Introdução: Os idosos com 80 anos ou mais, representam uma população relativamente nova e em expansão no Brasil e, portanto, há poucos estudos que analisem o comportamento dessa população em relação à adesão medicamentosa. Objetivo: investigar o perfil de adesão medicamentosa de idosos longevos e os fatores relacionados. Métodos: estudo transversal de abordagem quantitativa com 50 idosos longevos atendidos em um ambulatório especializado, localizado no município de São Paulo- SP. Os idosos foram entrevistados por meio de questionários com sobre questões sociodemográficas, teste de Morisky e Green modificado, Índice de Katz, e escala de Lawton e Brody. Os dados foram analisados de forma descritiva e inferencial, adotando o intervalo de confiança de 95%, considerando-se um nível de significância de 5%. O projeto foi aprovado pelo Comitê de Ética em Pesquisa da UNIFESP/ Hospital São Paulo, protocolo nº 00492918.4.0000.5505. Resultados: A maioria (68,0%) dos idosos eram homens, média etária de 87,8 anos e contavam (84,0%) com auxílio de cuidador informal em seus lares. A maioria (54,0%) apresentou média adesão às medicações, sendo os motivos mais prevalentes de não adesão: esquecer o horário das medicações (38,0%) e esquecer de tomar os medicamentos (24,0%). Observou-se correlação (p= 0,049) entre possuir auxílio de cuidador informal em seus lares e adesão medicamentosa. Conclusão: os resultados encontrados mostram a necessidade de investimentos na identificação do perfil de adesão medicamentosa dos idosos longevos com vistas ao planejamento multidisciplinar do cuidado, fornecendo suporte adequado às necessidades dos idosos e suas famílias.
