Despite of improved survival of premature infants, the incidence of long term pulmonary complications, mostly associated with ventilation-induced lung injury, remains high. Non invasive ventilation (NIV) is able to reduce the adverse effects of mechanical ventilation. Although nasal continuous positive airway pressure (NCPAP) is an effective mode of NIV, traumatic nasal complications and intolerance of the nasal interface are common. Recently high flow nasal cannula (HFNC) is emerging as a better tolerated form of NIV, allowing better access to the baby's face, which may improve nursing, feeding and bonding. HFNC may be effective in the treatment of some neonatal respiratory conditions while being more user-friendly for care-givers than conventional NCPAP. Limited evidence is available to support the specific role, efficacy and safety of HFNC in newborns and to demonstrate efficacy compared with NCPAP; some studies suggest a potential role for HFNC in respiratory care of the neonate as a distinct non invasive ventilatory support. We present the preliminary data of a randomized clinical trial; the aim of this study was to assess efficacy and safety of HFNC compared to NCPAP in preterm newborns with mild to moderate respiratory distress syndrome (RDS). Recentemente, la terapia con nasocannule ad alto flusso (HFNC) si sta diffondendo come modalità di assistenza ventilatoria non invasiva efficace e meglio tollerata, in quanto permette un migliore accesso al neonato facilitando così l'assistenza infermieristica, la nutrizione e il contatto con la famiglia e l'ambiente esterno. La terapia con HFNC può essere efficace nel trattamento di alcune patologie respiratorie del neonato essendo nello stesso tempo di più facile utilizzo da parte di chi si prende cura dei neonati, rispetto alla NCPAP. Una limitata evidenza è disponibile in relazione allo specifico ruolo, all'efficacia e alla sicurezza della HFNC nel neonato e per dimostrarne l'efficacia rispetto alla NCPAP; alcuni studi suggeriscono un possibile ruolo della HFNC nell'assistenza respiratoria del neonato come una forma distinta di supporto non invasivo. In the last few years we have observed an upward trend in the employment of ibuprofen as anti-inflammatory and antipyretic therapy. Therefore the pediatrician has often a precious option in the anti-inflammatory and antipyretic treatment in children instead of using steroids and paracetamol. In clinical practice ibuprofen can be used in the treatment of headache, toothache, otalgy, dysmenorrhea, neuralgia, arthralgia, myalgia, abdominal pain and fever: it is the first choice for these common diseases. However, the use of steroids is a routine, even if non-corticosteroid anti-inflammatory molecules could be useful. Certainly steroids are powerful anti-inflammatory, indicated for the treatment of chronic inflammatory disorders and in acute respiratory and allergic diseases. Beside, thanks to their chemical and pharmacological profile, they also provide patients with an antipyretic effect. However, the use of steroids must be reserved to cases in which other classical antipyretics such as non-steroidal anti-inflammatory drugs are not effective. The possible side effects and risks associated with stepping down steroids must be considered. Although "steroids-phobia" should be discouraged, steroids are to be reserved only as the first indication. In all other cases the pediatrician can use ibuprofen, whose efficacy and safety are widely demonstrated by now.
Breast milk has always been the best source of nourishment for newborns. However, breast milk can carry a risk of infection, as it can be contaminated with bacterial or viral pathogens. This paper reviews the risk of acquisition of varicella-zoster virus (VZV) and cytomegalovirus (CMV), herpesviruses frequently detected in breastfeeding mothers, via breast milk, focusing on the clinical consequences of this transmission and the possible strategies for preventing it. Maternal VZV infections are conditions during which breastfeeding may be temporarily contraindicated, but expressed breast milk should always be given to the infant. CMV infection acquired through breast milk rarely causes disease in healthy term newborns; an increased risk of CMV disease has been documented in preterm infants. However, the American Academy of Pediatrics (AAP) does not regard maternal CMV seropositivity as a contraindication to breastfeeding; according to the AAP, in newborns weighing less than 1500 g, the decision should be taken after weighing the benefits of breast milk against the risk of transmission of infection. The real efficacy of the different methods of inactivating CMV in breast milk should be compared in controlled clinical trials, rigorously examining the negative consequences that each of these methods can have on the immunological and nutritional properties of the milk itself, with a view to establish the best risk-benefit ratio of these strategies before they are recommended for use in clinical practice. Per quanto riguarda l'infezione da CMV, allattare o no con latte materno i neonati prematuri nati da madre CMV-positiva rimane ad oggi una questione controversa. Resumo Objetivo Comparar o mapeamento oficial com um mapeamento probabilístico da infestação por Aedes spp. nos municípios do Rio Grande do Sul, Brasil. Métodos Estudo ecológico com dados de amostras de criadouros em 2016-2017; obteve-se a classificação oficial em boletins epidemiológicos e estimou-se a probabilidade de infestação por município e semana, ajustando-se um modelo dinâmico de ocupação de sítios aos dados da vigilância epidemiológica municipal. Resultados 187.245 amostras coletadas em 473 municípios originaram 10.648 detecções de Aedes aegypti e 8.414 de Aedes albopictus; o mapeamento oficial concorda com o probabilístico em municípios da região noroeste e oeste do RS; os mapeamentos discordam nas regiões leste, centro, nordeste e sul, revelando municípios oficialmente não infestados com alta probabilidade de infestação e notificação de arboviroses. Conclusão A classificação oficial identificou infestação nos municípios infestados do noroeste e oeste, e não identificou infestação em municípios com possíveis falsos zeros e onde ela varia temporalmente.
O Programa de Gestão em Saúde da Residência Integrada Multiprofissional da Escola de Saúde Pública do Rio Grande do Sul foi criado em 2020. Em virtude da necessidade de concretizar o projeto transversal previsto no Projeto Pedagógico e de promover a integração entre os residentes, surge o Coletivo de Residentes do Programa de Gestão em Saúde. O presente trabalho objetiva relatar o processo de criação do Coletivo, enquanto componente curricular proposto pelos residentes, bem como a sua atuação e importância. Para tanto, foi realizado um relato de experiência de residentes do primeiro e do último ano (que ingressaram na primeira turma), aliado a uma pesquisa documental dos materiais produzidos pelo Coletivo. Os resultados indicam que o Coletivo proporcionou o desenvolvimento de atividades que qualificam a formação dos futuros gestores e o próprio Programa, em consonância com os princípios do Sistema Único de Saúde.
