Ebstein-Anomalie und Trikuspidalklappen-Dysplasie Definition Die Ebstein-Anomalie ist durch eine Verlagerung des freien Anteils des septalen und posterioren Segels der Trikuspidalklappe apikalwärts in den rechten Ventrikel charakterisiert. Die Klappe bewegt sich nur eingeschränkt. Bei der Trikuspidalklappen-Dysplasie dagegen setzt die Klappe an der richtigen Stelle an, ist jedoch auffällig verdickt. Bei beiden Herzfehlern kommt es zu einer meist ausgeprägten Insuffizienz der dysplastischen Klappe, die konsekutiv zu einer Dilatation des rechten Vorhofs, einer Kardiomegalie und einer Verschiebung der Herzachse nach links führt. Der rechte Ventrikel erscheint verkleinert [41, 60, 61]. Inzidenz Diese seltenen, manchmal zyanotischen Herzfehler treten bei ca. 1% aller Lebendgeborenen auf [32]. Assoziierte Anomalien Zusätzliche extrakardiale Auffälligkeiten sind sehr selten, Skelettfehlbildungen sowie Chromosomenstörungen sollten aber ausgeschlossen werden [23, 33]. Intrakardial findet man bei der Ebstein-Anomalie in 30 % und bei der Trikuspidalklappen-Dysplasie in 80 % begleitend eine Pulmonalstenose oder-atresie [41, 60]. Ultraschalldiagnostik Im Vierkammerblick sind die Dilatation des rechten Vorhofs und die Kardiomegalie für beide Herzfehler typisch und richtungsweisend (• " Abb. 1). Begleitend liegt eine Trikuspidalklappeninsuffizienz (• " Abb. 2) und bei schweren Formen eine Pulmonalstenose oder-atresie vor, die mittels Farb-Doppler gut dargestellt werden können [62, 63]. Perinatales Management Detaillierte Organdiagnostik und Chromosomenanalyse sind zu empfehlen. Zur Beratung der betroffenen Eltern und prognostischen Einschätzung sollte ein Kinderkardiologe hinzugezogen werden. Im Verlauf sollten engmaschige Ultraschallkontrollen erfolgen, um frühzeitig kardiale Dekompensationszeichen bei einer oft ausgeprägten Trikuspidalklappeninsuffizienz zu erkennen [60]. Als schwere Komplikationen der Kardiomegalie können sich ein Hydrops fetalis oder durch den chronisch erhöhten intrathorakalen Druck eine Lungenhypoplasie entwickeln [62, 64]. Bei guter Herzfunktion ist die vaginale Entbindung mit neonatologischem Stand-by anzustreben. Da die Variationsbreite dieses Herzfehlers groß und die klinische Symptomatik stark von einer zusätzlichen rechtsventrikulären Ausflussobstruktion abhängig ist, präsentieren sich die Neugeborenen postpartal von nahezu unauffällig bis akut zyanotisch. Bei ausgeprägter Zyanose ist fast immer eine frühzeitige chirurgische Intervention erforderlich, sodass die Entbindung in einem Zentrum mit entsprechenden Versorgungsmöglichkeiten erfolgen sollte. Prognose Bereits pränatal diagnostizierte Ebstein-Anomalien zeichnen sich insgesamt durch eine schlechtere Prognose aus als bei postnataler klinischer Manifestation [65]. Feten mit intrauteriner Trikuspidalklappeninsuffizienz und Kardiomegalie haben ein signifikant erhöhtes Risiko für eine
Überblick ! Herzfehler sind mit einer Inzidenz von 5 -8 auf 1000 Neugeborene relativ häufig [1, 2]. In Deutschland liegt die Prävalenz angeborener Herzfehler bei Lebendgeborenen in einer aktuellen Erhebung des Kompetenznetzes angeborener Herzfehler bei 7,6 %. Die häufigsten Herzfehler sind dabei der Ventrikelseptumdefekt, der Vorhofseptumdefekt und die Pulmonalklappenstenose, die zusammen ca. 70 % der Vitien ausmachen (PAN-Studie, Kompetenznetz angeborene Herzfehler: http://www.kompetenznetz-ahf.de). Da ca. 15 -20% aller Schwangerschaften im 2. Trimenon als Spontanabort enden und zu einem großen Teil mit Herzfehlbildungen assoziiert sind, begegnet dem Pränataldiagnostiker somit eine noch höhere Rate an kongenitalen Herzvitien als bei Geburt. In ca. 50 % der Fälle handelt es sich um schwere Herzfehler, die einer operativen Behandlung bedürfen [1, 3]. Ca. 20% der Behandlungen angeborener Herzfehler müssen bereits in der Neonatalperiode erfolgen. Unter heutigen optimierten Therapiemöglichkeiten liegt die Mortalität bei Herzoperationen im Neonatalalter in der aktuellen Qualitätssicherungsdatenbank der europäischen Gesellschaft für Herzchirurgie bei 11% (EACTS Congenital Database, http://www. eacts.org/index.html). Die Mortalität korreliert eng mit dem Schweregrad des Herzfehlers und dem Zeitpunkt der klinischen Manifestation [2 -4]. Die Detektionsrate der angeborenen Herzfehler bei der pränatalen Ultraschalluntersuchung ist vom Untersuchungszeitpunkt, der persön-lichen Kompetenz des Pränataldiagnostikers und der apparativen Ausstattung abhängig und schwankt nach Literaturangaben zwischen 5 und 85% [3 -5]. Im Rahmen der Pränataldiagnostik ist es wichtig, zwischen einem High-(Risiko ca. 10 %) und einem Low-Risk-Kollektiv (Risiko) < 0,5 %) für angeborene Herzfehler zu differenzieren. Feten mit einem hohen Risiko für eine kongenitale Herzfehlbildung sollten frühzeitig einem in der fetalen Echokardiografie erfahrenen Untersucher vorgestellt werden. Unglücklicherweise werden jedoch ca. 90 % aller angeborenen Herzvitien postpartal im Niedrigrisikokollektiv diagnostiziert [6].
