INTRODUÇÃOOs algoritmos e técnicas para acesso à via aérea difícil em adultos já estão bem estabelecidos, porém essas mesmas técni-cas muitas vezes não podem ser aplicadas a recém-nascidos ou crianças pequenas.RELATO DO CASO RN de 3 dias, pesando 3 kg, seguiu para cirurgia corretiva de rânula. Foram administrados 0,05 mg/kg de midazolam IV e realizou-se anestesia tópica em cavidade oral para diminuir a reatividade. Foram injetados 12 mg de propofol IV e efetuou-se laringoscopia direta, tendo-se observado retração intensa da lín-gua. A exposição da epiglote e das pregas vocais não foi possível. Com fio de nylon na língua, tentou-se tracioná-la e melhorar as condições da laringoscopia. Essa técnica não se mostrou efetiva. Foi alocada então uma máscara laríngea (ML) n o 1 e a criança foi ventilada com sucesso. Em seguida passou-se um fio-guia de sonda de Doboff peça ML sem dificuldades, sendo então retirada a ML. Foi introduzida uma cânula n o 3,5 com "cuff", direcionada pelo fio-guia. No preparo do paciente ocorreu extubação acidental. Foram repetidos os procedimentos anteriores, porém o fio-guia não progrediu por bater na ranhura da ML. Retirou-se a ML e passou-se o fio-guia pela ranhura antes de posicioná-la. Foram repetidas as manobras com sucesso. DISCUSSÃO Usualmente, não estão disponíveis, no centro cirúrgico, broncofibroscópios de diâmetros suficientes para a intubação de crianças de pouca idade. A intubação retrógrada em RN é difícil devido ao pequeno calibre das vias aéreas. A utilização de ML pode ser uma opção, contudo há relatos de necessidades de maiores pressões para ventilar esses pacientes, devido à obstrução parcial da epiglote quando se usa ML nº 1 e 1,5 (2). Além disso, utiliza-se a ML para guiar o broncofibroscópio. Nós utilizamos a ML para introduzir o fio-guia e, nas duas tentativas, a intubação foi conseguida com sucesso sem maiores dificuldades.
