TEMA: articulação compensatória na fissura palatina. OBJETIVO: contribuir para o aprofundamento de informações sobre os tipos de articulação compensatória descritos na literatura e, ainda, discutir as implicações e contribuições da avaliação clínica e instrumental na identificação destas produções. CONCLUSÃO: as articulações compensatórias merecem a atenção de clínicos e pesquisadores que atuam no Brasil, já que estas alterações são encontradas com grande freqüência em crianças e adultos com fissura palatina ou disfunção velofaríngea, o que compromete a qualidade de vida destes sujeitos. Os fonoaudiólogos devem aprofundar seus conhecimentos sobre os tipos de articulação compensatória e os procedimentos de avaliação, bem como devem estabelecer programas preventivos que favoreçam a aquisição fonológica sem o desenvolvimento dessas compensações.
Purpose: evaluation of the speech recognition concerning the word predictability based on a developed test. Methods: It was perfomed a clinical history, cognitive impairment and depression screening tests and conventional audiological conventional evaluation. A Speech Recognition Test using high-and low-predictability sentences has been developed and applied to 36 elderly (G1 and G3 -groups with normal hearing thresholds of up to 4 kHz, and G2 and G4 -groups with mild--to-moderate sensorineural hearing loss). Two groups underwent the tests in the silence and in the noise (signal/noise ratio of +5 dB) and the other two groups only in the noise (ratio ranging from +5 dB to 0 dB). Results: In general, Speech recognition test showed a higher score in the high predictability sentences and better performance for groups without hearing loss. In the silence, elderly with and without hearing loss have also obtained high scores and the lowest scores were achieved in situations with most intense noise. The benefit of predictability was positive for groups 1, 2 and 3. Group 4 showed a different behavior towards the use of predictability (sometimes positive or negative). Conclusion: For the elderly evaluated and the differences observed in one of the groups, it was not possible to precisely determine how the elderly benefit from predictability. The hearing loss and the noise have negatively influenced the test performance. Further researches in the area are necessary to confirm the validity of the material produced.Keywords: Speech perception; Speech discrimination tests; Aging; Presbycusis; Cognition RESUMO Objetivo: Avaliar o reconhecimento de fala, considerando a previsibilidade da palavra a partir de um teste elaborado. Métodos: Foi realizada anamnese, testes de rastreio de comprometimento cognitivo e depressão e avaliação audiológica convencional. Foi desenvolvido um Teste de Reconhecimento de Fala utilizando Frases de Previsibilidade Alta e Baixa e aplicado a 36 idosos (G1 e G3 -grupos sem perda auditiva até 4 kHz e G2 e G4 -grupos com perda auditiva neurossensorial de grau leve a moderado). Dois grupos realizaram o teste no silêncio e com ruído (relação sinal/ruído +5 dB) e os outros dois grupos somente com ruído (relações +5 dB e 0 dB). Resultados: O teste de reconhecimento de fala revelou, de modo geral, maior pontuação nas frases de previsibilidade alta e melhores desempenhos para os grupos sem perda auditiva. No silêncio, os idosos sem e com perda auditiva obtiveram pontuação máxima igual e as menores pontuações ocorreram na condição de ruído mais intenso. O benefício da previsibilidade foi positivo para os grupos 1, 2 e 3. O grupo 4 apresentou comportamento variado quanto ao uso da previsibilidade (ora positivo, ora negativo). Conclusão: Com a população de idosos avaliados e as diferenças observadas em um dos grupos considerados, não foi possível compreender, com exatidão, como os idosos se beneficiam do apoio da previsibilidade. RESUMOObjetivo: comparar o desempenho de 40 pré-escolares no teste de localização sonora com as respostas de seus pais para um questionário que investigou a ocorrência de episódios de otite média (OM) e os sintomas indicativos de desordens audiológicas e do processamento auditivo. Métodos: após aplicação e análise das respostas do questionário, dois grupos foram formados: GO, com histórico de OM, e GC, sem este histórico. Cada grupo com 20 pré-escolares de ambos os gêneros foi submetido ao teste de localização da fonte sonora em cinco direções (Pereira, 1993). Resultados: a comparação entre GO e GC não mostrou diferença estatisticamente significante (p=1,0000). Conclusão: as otites recorrentes na primeira infância não influenciaram no desempenho da habilidade de localização sonora dos pré-escolares deste estudo. Embora sejam dois instrumentos baratos e de fácil aplicação, o questionário e o teste de localização não foram suficientes para diferenciar os dois grupos testados. . Por esta razão, a capacidade de localização é um exemplo claro da maneira como os recém-nascidos integram a experiência auditiva para, rapidamente, ter uma função quase plenamente desenvolvida no primeiro ano de vida 5 . Os dois primeiros anos de vida são o período crucial em que a experiência auditiva pode e deve ter o efeito mais marcante sobre a localização 5 e para o processo de maturação do sistema auditivo como um todo, assim como para o desenvolvimento das habilidades de linguagem da criança 4,6,7 . É neste período em que a criança está mais receptiva para o aprendizado e uma prova disso está na expansão do vocabulário entre o primeiro e o terceiro ano de idade, que passa de cerca de 50 para, aproximadamente, 1000 palavras. Dessa forma, privações sensoriais auditivas podem ser responsáveis por efeitos negativos no desenvolvimento cognitivo. OBJETIVO: Descrever o julgamento de concretude de um conjunto de 162 palavras da Língua Portuguesa, visando a elaboração de um teste de reconhecimento de fala, bem como verificar a influência de variáveis como frequência de ocorrência das palavras, idade e série do curso de graduação dos participantes neste julgamento. MÉTODOS: Cinquenta estudantes universitários de um curso de Fonoaudiologia de uma universidade pública julgaram 162 palavras quanto à concretude, por meio de uma escala de sete pontos, sendo o menor grau de concretude representado pelo número um e o grau de concretude máximo, representado pelo número sete. Foi permitido ao sujeito escolher qualquer ponto da escala. RESULTADOS: Os resultados mostraram uma distribuição tri-modal, sugerindo que as palavras fossem classificadas em três categorias quanto ao grau de concretude. A faixa de julgamento das palavras de baixa concretude variou de 1,76 a 3,45, as de média concretude, na faixa de 3,46 a 4,95 e as de alta concretude, de 4,96 e até 6,70. Houve correlação positiva entre o grau de concretude das palavras e o coeficiente de variação de forma que, quanto maior a nota atribuída para a palavra, menor foi a variação de respostas. Não ocorreu correlação significativa entre a frequência de ocorrência das palavras e o julgamento de concretude. A influência da idade e da série do curso de graduação no julgamento foi significativa para algumas correlações. CONCLUSÃO: Os resultados revelaram três categorias quanto ao grau de concretude e sugerem que a concretude pode ser considerada um atributo independente das palavras, uma vez que a frequência de ocorrência das palavras, bem como a idade e a série do curso de graduação dos participantes não influenciou o julgamento de concretude. As palavras categorizadas com alto grau de concretude foram utilizadas em etapas posteriores da elaboração de um teste de reconhecimento de fala.
The use of prosthetic devices for correction of velopharyngeal insufficiency (VPI) is an alternative treatment for patients with conditions that preclude surgery and for those individuals with a hypofunctional velopharynx (HV) with a poor prognosis for the surgical repair of VPI. Understanding the role and measuring the outcome of prosthetic treatment of velopharyngeal dysfunction requires the use of tools that allow for documenting pre- and post-treatment outcomes. Experimental openings in speech bulbs have been used for simulating VPI in studies documenting changes in aerodynamic, acoustic and kinematics aspects of speech associated with the use of palatal prosthetic devices. The use of nasometry to document changes in speech associated with experimental openings in speech bulbs, however, has not been described in the literature.ObjectiveThis single-subject study investigated nasalance and nasality at the presence of experimental openings drilled through the speech bulb of a patient with HV. Material and MethodsNasometric recordings of the word "pato" were obtained under 4 velopharyngeal conditions: no-opening (control condition), no speech bulb, speech bulb with a 20 mm2 opening, and speech bulb with 30 mm2 opening. Five speech-language pathologists performed auditory-perceptual ratings while the subject read an oral passage under all conditions. ResultsKruskal-Wallis test showed significant difference among conditions (p=0.0002), with Scheffé post hoc test indicating difference from the no-opening condition.ConclusionThe changes in nasalance observed after drilling holes of known sizes in a speech bulb suggest that nasometry reflect changes in transfer of sound energy related to different sizes of velopharyngeal opening.
