Major complications after removal of the Nuss bar occur with some frequency. Although the double-bar removals in our cohort were associated with major complications, the reasons are poorly understood. Immediate management of the complications may require multidisciplinary care. Multicentric pooling of cases is needed for better risk stratification.
Bu çalışmada ileri amfizem nedeniyle endobronşiyal akciğer hacim küçültme uygulanan hastalarda endobronşiyal sarmal ile deneyimimiz sunuldu. Ça lış mapla nı:Çalışmaya ileri amfizem nedeniyle endobronşiyal sarmal ile endobronşiyal akciğer hacim küçültme uygulanan 46 hasta (45 erkek, 1 kadın; ort. yaş 61.7±8 yıl; dağılım, 43-80 yıl) dahil edildi. Hastaların yaşı, cinsiyeti, pulmoner fonksiyon testleri, tedavi sonrası morbiditesi, mortalitesi, tedavi öncesi ve sonrası (6 ay) altı dakika yürüme mesafesi, modifiye Medikal Araştırma Konseyi dispne skorları, kronik obstrüktif akciğer hastalığı değerlendirme testi ve Hastane Anksiyete ve Depresyon Ölçeği skorları kaydedildi. Bul gu lar: Hastaların ortalama sigara kullanım öyküleri 65 paket/yıl idi. Lob başına ortalama 11 (dağılım, 9-15) sarmal yerleştirildi (sağ üst lob=35, sol üst lob=19, sağ alt lob=2, sol alt lob= 4). Ortalama takip süresi 12.6 ay (±5.6 ay) idi. Tedavi sonrası birinci saniye zorlu ekspirasyon volümü, rezidüel volüm ve altı dakika yürüme mesafesi değerleri istatistiksel olarak anlamlı şekilde düzeldi. Yaşam kalitesinde de modifiye Medikal Araştırma Konseyi, kronik obstrüktif akciğer hastalığı değerlendirme testi ve Hastane Anksiyete ve Depresyon Ölçeği skorlarıyla ölçümlenen belirgin düzelme görüldü. Ameliyatın hemen sonrasında majör komplikasyon yaşanmazken 30 gün içinde üç hastada kronik obstrüktif akciğer hastalığı alevlenmesi, iki hastada pnömoni ve bir hastada önceki nörolojik hastalığın nüksü gelişti. So nuç: Endobronşiyal sarmal uygulaması ileri amfizemli seçilmiş hastalarda akciğer hacim küçültme cerrahisi ile karşılaştırıldığında daha düşük morbidite ve mortalite ile beraber pulmoner fonksiyonlarda ve yaşam kalitesinde belirgin düzelme sağlar. ÖZAmaç: Pektus ekskavatumun, Nuss barları ile minimal invaziv onarımında internal torasik arter (ITA) tıkanması literatürde bildirilmiştir. Bu çalışmada, ITA tıkanıklığının, Nuss bar takılmasından ve çıkarılmasından sonraki durumununun renkli Doppler ultrasonografi ile değerlendirilmesi amaçlanmıştır.Gereçler ve Yöntemler: Nuss bar operasyonu uygulanan 19 hasta çalışmaya prospektif olarak dahil edilmiştir. Renkli Doppler ultrasonografi ile Nuss bar takıldıktan sonra ve Nuss bar çıkartıldıktan sonra ITA akımı karşılaştırılmıştır. Hastalar cinsiyet, yaş, deformitenin simetrisi, Haller indeksi ve takılan bar sayısı açısından değerlendirilmiştir.Bulgular: Nuss bar operasyonu uygulanan 19 pektus ekskavatumlu hasta (17 erkek, 2 kadın, ortalama yaş: 21,78±6.35) çalışmaya alındı. Sekiz hastanın (%42,1) bar çekimi öncesi ve sonrası ITA akımı normal olarak değerlendirildi. Bar çekimi öncesi muayenede 6 hastada çift taraflı (n=2), ya da tek taraflı (n=4) ITA tıkanıklığı, ve 5 hastada çift taraflı (n=2), ya da tek taraflı (n=3) ITA akım bozukluğu tespit edildi. Bar çekildikten sonra etkilenen hastalardan (%57,9) sadece 2 hastada (%10) tıkanma devam etmekteydi. For more than 20 years, minimally invasive repair ABSTRACT Objective: Obstruction of the internal thoracic arteries (ITAs) by the Nuss bars in minimally invasive repair of pectus excavatum (MIRPE) has been documented in the literature. In this study, we aim to investigate the patency of ITAs before and after repair of the pectus excavatum with the Nuss procedure, by using color Doppler ultrasonography (US). Materials and Methods:Nineteen patients who underwent Nuss procedure were included in this study prospectively. Color Doppler US examinations were performed for comparing ITA patency after insertion of the Nuss bars and after their removal. Patients were evaluated according to gender, age, symmetry of deformity, Haller index, and the number of Nuss bars inserted.Results: Nineteen patients operated with MIRPE (17 male and 2 females with a mean age 21.78±6.35) were enrolled in the study. Objectives: Thoracotomy causes intense postoperative pain which may become chronic. Video-assisted thoracic surgery (VATS) leads to less postoperative pain compared with thoracotomy. In this study, we analyzed pain scores in patients who underwent lung resections with VATS or thoracotomy. Patients and Methods: Patients who underwent lung resections with uniportal, biportal VATS or thoracotomy between May 2015 -May 2017 were included in the study. Visual Analogue Scale (VAS) pain scores were recorded on postoperative day 1, 5 (or at discharge), 2nd week, 1st and 3rd months. Patients were classified in 3 groups, uniportal VATS (n=178), biportal VATS (n=15), thoracotomy (n=60). Demographics, resection type, mortality, morbidity and epidural catheter use were recorded. Results: Two hundred and fifty-three patients (average age was 57.3 ± 12.7, 94 females) were included in the study. Median hospital stay was 5 days. Uniportal and biportal groups had significantly lower pain scores in all intervals compared with thoracotomy. No chronic pain was seen in VATS groups. Uniportal and biportal groups had similar pain scores at all times. Epidural use or size of specimen did not affect pain in VATS patients (p=0.18 vs p=0.68). Conclusion: Video-assisted thoracic surgery decreases the need for epidural patient control analgesia. Specimen size does not affect postoperative pain and chronic pain is rare.
