By the time a clinical diagnosis of Parkinson's disease (PD) is made, a significant loss of dopaminergic neurons has already occurred. Identifying patients in the period between the presumed onset of dopaminergic cell loss and the appearance of clinical parkinsonism may be of major importance in the development of effective neuroprotective treatment strategies. In an effort to develop a feasible strategy to detect preclinical PD, a combination of olfactory processing tasks, including odor detection, odor identification, and odor discrimination was used to select groups of hyposmic and normosmic individuals from a total of 250 relatives (parents, siblings, or children) of subjects with PD. Single photon emission computed tomography (SPECT) with [123I]beta-CIT as a dopamine transporter ligand was used to assess nigrostriatal dopaminergic function in 25 hyposmic and 23 normosmic relatives of PD patients. An abnormal reduction in striatal dopamine transporter binding was found in 4 out of 25 hyposmic relatives of PD patients, two of whom subsequently developed clinical parkinsonism, and in none of the 23 normosmic relatives. These observations demonstrate that subclinical reductions in dopamine transporter binding can be detected in asymptomatic relatives of sporadic PD patients by means of [123I]beta-CIT and SPECT. The results further indicate that olfactory deficits may precede clinical motor signs in PD.

(uit: Het Parfum, Patrick Su¨skind) InleidingHet hersengebied, verantwoordelijk voor de waarneming en verwerking van geuren, is bij mensen relatief bescheiden van omvang. Toch zijn we in staat om zo'n 10.000 verschillende geuren te onderscheiden. Van deze gevoeligheid zijn we ons meestal pas bewust wanneer er sprake is van een reukstoornis. Voor de persoonlijke beleving betekent een verminderde reuk in veel opzichten een verarming of zelfs een gevaar omdat geuren niet alleen een sterke emotionele waarde hebben maar ook een signaal kunnen zijn voor dreigend gevaar. Vaak zijn reukstoornissen te herleiden tot bekende oorzaken. Die kunnen van tijdelijke aard zijn (verkoudheid), seizoensgebonden (hooikoorts) maar ook irreversibel (na schedelbasisfractuur met beschadiging van de reukzenuw). Soms kunnen reukstoornissen een eerste symptoom zijn van een verder nog asymptomatische aandoening zoals mogelijk het geval is bij de ziekte van Parkinson.Na een algemene bespreking van het functioneren van het reukorgaan wordt in dit artikel ingegaan op de ziekte van Parkinson en haar symptomen, de relatie tussen reukstoornissen en de premotorische fase van deze aandoening en de betekenis van reuktests ten behoeve van de preklinische diagnostiek. Anatomie van de reukHet perifere gedeelte van het reukorgaan is het olfactorisch epitheel, dorsaal in de neusholte. Overdekt door een slijmlaag liggen omringd door steuncellen de receptorneuronen die specifiek zijn voor verschillende geurmoleculen. De axonen van deze neuronen vormen de reukzenuw die door de lamina cribrosa van het zeefbeen heen projecteert naar de glomerulaire laag van de bulbus olfactorius. Deze bestaat feitelijk uit twee kleine ovale structuren die met elkaar in verbinding staan via de commissura anterior (figuur 1). De bulbus olfactorius bestaat uit verschillende lagen: (1) laag van reukzenuwen, (2) glomerulaire laag, (3) externe plexiforme laag, (4) mitraalcellaag, (5) interne plexiforme laag, (6) granulaire cellaag (figuur 2). Bij de mens ligt in het centrum van de bulbus nog een duidelijk te onderscheiden laag: (7) stratum album waar zich dopaminerge cellen bevinden die bij dieren op deze plaats nooit zijn waargenomen.Glomeruli zijn gespecialiseerde gebieden waar de olfactorische zenuwen contact maken met dendrieten van mitraal-en pluimcellen (Engels: tufted cells).
