Amaç: Üst çene birinci büyük azı dişlerin kanal tedavilerinin en önemli başarısızlık nedenleri arasında mesiobukkal 2. (MB2) kanalların varlığı gelmektedir. Oldukça karmaşık bir kök kanal anatomisine sahip bu dişlerin MB2 kanalların lokalizasyonu da çeşitlilik gösterebilmektedir. Bu çalışmada MB2 kanalların varlığının insidansının tespiti ile kanal ağızlarının meziobukkal 1. (MB1) kanal ağızlarına olan uzaklığının konik ışıklı bilgisayarlı tomografi(KIBT) görüntüleri kullanılarak ölçülmesi amaçlanmaktadır. Gereç Ve Yöntemler: Çeşitli nedenlerle çekilmiş rastgele seçilmiş 300 adet KIBT görüntüsü retrospektif olarak taranarak kanal tedavisi görmemiş en az bir adet üst birinci büyük azı dişi bulunan hastalara ait 126 adet görüntü seçilmiştir. Bu görüntüler içerisinden 191 adet diş çalışmaya dahil edilmiştir. Dişler bir ağız diş ve çene radyolojisi uzmanı tarafından MB2 kanal varlığına göre ayrılmış ve MB2 kanal ağızlarının MB1’e göre uzaklığı birkaç gün arayla iki kez ölçülmüştür. İki ölçümün ortalaması alınarak tek bir ölçüm elde edilmiş ve veriler IBM SPSS Version 22.00 istatistik programıyla analiz edilmiştir. Bulgular: Çalışmaya dahil edilen 190 adet diş içerisinden 82(%43,2) tanesinde MB2’e rastlanmıştır. MB2 kanalının MB1’e olan uzaklığının ölçümlerinde kadın ve erkek arasında anlamlı fark bulunurken (p=0,03), sağ ve sol dişler açısından anlamlı bir fark bulunamamıştır (p=0,829). İki kanal ağzının birbirine olan uzaklığı tüm dişlerde ortalama 2,56 ±0,34mm olarak tespit edilmiştir. Sonuçlar: Üst birinci büyük azı dişlerinde % 43,2 oranında MB2 kanal görülmektedir. Bu çalışma MB1 e göre konumu ve uzaklığı konusunda endodontist ve klinisyenlere bilgiler sunarak söz konusu dişlere yapılan kanal tedavisinin başarısının artması konusunda yön gösterici olabilir. SummaryBackground/Aim: The aim of this study was to evaluate and compare the imaging characteristics of common, radiolucent, unilocular, intraosseous lesions of the jaws using both panoramic radiography and cone beam computed tomography (CBCT); also, to evaluate sufficiency of panoramic radiography in determining characteristic features of jaw lesions.Material and Methods: Retrospectively selected images of 57 patients with histopathology results were evaluated by two oral radiologists. The lesions were assessed based on shape, location, borders, relationship with the mandibular canal, presence of destruction of cortical bone, and expansion of cortical bone, and presence of an unerupted tooth related to the lesion. In addition, the widest areas of the lesions were measured. A total of 9 (15.8%) odontogenic keratocysts, 9 (15.8%) apical granulomas, 24 (42.1%) radicular cysts, 12 (21.0%) dentigerous cysts and 3 (5.2%) central giant cell granulomas in 57 patients (20 women, 37 men) with a mean age of 36.93 ± 17.96 years were included. Fifty-seven CBCT and 56 panoramic images of these patients were evaluated.Results: Twenty-nine (50.8%) lesions were in the mandible and 28 (49.2%) in the maxilla. A statistically significant difference was determined for the areas in CBCT images (p=0.007).Conclusions: Panoramic radiography is not as successful as CBCT in demonstrating some characteristics of the lesions, such as expansion and destruction. The area measurements may be beneficial in establishing the differential diagnosis of the lesion.
İlave dişler, 20 adet süt ve 32 adet daimi dişe ek olarak gelişen diş ya da diş benzeri yapıları tanımlamak için kullanılmaktadır. Süpernümerer de denen bu ilave dişlerin çevresel ve genetik etkilerle oluştuğu düşünülse de etyolojisi tam olarak aydınlatılamamıştır. Çoklu süpernümerer dişler genelde kleidokranial displazi, Gardner sendromu ve Cruzon sendromu gibi çeşitli sendromlara sahip bireylerde gözlenmektedir. Sendrom ile ilişkili olmayan ve çok sayıda süpernümerer dişe sahip vakalar oldukça nadir gözlenmektedir. Bu nedenle, bu makalede hiçbir sendromu bulunmayan iki adet çoklu süpernümerer dişe sahip olgunun sunumu amaçlanmıştır. İlk olguda; 27 yaşında erkek hastada bir adet sürmüş altı adet gömülü olmak üzere yedi adet süpernümerer diş tespit edildi. İkinci olguda; 23 yaşında erkek hastada beş adet sürmüş yedi adet gömülü toplam on iki tane süpernümerer diş saptanmıştır.
Amaç: Bu çalışmanın amacı, panoramik radyograflar kullanılarak styloid ligament ossifikasyon (SLO) varlığı ile molar dişlerde pulpa taşı varlığı arasında anlamlı bir ilişki olup olmadığını incelemek ve bu kalsifikasyonların görülme sıklığını ve cinsiyetler arasında fark olup olmadığını değerlendirmektir. Gereç ve Yöntemler: Retrospektif olarak 2000 adet panoramik radyograf içinden herhangi bir anomali, kök yada kron kırığı, kanal tedavisi, protez kron veya derin dolgusu olmayan birinci ve ikinci molar dişlere sahip 487 adet görüntü (276 kadın, 211 erkek) seçildi. Oral radyolog olan iki gözlemci birlikte bu görüntüleri değerlendirdi ve SLO varlığını tek taraflı, iki taraflı veya var olmayan şeklinde kategorize etti ve pulpa kalsifikasyonuna sahip molar diş sayısını da not ettiler. Bu veriler IBM SPSS (sürüm 21) ile analiz edildi. SLO ve pulpa taşı arasındaki ilişki için Pearson korelasyon analizi ve cinsiyetler arasındaki fark için ise ki-kare testi kullanıldı. Ayrıca insidans için tanımlayıcı istatistikler ve frekans dağılımı kullanıldı. Bulgular: SLO ile pulpa kalsifikasyonu varlığı arasında anlamlı bir ilişki bulunamamıştır (p=0.282). Ayrıca, cinsiyetler arasında SLO varlığı açısından anlamlı bir fark bulunamamıştır (p=0.53). Bununla birlikte, pulpa kalsifikasyonu sayısı açısından cinsiyetler arasında anlamlı bir fark görülmüştür (p=0.021). Sonuç: Her ne kadar populasyonda SLO ve pulpa kalsifikasyonu sıklığı yüksek görülse de, bu kalsifikasyonlar arasında herhangi bir ilişki bulunamamıştır.
