Therapie betrug im Stadium 1 und 2 64 % (n = 25) und im Stadium 3 und 4 0 % (n = 9). Mit adjuvanter Strahlentherapie betrug sie im Stadium 1 und 2 71,4 % (n = 14) und im Stadium 3 und 4 28,5 % (n = 14). Ffir die adjuvante Chemotherapie (Cyvadic-Schema) konnte bisher nur ein 3-Jahres-(2berlebenszeitraum ermittelt werden. In dieser Gruppe betrug die Absterberate 9,9 %, w~ihrend von den Patienten aus dem gleichen Zeitraum ohne Chemotherapie 18,5 % starben.
Surgical Problems of the Semimalignant DesmoidSummary. The semimalignant desmoid should be radically exstirpated with a security zone of 2 cm. By adopting this technique we had no local recurrence in 9 of our 12 patients, whereas in 3 patients who had been operated on in different clinics, 13 local recurrences were removed. Ultraradical procedures like amputation or exarticulation are not justified with regard to the histological dignity of the growth and its lack of metastases. Radiation therapy may be successful in extra-abdominal unresectable tumors. Hormonal and chemotherapy has not yet proven effective. Local recurrence can be avoided by early detection and radical local excision.
Zusammenfassung. Bei lokalradikaler Exstirpation erzielten wit bei 9 yon 12 Patienten Rezidivfreiheit beiWahrung eines allscitigen Sicherheitsabstandes yon 2 cm, w/thrend bei 3 ausw~irts primfiroperierten Patienten 13 Rezidive entfernt wurden. Ultraradikale MaBnahmen (Amputation, Exartikulation) halten wir nicht ffir gerechtfertigt angesichts der histologischen Benignit~it und Metastasenfreiheit. Bestrahlungstherapie bei nicht resektablen extraabdominalen Lokalisationen kann erfolgreich sein, der Weft von Hormon-und Chemotherapie ist nicht erwiesen. Summary.The hemangiopericytoma is a very rarely occurring sarcoma of the soft tissues. At the Surgical Department of the University of Essen from 1962-1980 ten cases of hemangiopericytoma were treated by operation. During a follow-up period of 5-10 years local recurrence was seen in six cases, metastasis in five cases. Only three patients survived to the end of the follow-up period, Wide surgical excision is the preferred treatment, and post-operative irradiation should be done although clinical studies are contradictory.
An 380 Patienten wurden insgesamt 432 Colon-Kontrastdarstellungen vorgenommen. In 57 Fällen (150/o) konnte die Diagnose eines Dickdarmtumors röntgenologisch gestellt werden; die histologische Sicherung der Befunde gelang bei 46 Patienten. Das konventionelle Untersuchungsverfahren in Form eines Bariumeinlaufes erbrachte in 47 Fällen einen positiven Tumorbefund, während bei 30 Doppelkontrastdarstellungen des Kolons nach der Methode von Welin-Hettler zehnmal ein Tumor diagnostiziert wurde, der in jedem Fall sowohl chirurgisch als auch histopathologisch bestätigt werden konnte. Diese Ergebnisse sprechen für eine höhere Treffsicherheit des Doppelkontrastverfahrens besonders bei der Diagnostik von polypösen Darmtumoren. Die insgesamt aufwendigere, aber diagnostisch sicherere Methode des Doppelkontrasteinlaufes sollte vor allem dann angewandt werden, wenn eine Erstuntersuchung in Form eines einfachen Kolon-Kontrasteinlaufes bei Tumorverdacht nicht zu einem befriedigenden diagnostischen Ergebnis führte. Die rechtzeitige Erkennung von Kolontumoren ist entscheidend für die Frühbehandlung des Kolonkarzinoms und dessen relativ gute Prognose. Kontrastdarstellungen des Kolons sollten daher bei Patienten in der fünften und sechsten Lebensdekade in den Kreis der Vorsorgeuntersuchungen einbezogen werden. Kolontumoren stehen an Häufigkeit bei Frauen it 4,2% an fünfter und bei Männern mit 4,1% an sechster Steiler aller Todesursachen.
