Introdução: O carcinoma primário da glândula de Bartholin (GB) representa menos de 1% das neoplasias ginecológicas e é classificado como um subtipo do câncer de vulva, sendo estadiado e manejado como tal. Mais frequente na sétima década de vida, apresenta-se histologicamente em 40% das vezes como adenocarcinoma ou carcinoma de células escamosas e em 15% como carcinoma adenoide cístico, variante rara e foco do trabalho. Com menos de cem casos relatados na literatura, a fisiopatologia e resposta terapêutica não são bem definidas, dificultando seu manejo. Relato de caso: Paciente de 45 anos de idade, portadora de hipertensão arterial sistêmica e diabetes mellitus tipo 2, chegou ao serviço em 2019 para consulta hospitalar em ginecologia, queixando-se de nódulo doloroso em vulva havia um ano. Ao exame, constatou-se nódulo às 3h de introito vaginal, em topografia de GB, medindo cerca de 2 cm, fibroelástico, doloroso ao toque e à manobra de Valsalva, sem sinais flogísticos. A paciente foi submetida a exérese do nódulo em novembro de 2019, com laudo histopatológico de carcinoma adenoide cístico de GB, sem margem de segurança. Não aderiu a seguimento pós-cirúrgico e, em novembro de 2020, retornou com queixa de lesão dolorosa no local previamente ressecado. Ao exame, constatou-se a presença de nódulo endurecido no local, doloroso, não associado a linfonodomegalia regional. Foram solicitadas tomografia computadorizada de pelve e radiografia de tórax para avaliar a invasão de estruturas adjacentes ou a distância, ambas negativas. A paciente foi submetida a vulvectomia parcial em maio de 2021, com ampliação de margem de ressecção e laudo histopatológico confirmando carcinoma adenoide cístico na peça, com margens lateral e inferior comprometidas. Realizou acompanhamento ambulatorial, sem achados clínicos de doença residual nas consultas iniciais. Em outubro do mesmo ano, referiu retorno da dor na mesma topografia antes referida, com lesão endurecida e dolorosa ao exame físico, com projeção à vagina. Foi solicitada ressonância magnética de pelve, sem achados maiores em laudo inicial, sendo solicitada a revisão de cortes inferiores para estadiamento e conduta subsequente. Conclusão: O carcinoma adenoide cístico da GB é um diagnóstico raro, com poucos relatos na literatura, tornando-se uma condição que carece de conduta e manejo terapêutico robustos. A ausência de sinais ou sintomas específicos dificulta sua suspeição clínica, sendo seu diagnóstico confirmado majoritariamente após a exérese da lesão vulvar com análise histopatológica. A abordagem cirúrgica, com ou sem linfadenectomia, é preferencial para o carcinoma adenoide cístico da GB e observa-se alta taxa de recidiva local, com tendência a metástase hematogênica e invasão perineural, destacando-se a importância da investigação com solicitação dos exames de imagem pertinentes. A recorrência da lesão é um aspecto importantíssimo associado e foi observada na paciente em questão, o que amplia a discussão a respeito da conduta cirúrgica preconizada nesses casos.
