A 68-year-old woman with a history of multiple myeloma presented to the clinic with pain and vision loss in her right eye. Proptosis was observed in her right eye and eye movements were restricted in all directions. Best corrected visual acuity was 3/10 in her right eye. On biomicroscopic examination, hyperemia and subconjunctival hemorrhage were present. Fundus examination of the right eye revealed optic disc edema and choroidal folds. In magnetic resonance imaging two orbital masses were detected. Based on the patient’s history and ocular examination, we evaluated the masses as orbital metastasis of multiple myeloma. Palliative radiotherapy was recommended.
Mersin il merkezinde çalışan gözlükçülerin kontakt lens (KL) satışına ve kullanımına bakış açılarını değerlendirmek. Ge reç ve Yön tem: Mersin ili, 4 merkez ilçesinde yer alan toplam 20 gözlükçü çalışmaya dahil edildi. Daha önce hazırlanan anket formunun katılımcılar tarafından doldurulması sağlandı. Bulgular: Çalışmaya alınan gözlükçülerin %78,9'u reçetesiz KL satışı yapıyorken, %25'i aynı zamanda KL uygulaması da yapıyordu. Katılımcıların %75'i KL'ler ile ilgili bilgilendirme almamışken, %65'i bilgi düzeyi yönünden kendisini yetersiz görüyordu. Ayrıca katılımcıların yarısı KL kullanımı sırasında ortaya çıkacak problemler konusunda yeterli bilgi sahibi değilken, %75'i KL'ler hakkında bilgilendirme almak istiyordu. Sonuç: Bu anket sonucunda, çalışmamıza katılan satıcıların önemli bir kısmının reçetesiz olarak KL satışı ve uygulamasında bulunduğu görülmüştür. Komplikasyonların önüne geçilebilmesi için gözlükçülerin denetiminin yapılması, kullanıcıların eğitimi kadar önem taşımaktadır. Ma te ri als and Met hods: Twenty opticians who work in the 4 main districts in the provincial centre of Mersin were included in the study. A questionnaire form which was prepared before was filled out by the participants. Re sults: Seventy-eight percent of the opticians were selling CL without prescription, whereas 25% also carried out its fitting. Seventyfive percent of the participants did not get any education about contact lenses, while 65% of them were feeling themselves disqualified about CLs. Furthermore, half of the participants did not have enough knowledge about the complications associated with CL usage and 75% of them wanted to receive information about CLs. Conclusion: As a result of the questionnaire, we found out that a significant part of the participants were both selling and fitting contact lenses without prescription. We believe that auditing the opticians is as important as educating the contact lens users.
IntroductionAdhesion, fibrosis, and scarring formation following strabismus surgery are among the most important problems that unfavorably influence surgical success. The exact incidence of scars, adhesion, and fibrosis has not been reported but Ludwig retrospectively reviewed 134 reoperations in which a stretched scar was discovered and estimated that stretched scar tissue was a contributing factor in 10% of all strabismus reoperations and 50% of late overcorrections (1). Adhesion is the attachment that occurs in some tissues including the conjunctiva, Tenon's capsule, the intermuscular membrane, orbital adipose tissue, the sclera, and extraocular muscular tissue due to any reason during or after strabismus surgery. These adhesions may result from inappropriate surgical approach, excessive bleeding, cautery usage, suture reaction, postoperative infection, muscular capsule injury, migration of adipose tissue to the surgical area due to the laceration of the orbital septum, and multiple procedures performed in the same region (2). Fibrosis is the contracture occurring after inflammation of extraocular muscles. Appropriate surgical techniques should be performed to avoid postoperative adhesion and fibrosis and subsequent scarring (2,3).Adhesion, fibrosis, and scar formation after strabismus surgery bring along unfavorable consequences for both the patient and the surgeon. Need for a second surgery not only psychologically influences the patient negatively but also leads to further tissue injury during the second intervention and causes additional expense. Reducing adhesion, fibrosis, and scarring after strabismus surgery would provide results that are more satisfactory for both patients and surgeons by enhancing anatomical success. For this purpose, many materials and drugs have been studied in experimental or clinical trials; nevertheless, no Background/aim: The present study aimed to investigate the efficacy of mitomycin-C (MMC) and infliximab (INF) in reducing adhesion and fibrosis following strabismus surgery. Materials and methods:Forty eyes of 20 albino rabbits were separated into MMC and INF groups. Right and left eyes of rabbits were assigned to the drug and control groups, respectively. The superior rectus muscle was disinserted, the drug was administered to the surgical area for 5 min in the drug eyes (MMC 0.2 mg/mL or INF 5 mg/mL), and physiological saline was administered to the control eyes. Surgical areas were rinsed with 10 mL of physiological saline. The disinserted muscle was then sutured to the same area using 6.0 Vicryl. The rabbits were sacrificed after 4 weeks for histopathological examination.Results: Significant reduction was observed in fibrosis in the INF group as compared to the control group (P = 0.005). Although adhesion formation in the drug eyes reduced in the MMC and INF groups as compared to the control group, the difference was not significant (P = 0.280 and P = 0.579, respectively). Amaç: Okuler kolobom embriyonik optik yarığın kapanma defekti sonucunda oluşan nadir bir malformasyondur. İris, koroid, retina, optik sinir ve siliyer cismi tutabilir. Bu çalışmada kliniğimize başvuran değişik oküler tutulumu olan kolobomlu olguların klinik ozellikleri, eşlik eden bulguları ve takipleri açısından sunulması planlanmıştır. Ge reç ve Yön tem: Calışmamızda kliniğimize başvuran ve iris kolobomu tanısı alan 10 olgunun 16 gözü retrospektif olarak değerlendirimiştir. Sonuçlar: Olguların 7' si sporadik, 3'ü ailesel idi. Olguların birinde iris kolobomu izole iken, diğerlerinde iris kolobomuna koryoretinal kolobom eşlik etmekte idi. Yedi olguda optik disk tutulumu mevcuttu. Eşlik eden göz bulguları icinde 3 olguda tek taraflı mikroftalmi, 1 olguda tek taraflı anoftalmi, 1 olguda da tek taraflı mikrokornea saptandı. Ailesel olgularda eşlik eden şaşılık mevcuttu. It can affect iris, retina, choroid, optic disc or ciliary body. This study reviews the clinical diagnosis and the accompanying ocular and systemic abnormalities in cases of iris coloboma. Ma te ri al and Met hod: Sixteen eyes of ten patients referred to our clinic and diagnosed as iris coloboma were included in the study and were reviewed retrospectively. Re sults: Seven cases were sporadic, and three cases were familial. Isolated iris coloboma was present in only one case, and chorioretinal involvement was present in all the others. Seven cases had involvement of the optic disc. Other ocular anomalies were unilateral microphthalmia in 3 cases, unilateral anophthalmia in 1 case, and unilateral microcornea in one case. Strabismus was present in the familial cases. Dis cus si on: In patients, diagnosed with iris coloboma, routine eye examination should be completed, refractive errors should be corrected, and these patients must be follewed for amblyopia.
