Resumo Objetivo: compreender a assistência à saúde, equidade e acessibilidade no cotidiano de usuários da Atenção Primária à Saúde (APS). Metodologia: trata-se de um estudo qualitativo e descritivo, delineado pelo método de Estudo de Casos Múltiplos Holístico e pelo referencial teórico da Sociologia Compreensiva do Cotidiano. Participaram 77 usuários do SUS (Sistema Único de Saúde) de dois municípios de Minas Gerais, Brasil. Resultados: foram identificadas a noção do usuário sobre o SUS, sua idealização, a realidade vivida considerando a acessibilidade e a assistência na APS, relacionando-as com o direito à saúde. Conclusões: a APS ainda enfrenta dificuldades para efetivamente ser a porta de entrada preferencial nas realidades estudadas. Observam-se limites de estrutura na APS para alcançar a assistência integral em rede. A superação de dificuldades e limites é central para o fortalecimento do SUS.
Objetivo: Compreender o acesso e a acessibilidade à saúde na perspectiva de usuários, em atendimento à demanda espontânea em unidades de Atenção Primária à Saúde (APS). Métodos: Estudo de Casos Múltiplos Holístico-qualitativo, fundamentado na Sociologia Compreensiva do Quotidiano com 60 participantes, em município de grande porte de Minas Gerais. Resultados: Revelam a Saúde como uma questão de luta no quotidiano da APS, destacando as dificuldades enfrentadas pelos usuários do Sistema Único de Saúde (SUS), no que concerne o acesso aos serviços, ações, exames e medicamentos. Denotam a desorganização administrativa e morosidade do Sistema, a questão política, a judicialização. A situação contemporânea do SUS foi questionada: o que precisa no SUS? Conclusão: A realidade vivenciada pelos usuários indica que ainda não foi alcançada a idealização de um Sistema universal, equânime e integral. Ademais, a pesquisa possibilitou reflexões de todos os atores envolvidos, usuários, profissionais de saúde e gestores, sobre o exercício da participação social no SUS e a formação de sujeitos proativos na tomada de decisões políticas para o cumprimento e vigor das legislações que implementam o SUS.
Objectives: to understand access in a day in the life Primary Health Care as a fundamental right to health, from users’ perspective. Methods: a holistic-qualitative multiple case study based on Comprehensive Sociology of Everyday Life. 77 Unified Health System (SUS) users participated in the study. Results: according to users’ voice, the right to health is still a matter of struggle, in more than 30 years of SUS. SUS is considered as the place where they have to appeal. The experiences are diverse, from the ease to the difficulty of access, neglecting users’ needs, but despite the inequality in relation to the offer of services, the solution always arrives. Final Considerations: to overcome this historical obstacle of comprehensive access to health, as a fundamental human right, it is necessary to overcome political and administrative decisions that hinder the construction of legitimate SUS, in a truly democratic participation of all social actors.
O objetivo deste estudo é compreender os problemas éticos e bioéticos nas vivências de enfermeiros no cotidiano da atenção primária à saúde (APS) e seu impacto sobre a segurança do profissional. Trata-se de um estudo de casos múltiplos holístico-qualitativo, fundamentado na sociologia compreensiva do cotidiano, com 54 enfermeiros atuantes na aps. Apresenta dois casos definidos pelos cenários de duas capitais do sul do Brasil. Os dados foram submetidos à análise de conteúdo temática e à técnica analítica da síntese cruzada dos casos. Os resultados evidenciam que os problemas éticos e bioéticos se relacionam às precariedades do sistema e do atendimento, às condições de infraestrutura, ambiente e ambiência, e às fragilidades em torno das relações usuário-família- equipe, equipe-equipe e equipe-gestão, impactando negativamente na segurança do profissional. A segurança foi favorecida mediante a experiência e as habilidades adquiridas pelo enfermeiro em seu exercício profissional, a sistematização da assistência, os protocolos assistenciais, o uso de redes e mídias sociais, e a sensibilização das equipes por meio de espaços de discussão e capacitação referentes à ética e à bioética, e da atuação profissional segura. A compreensão da segurança do profissional enfermeiro, neste estudo, integra a objetividade que normatiza as condutas éticas e bioéticas, e a subjetividade de enfermeiros(as) ao apontar questtionamentos sobre sua segurança profissional perante sentimentos, angústias e anseios em sua prática cotidiana.
Objetivo: compreender as vivências cotidianas de usuários, em demanda espontânea, para o acesso e a acessibilidade no Sistema Único de Saúde na porta de entrada da Atenção Primária à Saúde. Materiais e métodos: estudo de casos múltiplos holístico-qualitativo, fundamentado na sociologia compreensiva do cotidiano, com 60 participantes de município de grande porte do estado de Minas Gerais,Brasil. Resultados: a demanda espontânea, no cotidiano, está voltada para o agendamento de consultas, para o atendimento médico e para o acesso aos exames, em uma assistência ao adoecimento. Os usuários apresentam dificuldades para o acesso e a acessibilidade às ações e aos serviços. Conclusões: o acesso aos serviços de saúde da Atenção Primária/Estratégia Saúde da Família continua desafiador, restritivo e burocrático, regulado pela demanda espontânea do usuário e pela agenda médica.
