Background Elderly in need of long-term care tend to have worse health and have higher need of medical care than elderly without need for long-term care. Yet, characteristics associated with long-term care need can impede health care access: Higher levels of long-term care need come with physical and cognitive decline such as frailty and memory loss. Yet, it has not been investigated whether level of long-term care need is related to medical care utilization. Methods We investigated the association between the level of long-term care and medical specialist utilization among nursing home residents and home care recipients. We applied zero-inflated Poisson regression with robust standard errors based on a sample of statutory health insurance members. The sample consisted of 100.000 elderly over age 60. We controlled for age, gender, morbidity and mortality, residential density, and general practitioner utilization. Results We found a strong gradient effect of the level of long-term care for 9 out of 12 medical specialties: A higher level of long-term care need was associated with a lower probability of having a medical specialist visit. Yet, we did not find clear effects of the level of long-term care need on the intensity of medical specialist care. These findings were similar for both the nursing home and home care setting. Conclusion The findings indicate that inequalities in medical specialist utilization exist between elderly with differing levels of long-term care need because differences in morbidity were controlled for. Elderly with higher need of long-term care might face more access barriers to specialist medical care.
In Kontrast zu 780.000 hochbetagten und multimorbiden Menschen in deutschen Pflegeheimen steht die umfassend untersuchte Fehlversorgung in diesen Einrichtungen. Der Optimierungsbedarf zur Versorgungstransparenz und-qualität ist erheblich. Ausgehend von der These, dass ein multidimensionaler, sektoren-und berufsübergreifender Blick auf die Versorgungsqualität im Pflegeheim erforderlich ist, testeten die Autoren erstmals in Deutschland die Nutzung von Routinedaten der Kranken-und Pflegekassen (AOK) für diesen Kontext. Hierfür wurden insgesamt sechs Kennzahlen zur Arzneimittelversorgung, zu nosokomialen Erkrankungen, zu Hospitalisierungen sowie zur haus-und fachärztlichen Versorgung im Pflegeheim entwickelt und empirisch getestet. Im Ergebnis zeigt sich, dass die Operationalisierung routinedatenbasierter Qualitätskennzahlen für das stationäre Pflegesetting machbar ist. Auch die zum Teil erheblichen Versorgungsunterschiede zwischen den Pflegeheimen werden transparent gemacht. Auf der Agenda stehen nun die methodische Schärfung und Risikoadjustierung, damit langfristig routinedatenbasierte Qualitätsindikatoren die Entwicklung der pflegeheiminternen Qualität und die Versorgungstransparenz für die Pflegekassen empirisch unterstützen können. In contrast to 780,000 elderly and multimorbid people in German nursing homes, there is an extensively analysed poor care in these facilities. The need for an improved transparency and quality of care is considerable. Based on the assumption that a multidimensional, cross-sectoral and cross-occupational view of the quality of care in nursing homes is called for, the authors tested the use of routine data from health and nursing insurance funds (AOK) for the first time in Germany for this context. For this purpose, a total of six key data on provision of medicines, nosocomial diseases, hospitalisations and general and specialist care in nursing homes were developed and empirically tested. The results show that the operationalisation of routine data-based quality indicators for inpatient care setting is feasible. The sometimes considerable differences in care between nursing homes are also made transparent. Methodological sharpening and risk adjustment are now on the agenda so that routine data-based quality indicators can empirically support the development of nursing home quality and provide transparency for the nursing care insurance funds in the long run.

Zusammenfassung Die Mehrzahl der Pflegebedürftigen wird auch heute in der eigenen Häuslichkeit unter Einbindung unterschiedlicher Hilfen versorgt. Mit Hilfe einer Online-Befragung wurden rund 1.100 Hauptpflegepersonen zu den aufgebrachten finanziellen und zeitlichen Eigenleistungen befragt. Die Analysen zeigen auf, dass die ambulante Pflege maßgeblich durch die Hauptpflegeperson und weitere in die Pflege eingebundene Personen getragen wird. Es zeigt sich ein deutlicher Zusammenhang zwischen Belastungsfaktoren bezogen auf das Vorliegen einer Demenz sowie der Pflegeschwere und der selbst geleisteten Pflege und Betreuung. Privat aufgewendete finanzielle Mittel werden nur von jedem Vierten der Befragten angegeben. Diese privaten Aufwendungen sind ebenfalls bei Haushalten mit demenziell Erkrankten überdurchschnittlich hoch und steigen mit dem Pflegegrad an. Einkommen, Bildung oder Erwerbstätigkeit hingegen haben keinen signifikanten Einfluss auf die privat aufgewendeten finanziellen Mittel. Wenn auch die Mehrzahl der Pflegehaushalte die Situation bewältigen kann, zeigen bis zu einem Viertel der Haushalte hohe Belastungswerte. Die Erhebung rückt damit Fragen nach gezielter Unterstützung und einem differenzierten Leistungszuschnitt in den Fokus.
