ABSTRACT. Epilepsy, a chronic neurological condition which is associated with neurobiological and psychosocial changes, affects 0.5 to 1% of the world's population, presenting in most cases a deficit in reasoning, memory and attention. Objective: To contribute to the implementation of screening strategies for cognitive decline and memory deficits in patients with epilepsy. Methods: Two questionnaires, MMSE and MoCA, were used in this cross-sectional and observational study. Fifty-four patients diagnosed with different types of epilepsy (55% refractory) were assessed; they were all over 18 years old, of both genders, with autonomy to answer the questionnaire. They were followed exclusively at an outpatient clinic of the Neurology Service Department, specialized in epilepsy, which is part of the tertiary healthcare level of the Brazilian Unified Health System (SUS). Results: The final sample consisted of 54 patients. There was a significant correlation (p<0.001) between the scores of both tests, indicating that low values in the MMSE score also corresponded to low values in the MoCA score. Sensitivity was 90% (ROC curve adjusted) and 87.5% of the patients with a normal score in the MMSE test obtained alterations in the MoCA scores. None of them showed a low MMSE score with a normal MOCA score. The Spearman correlation coefficient was 0.80. Also, there was a significant relationship between both immediate memory and delayed recall memory and the type of seizure (p<0.03) and level of schooling (p<0.001), respectively. Conclusion: The MoCA is a well-suited test to be performed in epilepsy patients to evaluate their cognition as it seems more extensive and complete compared to MMSE.
Cet article s’ intéresse à la manière dont les candidats recherchent et interprètent les informations à propos d’un potentiel employeur dans un contexte où les organisations, en particulier les grandes, utilisent de multiples sources de diffusion de leur marque employeur et dans lequel les candidats peuvent facilement trouver des informations plus informelles à leur sujet, notamment via leur réseau personnel et professionnel, réel ou virtuel. Une étude qualitative menée auprès de 20 personnes à la recherche d’un nouvel emploi propose d’analyser le processus par lequel les candidats se renseignent sur un potentiel employeur, les sources d’informations utilisées, les informations recherchées ainsi que les contenus plébiscités. Cette étude cherche à mieux comprendre comment les candidats perçoivent la teneur et la crédibilité des informations recueillies à propos d’un employeur potentiel et comment cela influence leurs attitudes à l’égard de l’employeur et leur décision de poursuivre, ou non, le processus de candidature.
Introdução: O acidente vascular cerebral constitui uma das principais causas de incapacidade, provocando alterações que se manifestam numa restrição da funcionalidade da pessoa, contribuindo para um crescimento de quedas acidentais. A identificação de fatores de risco de queda por parte do Enfermeiro Especialista em Enfermagem de reabilitação é imperativa para evitar estes eventos. Objetivo: Identificar fatores de risco de queda em pessoas com Acidente Vascular Cerebral. Método: Revisão sistemática da literatura, com pesquisa na plataforma EBSCO Host®, na CINAHL e MEDLINE, no período de 1 a 31 de outubro de 2017. Utilizou-se a seguinte equação de pesquisa, (Accidental Falls) AND (Stroke) AND (Risk Factors). Resultados: Obteve-se um total de 19 estudos e um total de 22 fatores de risco de queda, 12 dos quais presentes na Taxonomia NANDA-I. Conclusões: Esta pesquisa permitiu identificar fatores de risco de queda na pessoa com AVC. Este estudo constitui um contributo ao desenvolvimento do conhecimento da disciplina de Enfermagem, e que tem implicações na prática clínica, na educação e em novas pesquisas numa área sensível aos cuidados de enfermagem. Introdução: O Brasil é o maior sistema público de transplantes do mundo. Recentemente, o escore MELD-S médio foi incorporado no Brasil como modelo de preditor de gravidade de doença hepática. Em estudos internacionais, foi observado que o sódio sérico abaixo dos valores normais está associado à perda do enxerto; no entanto, sua relação com fatores prognósticos na sobrevida pós-operatória ainda incerta. Objetivos: Contribuir com a implementação de estratégias de rastreio de fatores prognósticos no pós-transplante hepático. Métodos: Estudo longitudinal retrospectivo, incluindo pacientes que realizaram transplante hepático no Hospital Angelina Caron, entre as datas de 01/01/2016 e 31/12/17 e que realizaram acompanhamento médico durante um ano após a data do transplante. Foram analisadas diferentes variáveis para estimar a taxa de sobrevida e fatores prognósticos pós-transplante. Resultados: Amostra final de 148 pacientes, dos quais n=105 pertenciam ao sexo masculino, com média de idade de 52 anos +11,7, e n=139 autodeclararam-se de cor branca. Encontraram-se n=50 indivíduos com cirrose alcoólica e n=32 com hepatite viral. O tempo na lista de espera para o transplante hepático foi de 68 dias+113. A pontuação média do MELD foi de 20,7+5,56 e MELD-Na de 13,8+5,31. Encontrados valores MELD-Na <15 (n=105), MELD-Na >15 (n=43). A taxa de sobrevida geral pós-seis meses foi de 68,9%, de 61,4%, em 12 meses, e de 56,7%, em 24 meses. Hiponatremia pré-operatória (<135 mEq/L) presente em 33 pacientes, dos quais n=14 vieram a óbito seis meses pós-transplante, n=4, em 12 meses, e n=1, em 24 meses. Hiponatremia e óbito obtiveram p<0,05. MELD-Na >15 (n=43), dos quais, 17 vieram a óbito seis meses pós-transplante, três, em 12 meses e um, em 24 meses. Avaliando o escore MELD, n=89 foram preditores significativos de óbito (p<0,05), ajustando-o para o escore MELD-Na, n=43 foram preditores significativos de óbito (p<0,05). Conclusão: A vigilância da hiponatremia pré-operatória e o uso do MELD-Na para definição da prioridade na lista de espera do procedimento devem ser fortemente incentivados como forma de redução da taxa de mortalidade pós-transplante hepático.
