A esquizofrenia continua suscitando discussões, às vezes polêmicas, sobre a definição do próprio conceito e das suas implicações, ou ainda sobre a sua etiologia. Todavia, percebe-se hoje em dia um certo consenso sobre a pluridimensionalidade das causas que, numa interação complexa e ainda enigmática, produzem esta psicose. Este consenso geral é comprovado pela tendência de falar cada vez mais em termos de etiopatogenia e não mais de "causas" lineares ou unilaterais.Na pluralidade dos fatores que intervém nesta etiopatogenia da esquizofrenia, podemos distinguir globalmente dois campos, que recebem a preferência dos pesquisadores em função das premissas e interesses de cada um. De um lado, temos a abordagem que se orienta mais nas ciências exatas, nas áreas da bio-química, da neuro-fisiologia e da genética; de outro, os que se inspiram das ciências humanas. Aqui se destacam principalmente quatro dimensões: A família e sua influência, com ênfase na aprendizagem ou na comunicação; o social (p. ex. a noção de "papel social") e os conflitos interpessoais; a análise dos conflitos intra-pessoais, numa visão psico-dinâmica, e finalmente a problemática existencial do esquizofrênico.As oposições entre estes dois tipos de abordagem, se eles ainda existem, já não são mais da mesma veemência e intransigência como ocorria ainda alguns anos atrás: a impossibilidade de se chegar a uma definição causai restrita obrigou a alargar os horizontes de cada um. Os dois campos se encontram por exemplo quando se trata de discutir a influência da her editar iedade. Tradicionalmente, esta era considerada como área própria da biologia genética; as pesquisas das últimas décadas, estudando sobretudo a família do esquizofrênico, mostraram que a transmissão de certos traços familiares não é necessariamente ligada à bagagem genética, mas pode ser efetuada por outros veículos, como a pressão do grupo, a linguagem, as expectativas inconscientes, os desejos de dominação ou de vingança. Di Lascio (1975) 4 lembra, aliás, que devem ser contados, entre os elementos psico-sociais também os que decorrem do contexto geográfico e cultural e que participam ativamente no espectro etiopatogênico da esquizofrenia.Reiss (1976) 12 , por sua vez, falando dos numerosos trabalhos sobre a família (e a mãe) do esquizofrênico, sublinha possibilidade de integrar os
