Обсуждаются вопросы перестройки сократительных и цитоскелетных белков миокарда при кардиомиопатиях (КМП). Анализируются роль генетических факторов, кодирующих сократительные белки саркомеров, белков входящих в структуры тонких и толстых филаментов, а также белков внеклеточного матрикса в процессах формирования и развития гипертрофической (ГКМП) и дилатационной (ДКМП) кардиомиопатий. Рассматриваются механизмы влияния изомерно-измененных белков на регуляцию генерации силы, её передачи, утилизацию АТР, нарушение трансмембранных сигналов, что в конечном счете приводит к дисфункции кардиомиоцитов и может обусловливать различные фенотипические проявления КМП.Ключевые слова: Хроническая сердечная недостаточность, сердечно-сосудистые заболевания, гипертрофическая кардиомиопатия, дилатационная кардиомиопатия, мутация генов.ВВЕДЕНИЕ. Молекулярная диагностика наследственно обусловленных врожденных пороков является одной из важных задач медико-биологической науки. Изучение молекулярных механизмов генетических изменений в развитии кардиомиопатий (КМП) составляет сегодня одно из приоритетных направлений кардиологии. Актуальность изучения КМП определяется их высоким удельным весом в структуре сердечной заболеваемости и смертности [1][2][3]. КМП длительное время рассматривались как заболевания с неизвестной этиологией. Установление мутации гена тяжелых цепей b-миозина в качестве причины развития семейной гипертрофической кардиомиопатии (ГКМП) в 1989 г. открыло новую эру в изучении генетически обусловленных заболеваний миокарда и, в частности, КМП [4]. Генетическая природа КМП получила свое подтверждение в последовавших аналогичных исследованиях [5,6]. В последние годы накоплены многочисленные данные, посвященные изучению полиморфных локусов кандидатных генов, вызывающие различные формы КМП [7,8].К настоящему времени известно, что большинство ГКМП и около 50% дилатационных кардиомиопатий (ДКМП) являются генетическими заболеваниями [9]. Обнаружено, что мутации генов, кодирующие сердечные белки, могут привести также к развитию рестриктивной (РКМП) [10], аритмогенной [11] и неклассифицированной кардиомиопатии [12].Кардиомиопатии характеризуются полиморфизмом клинических проявлений, связанных с тяжестью нарушений функций сердца, обусловленных состоянием сердечной мышцы.
