prática assistencial ao paciente crítico com COVID-19. Dentre os principais desafios a serem enfrentados pelas equipes de unidades de terapia intensiva (UTI), evidenciados pela literatura, sintetizamos nesse trabalho: a manifestação clínica e diagnóstico para isolamentos rápidos; o manejo da insuficiência respiratória aguda; o papel das terapias medicamentosas reaproveitadas, experimentais e adjuvantes; a prevenção de infecções para pacientes e profissionais; a gestão da mortalidade de pacientes e o gerenciamento da equipe de UTI. O estado atual de conhecimento sobre a COVID-19 é considerado insuficiente e cheio de lacunas, o que configura outro desafio e enseja a necessidade de realização de mais pesquisas clínicas.

Este artigo objetivou descrever e analisar a rede de apoio social de pacientes com diagnóstico de esquizofrenia de um Centro de Atenção Psicossocial II (CAPS II) do Distrito Federal, Brasil, por meio da Análise de Redes Sociais (ARS). Observou-se que a maioria consegue retribuir o apoio recebido, especialmente para os membros da família. Estender essa reciprocidade poderia ser uma proposta a ser desenvolvida, a fim de fortalecer as relações para além do âmbito familiar e incentivar melhorias na rede de apoio social. Uma cultura participativa em todos os contextos, nos quais se encontram inseridos os portadores de esquizofrenia, pode contribuir, sobremaneira, para o aumento da autonomia e da autoestima dos mesmos. Verifica-se que é viável, por meio deste estudo, reconhecer as limitações dos pacientes em questão, assim como identificar suas potencialidades.
Objetivo: Descrever percepções e preferências de familiares de pacientes adultos de Unidade de Terapia Intensiva (UTI) sobre a comunicação da suspeita e abertura do protocolo de morte encefálica (ME). Métodos: Estudo de campo, exploratório, descritivo, observacional, transversal, quanti-qualitativo, usando os instrumentos de diário de bordo, checklist e questionário específico. Participaram cinco familiares de pacientes com suspeita de ME em UTI de hospital público terciário do Distrito Federal. Realizada análise descritiva da rotina de comunicação sobre ME e das respostas objetivas e de conteúdo para as discursivas. Resultados: Preferências e percepções da má notícia sobre suspeita e abertura de protocolo de ME foram compatíveis. Preferências preponderantes das famílias: receber a notícia pessoalmente, com atenção, contato visual, honestidade e respeito ao nível de compreensão, com espaço para expressar a compreensão, com pausas e disponibilidade para esclarecer dúvidas. Conclusão: A compatibilidade entre percepções e preferências indica que a rotina de comunicação no cenário de estudo tem contemplado as preferências das famílias. A comunicação da suspeita e abertura do protocolo de ME é complexa, seriada e de difícil compreensão, com reação emocional de “choque”. Recomenda-se abordagem interdisciplinar às famílias, uso de protocolos de comunicação de más notícias e conhecimento quanto a preferências de familiares.
A temática de finitude é notadamente desafiadora para o ser humano, e especialmente, para as equipes de saúde. Assim, o Planejamento Antecipado de Cuidado (ACP) e Diretivas Antecipadas (DA) se apresentam como recursos para otimizar o cuidado humanizado centrado no paciente. Este estudo objetiva identificar os desfechos de ACP e DA no cuidado do paciente crítico em UTI.
