Introducción. La cancelación de cirugía programada es un indicador de calidad que puede relacionarse con ineficiencia en los procesos de planificación estratégica para la seguridad del paciente y es un fenómeno frecuente en las instituciones de salud.Objetivo. Identificar las causas de cancelación de cirugía programada durante el segundo trimestre de 2016 en una clínica de alta complejidad de Popayán, Colombia.Materiales y métodos. Estudio descriptivo retrospectivo de corte transversal con 185 cirugías canceladas de 2 787 programadas. La información se obtuvo de los registros magnéticos de la coordinación de cirugía de la institución objeto de estudio y se realizó una distribución del evento según los actores del Sistema General de Seguridad Social en Salud (SGSSS): asegurador, prestador y paciente.Resultados. La proporción de cancelación fue del 2.7%. El 56.7% de las causas fueron atribuidas al paciente, el 40.5% al prestador y el 2.7% al asegurador.Conclusiones. Las causas de cancelación de cirugía programada identificadas son altamente prevenibles y evidencian fallas organizacionales y procedimentales que requieren mejoramiento a nivel administrativo y asistencial para disminuir los costos de no calidad.
Antecedentes. Un estudio a nivel nacional permitió establecer los riesgos a los cuales estaban expuestos los trabajadores del sector informal del comercio. Sin embargo, no fue incluido el Departamento del Cauca.
Objetivo: establecer la validez de criterio de una herramienta para la detección de la pérdida auditiva en población infantil colombiana. Metodología: estudio de concordancia en 160 niños entre 5 y 10 años a quienes se les valoró la audición mediante dos pruebas: la “herramienta de detección de bajo costo para identificar y clasificar la pérdida auditiva”, diseñada en Brasil y la audiometría tonal como prueba oro. Se determinó la sensibilidad y la especificidad de la herramienta y la estimación de la concordancia entre las dos pruebas mediante el índice de Kappa. Los resultados fueron analizados con el paquete estadístico SPSS – 19 y Epidat 3.1. Resultados: la “herramienta de detección de bajo costo para identificar y clasificar la pérdida auditiva” presentó una sensibilidad del 35 %, especificidad del 25 %, un valor predictivo positivo del 12 %, valor predictivo negativo del 56 % y un Índice de Kappa de -0,24. Discusión y conclusiones: la “herramienta de detección de bajo costo para identificar y clasificar la pérdida auditiva” presenta poca capacidad para clasificar correctamente al enfermo como verdadero enfermo y al sano como verdadero sano. Además, la pobre concordancia entre las dos pruebas, no permite clasificarla como una herramienta válida para la identificación y clasificación de la pérdida auditiva en población infantil colombiana. La prevalencia de hipoacusia encontrada fue del 23.1%
Objetivo: determinar los niveles de presión sonora (NPS) en un Centro de Desarrollo Infantil (CDI) de Popayán –Cauca. Métodos: estudio descriptivo, transversal en una población objetivo de 341 sujetos. Se evaluó los NPS en diez aulas académicas, el área administrativa, la ludoteca, el comedor, el pasillo, el jardín social, el patio de juegos y el parqueadero. La medición se llevó a cabo durante una semana, en varios momentos como la hora de llegada, hora de salida, hora de desayuno, hora de almuerzo y en el descanso de los niños. Para la medición del NPS se utilizó un sonómetro digital calibrado (Sound Meter). Resultados: El día con mayor NPS fue el martes con 77 dB, seguido del día viernes con 75. El aula con mayor NPS (82 dB) fue Jardín C, mientras que las aulas de Sala cuna y Párvulos B, presentaron NPS más bajos (72 dB). El lugar con menos NPS fue la ludoteca con 59 dB, contrario al comedor con 80 dB. Los NPS más altos se presentaron en la hora de llegada de los niños al CDI, la hora de salida y la hora del almuerzo con un promedio de 83 dB. Conclusiones: Los NPS encontrados superan los límites permitidos según la normatividad colombiana y de no tomarse medidas preventivas, pueden convertirse en un riesgo para la audición e incidir en el adecuado desarrollo del lenguaje y en el desempeño escolar.
Objetivo: Describir las tres técnicas quirúrgicas más utilizadas en el manejo del Hematoma Subdural Crónico (HSDC). Metodología: Estudio descriptivo de corte transversal realizado en 63 pacientes con diagnóstico de HSDC intervenidos quirúrgicamente mediante las técnicas trepano-drill, craneotomía con trefina y Craneotomía convencional, desde enero de 2013 hasta febrero de 2016 en una Institución Hospitalaria de la Ciudad de Popayán. Los datos obtenidos fueron analizados mediante el software de estadística SPSS-19. Resultados: De las tres técnicas utilizadas, la Trepano-Drill fue la más común (84,1%). Además, los pacientes intervenidos mediante esta técnica presentaron una menor cantidad de complicaciones intrahospitalarias (26,4%), su estado neurológico fue normal en el 77,4 % y solo el 34% requirió de Unidad de Cuidados Intensivos. Conclusión: No se encontró diferencia estadísticamente significativa entre las tres técnicas utilizadas. Sin embargo, el drenaje del hematoma subdural crónico utilizando la técnica quirúrgica trepano-drill, mostró mayores ventajas sobre las otras dos técnicas evaluadas: craneotomía con trefina y craneotomía convencional.
