Introducción: Las alteraciones hematológicas son muy frecuentes y de origen multifactorial, se han relacionado con un aumento en el riesgo de progresión de la patología y morbimortalidad en los pacientes infectados por el virus de inmunodeficiencia humana. A pesar de que existe una amplia variedad de estudios científicos sobre la prevalencia y patogénesis de estas alteraciones en pacientes HIV positivos, son escasos las investigaciones en nuestro medio que informen sobre el efecto positivo o negativo en la administración de medicamentos antirretrovirales.Objetivo: Determinar las principales alteraciones hematológicas asociadas a terapia antirretroviral en pacientes infectados por el virus de inmunodeficiencia humana.Metodología: Revisión bibliográfica de tipo narrativa. Para la investigación se utilizaron las siguientes bases de datos, como: PubMed, Cochrane y Scopus. Resultados: La prevalencia de al menos una alteración hematológica fue del 1,1-58,6% entre los pacientes infectados por HIV que reciben TAR. La citopenia más frecuente fue la anemia seguida de leucopenia y trombocitopenia. Entre los factores de riesgo más significativos, se identificaron pacientes mayores a 50 años, sexo femenino, esquema antirretroviral basado en zidovudina, IMC y recuento de células T CD4 bajo, carga viral elevada, estadio avanzado e infecciones oportunistas.Conclusiones: La toxicidad de los medicamentos antirretrovirales y las condiciones clínicas del paciente HIV positivo, dan como resultado una hematopoyesis alterada afectando a las tres líneas celulares, los pacientes pueden presentar anemia, leucopenia, trombocitopenia y/o pancitopenia. La falta de vigilancia e identificación de estos trastornos repercute gravemente en la historia natural de la enfermedad en el paciente.
Introducción: La diabetes mellitus es una enfermedad metabólica en constante aumento y con proyecciones alarmantes en cuanto a su incidencia; asimismo, las complicaciones que conlleva esta patología son muy frecuentes, como el desarrollo de úlceras a nivel del pie y su infección, que representan los principales motivos de hospitalización y amputaciones de miembros inferiores.Objetivo: Describir el perfil bacteriológico de las infecciones del pie diabético y su tratamiento antibiótico de elección.Métodos: El estudio fue no experimental de tipo revisión sistemática siguiendo la metodología PRISMA, en bases de datos como Pubmed, ScienceDirect, Scopus, Lilacs y Dialnet, considerando determinados criterios de inclusión como artículos publicados entre enero 2017 – diciembre 2022. Resultados: Se identificaron un total de 9 artículos que cumplían con los criterios de elegibilidad, identificándose cinco bacterias más frecuentes como Staphylococcus spp, Pseudomonas aeruginosa, entre otras. Por otra parte, los grupos antibióticos como penicilinas asociadas a los inhibidores de las betalactamasas, cefalosporinas de tercera generación, entre otros, son fármacos con alta sensibilidad antibiótica para los agentes más frecuentes de las infecciones del pie diabético. Conclusiones: En base a la revisión realizada es posible concluir que las infecciones del pie diabético generalmente son polimicrobianas, siendo las bacterias del grupo Gram negativo las más predominantes en comparación a las del grupo Gram positivo; adicionalmente, para el tratamiento antibiótico de las infecciones del pie diabético, se debe considerar cubrir los agentes infecciosos más frecuentes.
Introducción: La vitamina D se ha considerado como un componente protector para la diabetes mellitus tipo II, esto se debe a que la sensibilidad a la insulina se estimula por la expresión de los receptores insulínicos y la activación de los receptores de la proliferación de peroxisomas, la cual se estimula de una manera indirecta sobre la secreción insulínica y va a estar regulada por la calbindina. La deficiencia de la vitamina D, de igual forma es un problema de salud a nivel mundial, donde en nuestro país, Ecuador, tras ubicarse en la zona ecuatorial, existe pocos estudios que comprueben que al estar en una zona de alta radiación solar influye o no en los valores de la vitamina D, ya que únicamente se comprobó la deficiencia de la vitamina D en enfermedades como la Esclerosis múltiple, con una prevalencia del 42 %.Objetivo: describir el rol de la vitamina D en pacientes con diabetes mellitus tipo 2.Metodología: es una revisión bibliográfica de tipo narrativa. Se recopilo la información por medio de base de datos como: Pubmed y Scopus. Límite de tiempo 5 años, idioma inglésResultados: La suplementación de vitamina D en los pacientes diabéticos, contribuye a disminuir la resistencia a la insulina y durante los análisis de los controles glicémicos, se observó una disminución de la glucosa como también de la hemoglobina glicosilada.Conclusiones: La vitamina D tiene cierta aceptación como un tratamiento en si para la diabetes mellitus tipo 2.
