Epidemiological and clinical aspects of 50 consecutive patients, 47 adults and three children, hospitalized between 1977 and 1987 for human leptospirosis, were reviewed. 45 (90%) of the patients were from rural regions. 32 (64%) cases occurred in individuals at occupational risk for the infection. 35 (70%) cases were registered in the warm season. The source of infection was known in 34 (68%) cases. Weil's disease was diagnosed in 31 (62%) patients, aseptic meningitis in 12 (24%) and acute unexplained fever in seven (14%). Haemodialysis was required for 11 (35%) patients with Weil's disease. Three (6%) patients died. Cause of death was massive gastrointestinal haemorrhage in two and renal failure in one. Leptospira icterohaemorrhagiae was responsible for 39 (78%) cases, Leptospira canicola for six (12%), Leptospira grippotyphosa for two (4%), and Leptospira australis, Leptospira ballum and Leptospira sejroe, for one case each. A muscle biopsy was performed in six patients and a renal biopsy in three. Focal necrotic muscular changes, with mild mononuclear infiltrate, were found. Pigmented casts in distal convoluted tubules, mild interstitial inflammatory infiltrate and mesangial enlargement of some glomeruli were observed in kidney biopsies. A good knowledge of the protean clinical manifestations of leptospirosis and an accurate laboratory study are required for a correct diagnosis.
A nodulose por metotrexato (MTX) é um dos efeitos colaterais pouco conhecidos do uso desse medicamento em doses baixas. Embora classicamente descrita em casos de artrite reumatóide, tem aparecido, também, em outras doenças reumáticas. Descreve-se aqui um caso de nodulose por MTX em uma paciente com artrite reumatóide soropositiva, que utilizava esse medicamento há um ano, com bom controle do processo articular. Segue-se uma breve revisão sobre o assunto. INTRODUÇÃONos últimos anos, o metotrexato (MTX) em doses baixas tem sido utilizado amplamente no tratamento de várias enfermidades não oncológicas, tais como artrite reumatóide do adulto, artrite idiopática juvenil, polimiosite, dermatomiosite, artrite psoriática e psoríase cutânea, pelas suas propriedades imunomoduladoras e poupadoras de corticóide (1) . Entretanto, vários efeitos colaterais são possíveis com o seu uso, requerendo atenção especial do médico que o prescreve. Hepatoxicidade, disfunção de medula óssea e pneumonite estão entre os mais conhecidos (2) . Embora menos valorizadas, as alterações mucocutâneas fazem parte desse elenco, já tendo sido descritas: acne, alopecia, eritema multiforme, estomatite, úlceras da boca, erupções bolhosas, urticária, paniculite e nodulose, entre outras (3,4) . A nodulose é um dos efeitos secundários ao uso do MTX menos conhecido e que cursa com a formação de pequenos nódulos nos dedos, cotovelos, pés e orelhas (2) , podendo atingir vísceras como coração (5) , laringe (6) , pleura e pulmões (7,8) . No presente artigo é relatado o caso de uma paciente com artrite reumatóide que desenvolveu nodulose pelo uso de MTX, acompanhado de uma pequena revisão sobre o assunto. ABSTRACT RELATO DE CASOEAM, sexo feminino, 47 anos de idade, tem artrite reumatóide soropositiva há sete anos, estando em uso de difosfato de cloroquina, prednisona (entre 5-10mg/d) e tendo feito uso de diversos antiinflamatórios não hormonais desde a época do diagnóstico. Tentativa de tratamento com leflunomide, feita há dois anos, durante oito meses, foi considerada ineficaz, tendo sido interrompida.Comparece à consulta, encaminhada pelo Serviço de Ortopedia, após ter sido submetida à cirurgia reparadora de tendões extensores do 2 o e 3 o dígitos e sinovectomia de 2 a e 3 a metacarpofalangianas da mão direita. Research Highlights: Tree decline can alter soil carbon cycling, given the close relationship between primary production and the activity of roots and soil microbes. Background and Objectives: We studied how tree decline associated to old age and accelerated by land-use change and increased drought in the last decades, affects soil properties and soil respiration (Rs). Materials and Methods: We measured Rs over two years around centennial European beech (Fagus sylvatica L.) trees representing a gradient of decline in a sub-Mediterranean forest stand, where the number of centennial beech trees has decreased by 54% in the last century. Four replicate plots were established around trees (i) with no apparent crown dieback, (ii) less than 40% crown dieback, (iii) more than 50% crown dieback, and (iv) dead. Results: Temporal variations in Rs were controlled by soil temperature (Ts) and soil water content (SWC). The increase in Rs with Ts depended on SWC. The temperature-normalized Rs exhibited a parabolic relationship with SWC, suggesting a reduced root and microbial respiration associated to drought and waterlogging. The response of Rs to SWC did not vary among tree-decline classes. However, the sensitivity of Rs to Ts was higher around vigorous trees than around those with early symptoms of decline. Spatial variations in Rs were governed by soil carbon to nitrogen ratio, which had a negative effect on Rs, and SWC during summer, when drier plots had lower Rs than wetter plots. These variations were independent of the tree vigor. The basal area of recruits, which was three times (although non-significantly) higher under declining and dead trees than under vigorous trees, had a positive effect on Rs. However, the mean Rs did not change among tree-decline classes. These results indicate that Rs and related soil physico-chemical variables are resilient to the decline and death of dominant centennial trees. Conclusions: The development of advanced regeneration as overstory beech trees decline and die contribute to the Rs homeostasis along forest succession.
Descreve-se o caso de um paciente com Granulomatose de Wegener, em tratamento com esteróides e ciclofosfamida, que, apesar do tratamento, evoluiu com dispnéia progressiva e rouquidão. A investigação mostrou tratar-se de uma estenose subglótica, sendo a dispnéia prontamente aliviada com a traqueostomia. São discutidos diagnósticos diferenciais e modalidades de tratamentos de estenose subglótica em portadores de Wegener.
Resumo: FUNDAMENTOS -A esclerose sistêmica é colagenose pouco comum e muito rica em manifestações cutâneas. OBJETIVO -Estudar a prevalência das manifestações cutâneas na esclerose sistêmica em geral e nos seus diferentes subtipos (formas limitada, generalizada e mista). MÉTODOS -Analisaram-se 32 pacientes de esclerose sistêmica (20 com forma limitada, oito com generalizada e quatro com forma mista) quanto à esclerose de pele, fenômeno de Raynaud, cicatrizes estelares, telangiectasias, leucomelanodermia, microstomia, calcinose e prurido. RESULTADOS -Encontraram-se esclerose de pele e fenômeno de Raynaud em 100% dos pacientes; cicatrizes estelares em 65,6%; telangiectasias em 43,7%; leucomelanodermia em 43,7%; microstomia em 31,25%; prurido em 28,1% e calcinose em 12,5%. Não se observaram diferenças entre as formas localizada e difusa da doença, sendo p=1 para cicatrizes estelares; p=0,69 para telangiectasias; p= 0,22 para microstomia, p=1 para calcinose e prurido. A forma mista de doença não diferiu das formas isoladas (limitada e difusa) quanto aos mesmos achados. CONCLUSÕES -As manifestações mais comuns na esclerose sistêmica são a esclerose de pele e o fenôme-no de Raynaud, e a mais rara é a calcinose. As três formas apresentam freqüências semelhantes de Raynaud, cicatrizes estelares, microstomia, telangiectasia, calcinose e prurido.
