Purpose To present a preliminary clinical experience and a dosimetric comparison of kidney‐sparing volumetric modulated arc therapy (VMAT) with three‐dimensional conformal radiotherapy (3D‐CRT) for whole abdominal irradiation (WAI), in the setting of Wilms tumor (WT) treatment. Materials and methods From a total of 20 consecutive WT cases treated with adjuvant irradiation, seven were submitted to WAI with VMAT. Renal function and survival rates were evaluated, and, for comparison purposes, similar VMAT and 3D‐CRT treatment plans were performed for WAI patients, and differences were dosimetrically evaluated regarding doses to the remaining kidney and other organs at risk and the planning target volume (PTV). Results After a median follow‐up time of 40.8 months (35.3‐52.2), no acute significant intestinal toxicity was observed, and median creatinine clearance was 110.1 and 103.3 mL/min/1.73 m², respectively, before treatment and at last follow‐up for WAI patients (P = 0.128). For comparative plans, maximum and median doses were lower for the remaining kidney with VMAT than with 3D‐CRT. VMAT was associated with better PTV coverage as compared with 3D‐CRT, with superior results for all the evaluated parameters (D95, D2, V100%, V98%, V95%; P = 0.018). Conclusion The use of VMAT technique is associated with lower radiation doses to the remaining kidney and better coverage to the PTV than 3D‐CRT technique for WAI, with preliminary clinical experience showing a favorable toxicity profile. Long‐term results from prospective studies might prove the ability of VMAT to spare renal function in the setting of WT treatment.
O câncer de estômago é uma das neoplasias mais frequentes no Brasil, figurando, na Bahia, como a primeira causa de morte por câncer durante as últimas décadas. O objetivo deste trabalho é descrever a tendência da taxa de mortalidade por câncer de estômago no estado da Bahia e na cidade de Salvador, de 1980 a 2007. Trata-se de estudo de agregados cujos dados sobre os óbitos e sobre a população foram obtidos no SIM/Datasus e no IBGE/Datasus, respectivamente. Para a análise da tendência temporal utilizou-se a regressão de Poisson, com avaliação de superdispersão. Os resultados representam redução ou incremento percentual médio anual, ajustadas pelo número de óbitos por causa mal definida. Em Salvador, observa-se uma tendência geral de queda nas taxas padronizadas de mortalidade por câncer gástrico de 2,31% entre os homens e de 2,58% entre as mulheres. Já no estado da Bahia, observa-se uma leve tendência crescente, tendo os homens um aumento de 0,45% e as mulheres de 0,05%. Conclui-se que as estratégias para a prevenção do câncer de estômago devem incluir melhorias no saneamento básico, mudanças no estilo de vida da população e modificação do consumo alimentar (melhores métodos de conservação dos alimentos). A ação governamental deve incluir uma política que viabilize o acesso à energia elétrica e facilite a aquisição de refrigeradores, além do fomento de programas de educação para a saúde voltados para a prevenção primária.
using Cox regression was used to assess the association of OS and variables including RT, age, and year of diagnosis. Results: 1382 patients diagnosed with TALL were identified. The median age at diagnosis was 9 years. There was a significant increase in the use of RT with 48.1% of patients between 2003 and 2012 receiving RT compared with 35.4% between 1980 and 2002 (P<0.001). Age at diagnosis and sex were not associated with differential use of radiation. On univariate analysis, RT was associated with an improvement in 5-year OS compared with no RT (81.6% vs. 73.7, P<0.001). OS was also improved for patients between 2003 and 2012 compared with 1980 and 2002 (84.8% vs. 69.0%, P<0.001), and age less than 10 years compared with age 10 years or older (80.6% vs. 73.1%, PZ0.001). On multivariate analysis, RT use (HRZ 0.672, PZ0.002), year of diagnosis between 2003 and 2012 (HRZ 0.464, P<0.001), and age at diagnosis <10 years (HRZ 0.635, P<0.001) all independently predicted for improved OS. Conclusion: In contrast to the majority of pediatric malignancies where RT use is decreasing, utilization of RT for TALL has been increasing. RT use is associated with improved OS independent of year of diagnosis and age of the patient. Limitations include the absence of data in the SEER database regarding chemotherapy, RT site, and CNS risk stratification of ALL patients.
O câncer de mama, doença crônica com repercussões orgânicas, econômicas e psicossociais, é a principal doença neoplásica do sexo feminino, tanto em países desenvolvidos quanto naqueles em desenvolvimento, e constitui-se em problema de saúde pública de dimensões mundiais. O objetivo deste estudo é descrever a tendência da taxa de mortalidade por câncer de mama feminina no estado da Bahia de 1980 a 2007. Os dados sobre os óbitos e sobre a população foram obtidos no SIM/Datasus e no IBGE/Datasus, respectivamente. Calcularam-se as taxas de mortalidade por faixa etária, taxas brutas e taxas padronizadas por idade, ajustadas pelo número de óbitos por causa mal definida. Para a análise da tendência temporal, utilizou-se a regressão de Poisson, com avaliação de superdispersão. Os resultados apontam que, em Salvador, a taxa bruta teve um aumento médio anual de 1,13% e a taxa padronizada, uma redução de 0,34%. A inspeção da curva revela um incremento de 0,46% até 1997 e uma redução de 2,33% desse ano em diante. Observou-se uma tendência crescente de 1,39% na faixa etária de 20 a 49. Entre as mulheres com idade maior ou igual a 50 anos ocorreu uma redução de 2,73% a partir de 1997. No estado da Bahia, as taxas de mortalidade padronizadas variaram de 4,71/100.000 em 1980 a 7,92/100.000 em 2007. Observou-se um aumento de 2,70% na taxa não padronizada por idade, ajustada pelo número de óbitos por causa mal definida. A taxa padronizada experimentou um aumento médio anual de 1,52%. As taxas ajustadas pelo número de óbitos por causa mal definida entre as mulheres abaixo e acima de 50 anos tiveram um incremento de 3,02% e de 2,45%, respectivamente. Concluiu-se que se verifica um aumento gradativo da mortalidade por câncer de mama feminina na Bahia e uma discreta tendência à redução na cidade de Salvador. Um período maior de observação será necessário para a verificação da manutenção dessas tendências.
